Directs and supervises physical therapy assistants, students, and other support personnel. Assesses competence, delegates specific tasks, and establishes channels of communication. Administers treatment involving application of physical agents, using equipment, moist packs, ultraviolet and infrared lamps, and ultrasound machines. Evaluates, fits, and adjusts prosthetic and orthotic devices and recommend modifications. Identifies and documents goals, anticipated progress and plans for reevaluation. Develops and implements the individual treatment plan and coordinates service delivery to children and their families utilizing trauma informed approaches. This includes the completion of assessment, and networking with referral sources, parents and other community resources. 2. Provides individual/family therapy at a minimum of 1 hr. per week, per individual and based on current needs and treatment plans of each child. Facilitate therapeutic groups. Engage family in services and provide parent education information and work to support referral agent's permanency plan and/or stability plan. 3. Maintains current records on each child in accordance with agency policies and state regulations. 4. Communicate and collaborate with families, referral agents, and other interested parties, (i.e., judges, schools, DCBS, DJJ, and other referral agents) regarding children's treatment. Residential - Co-chairs team meetings, admission conferences, staffings, in-service training and other meetings as required. Provides current clinical updates regarding children and families on their caseload and documents weekly and quarterly treatment meetings. 5. Residential - Reports observations regarding client medical issues to agency's Medication Coordinator. 6. Residential - Participates in psychiatric and psychological consults regarding residents assigned to caseloads. 7. Residential - Participates in admission process by reviewing referral packet and interviewing the resident and his family. 8. Residential - Collaborates with Case Managers to ensure resident's needs are addressed. 9. Residential - Collaborates with referral source for transition and permanency planning. 10. Develop discharge and transition plan for assigned caseloads. 11. Attend case consultation and clinical meetings. 12. Assists with case coverage across programs as needed. 13. Obtain and maintain training in agency evidence-based practices and treatment modalities which may include: Nurturing Parenting, Trauma Focused CBT, Impact Plus provider certification, JSOCCP certification, Seeking Safety, Seven Challenges and Trauma Informed Care. Working Conditions While performing the duties of this job, the employee is regularly required to stand, walk and talk or hear; use hands to finger, handle or feel objects; and reach with hands and arms. The employee is occasionally required to sit, kneel, squat, and hold objects or carry.
