Primary Job Responsibilities/Duties: The Case Manager - Mental Health is responsible for, but not limited to: Conduct initial intake assessments and act upon critical needs appropriately Utilizes assessment tools (CARES) to identify support service needs and make immediate referrals Orient clients to the shelter program, requirements and consequences Provide client with Independent Living Plan (ILP) (CARES), working in collaboration with other Social Service providers and ensure the delivery of comprehensive support services. Assist non-working clients to obtain Public Assistance Complete psycho-social assessments within the first 30 days of intake Track and monitor client progress Empower clients to become involved in their own planning and goal setting Conduct weekly/bi-weekly meetings with clients, or as needed Refer clients and encourage job development for subsequent employment if appropriate Refer clients to appropriate resources to assist with set goals Meet with clients to address issues, challenges and successes to assist in strengthening their capacity for long-term self-sufficiency Maintain client files to include conversations, warnings and progress reports and documenting all incidents Maintain confidential case record for all clients Report critical incidents immediately to the Site Director/ Director of Social Services Work collaboratively with other Housing Specialist and Employment staff to identify client readiness for employment and/or housing Treat all clients, visitors and employees with kindness, respect and dignity Report critical incidents immediately to the Director/Supervisor of Social Services Refer clients to internal and external resources and serve as an advocate when necessary Provide case management and counseling periodically; one year beyond the client's departure Develop and maintain connections with other programs and units within the organization to facilitate inter-agency referrals Responsible for escorting clients to housing viewings, DHS or HRA appointments Responsible for placing clients into suitable housing Case notes must be completed within 24 hours after meeting with clients Substitutes for other staff when need arises Perform general clerical duties Physical Requirements: Prolonged periods of sitting at a desk and working on a computer. Ability to sit for extended periods and perform repetitive tasks. Must be able to lift and carry up to 5 pounds. Must be able to travel to multiple NYC sites as needed. Must be able to access and navigate each department at the organization's facilities. Ability to climb stairs. Work Environment / Schedule Requirements: Office setting with regular exposure to computer screens and moderate noise levels. May be required to work shifts, including evenings or night tours and/or Saturdays, Sundays and holidays with holiday pay differential. Transitional Housing Program staff are required to continue their shift until relieved by another staff member or authorized to leave by a supervisor. Occasional evenings, weekends, and overnights with long-distance travel (annual overnight or multi-night youth retreat, college trips, GK events). Administrative Responsibilities Collaborate with school personnel on logistics for program implementation (e.g., space, security needs, permits, trip forms/permission slips). Collect, input, and review attendance, evaluation, and other data related to projects and student progress. Maintain necessary documentation/case files, as well as program data entry and records. Complete administrative tasks related to maintaining assigned GK equipment, fiscal procedures (fund requests and expense reports) and other tasks. Qualifications: Understanding of, and willingness to learn about, a wide array of NYC social service resources for youth and families. Experience using interactive experiential learning strategies, counseling and conflict resolution skills, and youth development/leadership strategies with middle school and/or high school-age youth. Experience and enthusiasm for designing and leading educational workshops that build on each other. Commitment to and understanding of working in culturally diverse settings and using antiracist pedagogical practices Experience working in or closely with NYC public schools in schools a plus. Knowledge of and passion for global issues, political science, history, education and social justice, and human rights. Excellent facilitation, consensus-building, and interpersonal skills. Initiative, creativity, willingness to learn, and ability to manage multiple tasks independently. Strong communication, writing, organizational, and technology skills (Microsoft Suite, Google Suite, Zoom). International experience is a plus Fluency in other languages is a plus (particularly Spanish, Haitian Creole, Arabic, and South Asian languages). Salary/Benefits: This full-time position comes with an annual salary of $45,000 - 47,000.
