Position Summary:

Key duties and Responsibilities:
Develops a plan of care for each patient with the patient's input and agreement, which includes safety, educational, psychological and developmental needs of the patient.
Interprets and assesses data about the patient as evidenced by appropriate goal setting and revision of goals throughout the plan care while explaining treatment goals, plans, techniques and outcomes to patient and or family.
Displays expertise in recommending various therapeutic procedures and modalities.
Complies with professional and departmental documentation standards.

Attends continuing education seminars as required by licensure and schedules and presents an educational program, including tests, to Rehab staff or other medical professionals.
Communicates regularly with Rehabilitation Supervisor, other team members and referring physicians for patient care.

**ROLE AND RESPONSIBILITIES**

+ Completes all documentation required by state regulations, Joint Commission standards, and CHC Behavioral Health department by laws in a timely manner, included by not limited to intake, prior authorization request, progress notes, treatment plan documentation, and discharge.
+ Completes and locks all treatment notes no later than the end of the next business day.
+ Completes reports as required for outside agencies such as DCF, Family Relations, probation, and others with appropriate releases signed by clients.
+ Participates as a member of agency wide committees as assigned.
**QUALIFICATIONS**
+ Licensure / Certification / Registration: LCSW, LMFT, LPC, or Licensed as Clinical Psychologist in State of CT. preferred, or may be license eligible with required current Associate license (MLSW, PC-A, MFT-A) with ability to obtain full clinical licensure within 3 years of hire. Professional clinical licensure (LCSW, LPC, LMFT) preferred.
+ Minimum of two years of experience with outpatients including experience with children and/or adults.

GENERAL DUTIES AND RESPONSIBILITIES

Formulate psychosocial evaluations, diagnoses, and treatment plans with supervisory support. Demonstrate an increasing comprehension of the dynamics of human behavior, family systems, substance abuse, co-occurring disorders and appropriate intervention techniques. Provide treatment services appropriate to the personal and cultural identity and language of the client. Consistently evaluates and treats co- occurring disorders with using a standardized co-occurring assessment tool focusing on both substance abuse and mental health disorders Provides specific documentation of history and chronology of both mental health and substance abuse disorders; Treatment plans routinely address mental health and substance abuse disorders equivalently and in specific detail Treatment interventions in addition to medications are used to address mental health disorders. Provide integrated evidence based practice to clients as evidenced by supervisor observation, case presentations and audio or video taped sessions with clients. Determine the client's readiness for treatment and change as well as the needs of others involved in the current situation. Formulate mutually agreed-on and measurable treatment goals and objectives. Carry out appropriate treatment plans, according to agency best practice standards, and evaluate the progress throughout treatment and at termination. QUALIFICATIONS
State of Connecticut Professional License related to behavioral health (LCSW, LPC, LMFT or Licensed Psychologist), master's degree in the human services field and two years post graduate work. Job purpose
Responsible for providing behavioral (BH) health care to children and adolescents in an integrated community health clinic. Will provide evaluation, diagnosis, and treatment to children and their families. Will provide treatment including individual, and/or group therapy and brief substance abuse counseling for children and adolescents, and/or family therapy for families, at FHCHC, or will provide appropriate
referrals to other community child and adolescent BH providers.
Duties and responsibilities
The Child/Adolescent Behavioral Health Clinician for Integrated Care is a liaison to pediatricians in a preeminent academic medical center. Typical duties include but are not limited to:
Facilitate warm handoffs to pediatricians
Provide mental and BH treatment to children and adolescents in an integrated community healthcare clinic
Evaluate and diagnose mental, emotional, developmental and behavioral difficulties in children, as well as family dysfunction based on clinical standards of practice
Work in close partnership with the primary care clinical teams
Conduct patient screening using standardized tools
Formulate treatment plans and reviews
Provide treatment, including individual, group, and/or family therapy, and some substance abuse counseling when necessary
Provide acute triage services to the primary care clinical teams
Report to DCF when necessary if neglect or abuse is suspected
Provide clinical services that are strength based and client centered to children and their families.
Coordinate with other clinical and social service providers, care coordinators, and patient navigators
Document all aspects of clinical care in the EHR and in a timely manner
Participate in professional activities such as: continuing education activities, clinical conferences, meetings with representatives of community groups when required to do so
Ensure patient confidentiality at all times
Perform other necessary duties as required by FHCHC to achieve the goal of providing high quality health care
Qualifications
Master's degree (M.A., M.F.T., M.S.W.) required. Must have an independent license in the state of CT i.e., LMFT, LPC, LCSW, or an associate license LPCA, MFTA, LMSW or Psychologist (PhD or PsyD). One year of behavioral health experience, and oral and written fluency in both Spanish and English required.
The successful candidate will have:
Experience working with children, adolescents, and families impacted by ASD or other developmental disabilities is preferred.
Strong understanding of autism and its impact on development, as well as the ability to adapt therapy methods to individual needs.
Training, knowledge and ability to independently carry out all the above duties
Experience in providing clinical services to children and adolescents
Strong knowledge base of medical terminology/procedures for work in an integrated healthcare clinic desired
Previous experience in health care setting, ideally in an integrated outpatient setting desired
Ability to work in a high energy, fast paced environment and maintain a professional demeanor and attitude
Excellent interpersonal and communication skills with patients, their families, internal and external staff
