Responsibilities of the Licensed Therapist
Provides direct patient care for Wayspring patients and care collaboration for complex patients with multiple needs, determines the best strategy and support for the patient to encourage engagement (Clinical Care Navigators, Psychiatric NPs and Primary Care NPs).
Continues or conducts initial assessments for new patients
Creates individualized treatment plans according to patient needs and circumstances
Meets with patients regularly to talk about treatment plan, track progress on treatment plans, and adjusts treatment plans as necessary
Conducts ongoing screenings of patient progress
Complies with documentation standards within clinic documentation platform, Athena (EMR) to ensure regulatory compliance

Ensures that Wayspring's Clinical Care Navigators understand and adhere to patients' treatment plans and act as a clinical resource as needed.

Requirements and Preferred Qualifications
Master's degree in eligible field (social work, psychology, counseling) required
Active state licensure
Willingness to obtain additional state licensure as requested (Wayspring covers cost associated)

NYAP is seeking a contract Therapist for Washington County, Kentucky.
The person in this role will need to meet the state mandated therapy of 2 - 3 hours per month per youth in their area at a schedule convenient to them and foster family.
The ideal candidate would have a private practice (fully licensed) in that county, have experience treating youth with trauma, and be willing to become a NYAP contractor and accept our referrals.
Pay: $100 per hour of required therapy ( 2- 3 x's per month based on level of youth).
Position summary
Under the general supervision of the Clinical Supervisor or designee, this position works to make a meaningful difference in the lives of the children and families served. This position works closely with youth, family, foster parents, and community partners, providing counseling or therapy, advocacy, and support toward the ultimate goal of living safely and successfully in the community.
Responsibilities
Actively supports, represents, and extends the mission, vision, and values of the organization.
Provides appropriate individual, group, and family mental health services, case management, transportation, and other professional services, meeting or exceeding established outcomes thresholds on behalf of persons served.
Travels daily, to provide community based services to, and on behalf of, youth and families in compliance with organizational, contract, and regulatory requirements.

Participates in the Continuous Quality Improvement activities on a monthly/quarterly basis.
Actively participates in the weekly supervision process.
Minimum Qualifications
Master's Degree in Social Work or comparable Human Services field from an accredited institution.
2 years of work experience working in direct service with youth and families strongly preferred.
Must have a valid driver's license, reliable transportation, automotive insurance, and a good driving record.
A willingness to work flexible and non-traditional hours in the service of foster caregivers, families of origin, and persons served.

This position is responsible for providing counseling services for individuals diagnosed with substance use disorders and co-occurring mental health diagnosis. This position maintains a client caseload, following established treatment guidelines and agency policies and procedures. This position provides therapeutic interventions utilizing a variety of treatment modalities, with emphasis on substance use issues, individually matched to the needs of each client served. This position will report to the Clinical Director.
JOB SPECIFIC FUNCTIONS:
Complete clinical assessments, treatment planning, consultation, and appropriate referrals according to the individual's professional discipline and education. Services shall be provided in accordance with the established Kentucky mental health and substance abuse standards as well as within the scope of practice of the individual.
In conjunction with Clinical Supervisor ensure adherence to the required HIPAA and documentation requirements as identified by county, state, and federal regulatory agencies.
Conduct psychosocial evaluations and provide therapeutic interventions including crisis counseling to individuals, groups and families as needed to facilitate.
Maintain personal professional liability insurance.
Attend trainings for all clinical and program staff on Mental Health and AODE and other clinical topics relevant to services provided by Isaiah House Inc.
Participate with team case conference meetings on a weekly basis to ensure coordinated implementation of treatment plans and service utilization such that the comprehensive care of clients reflects the philosophy and objectives of Isaiah House Inc.
Create safety plans, and assess client risk level through utilization of clinical screeners.
Perform a semiannual self-evaluation.

Be well versed in all company policies, procedures, and rules.
