The Alignment Care Anywhere program is a physician-led and advanced practice clinician-driven program that is intended to care for and support Alignment Healthcare patients by proactively providing an additional level of medical and social support in the comfort of their home. In this role, you will conduct in-home health assessments on a panel of enrolled Medicare Advantage members within a geographic area of responsibility. Our members are typically aged 65 and over and home visits will range from 30 minutes to 60 minutes depending on whether it is a follow-up or a comprehensive initial visit. The nature of the home visit will vary and visit frequency will be based on the member's acuity level (i.e., weekly, bi-weekly, monthly, etc.). Assists in establishing client's eligibility for Medi-Cal or other benefits and advocates for continuation of benefits when appropriate.
Develops financial management plans for clients who receive Representative Payee Services and authorizes appropriate expenditures.
Interacts productively within the team in order to provide holistic case management services to an identified group of clients.
Provides “on-the-spot” counseling that is both helpful to the clients and consistent with the philosophy of the program.
Drives on program business using personal vehicle for the purposes of medication outreach, home visits, transporting clients to appointments/activities, community/county meetings/trainings, MH court.
Attends to the safety, health, and wellbeing of clients.
Completes paperwork as assigned in a timely manner.

Education, Training and Experience:

Using a variety of trauma informed techniques, the Youth Stabilization Specialists provide crisis prevention and de-escalation services to youth ages 12-18 and actively engage youth in activities of daily living including cooking, cleaning, personal hygiene, and transportation. Job Qualifications:
Youth Stabilization Specialists, according to state licensing requirements, must meet one of the following seven qualifications:

Duties and Responsibilities:
In collaboration with the psychiatrist and other licensed members of the IDT, the Licensed Behavioral Health Clinician:
Identifies participants in need of clinical services based on acuity/need across all centers.

Trains and supports staff to increase competencies in serving participants with behavioral health needs.

Assess potential participants interested in CEI with significant behavioral or mental health backgrounds for appropriateness to PACE based on standardized health and safety criteria. Make recommendations for approval or denial of enrollment accordingly.

Qualifications:
Master's degree in Social Work, Marriage and Family Therapy, or Professional Clinical Counselor (LCSW, LMFT, or LPCC license required).
2 years' experience providing diagnostic assessment, treatment planning, crisis intervention and psychotherapy required.
One year experience working with a frail and/or elderly population with Health Psychology model preferred.
Demonstrated ability to work in a multicultural and multidisciplinary team environment.
Ability to work independently.

THE POSITION

Sick Leave: 12 working days of sick leave annually with unlimited accumulation.
Sick leave incentive
: An employee is eligible to receive eight hours administrative leave if the leave balance equals at least one- half of the cumulative amount that the employee is eligible to accrue. The employee must also be on payroll during the entire calendar year.
Bereavement Leave: 3 days of paid leave for the death of qualifying family member, 2 additional days of accrued leave for death of employee's spouse, domestic partner, parent or child.
Merit Salary Increase: New employees will receive the starting salary, which is the first step of the salary range. After employees serve 52 weeks (2080 hours) on each step of the range, they are eligible for a merit increase to the next step.
Job Sharing: Employees may agree to job-share a position, subject to approval by a Department Head and the Director of Human Resources.
Educational Reimbursement Program: Eligible employees may be reimbursed for career-related course work up to a maximum of $850 per fiscal year. Eligible employees enrolled in an approved four (4) year College or University academic program may be reimbursed up to $800 per semester for a maximum of $1600 per fiscal year.
Parking Supplemental Downtown Stockton: The County contributes up to $20 per pay period for employees who pay for parking and are assigned to work in the Downtown Core Area.
School Activities: Employees may take up to 40 hours per year, but not more than eight (8) hours per month, to participate in their children's school activities.
Selection Procedures
Applicants who meet the minimum qualifications will go through the following examination process:
Written Exam: The civil service written exam is a multiple choice format. If the written exam is administered alone, it will be 100% of the overall score. Candidates must achieve a minimum rating of 70% in order to be placed on the eligible list.
Oral Exam: The oral exam is a structured interview process that will assess the candidate's education, training, and experience and may include a practical exercise. The oral exam selection process is not a hiring interview. A panel of up to four people will determine the candidate's score and rank for placement on the eligible list. Top candidates from the eligible list are referred for hiring interviews. If the oral exam is administered alone, it will be 100% of the overall score. Candidates must achieve a minimum rating of 70% in order to be placed on the eligible list.
Written & Oral Exam: If both a written exam and an oral exam is administered, the written exam is weighted at 60% and the oral exam is weighted at 40% unless otherwise indicated on the announcement. Candidates must achieve a minimum rating of 70% on each examination in order to be placed on the eligible list.
Rate-out: A rate-out is an examination that involves a paper rating of the candidate's application using the following criteria: education, training, and experience. Candidates will not be scheduled for the rate-out process.
Note: The rating of 70 referred to may be the same or other than an arithmetic 70% of the total possible points.
Testing Accommodation: Candidates who require testing accommodation under the Americans with Disabilities Act (ADA) must call Human Resources Division at ************** prior to the examination date.
Veteran's Points: Eligible veterans, unmarried widows and widowers of veterans of the United States Armed Forces who have been honorably discharged and who have served during wartime shall be given veteran's points in initial appointment to County service. Eligible Lists: Candidates who pass the examination will be placed on an eligible list for that classification. Eligible lists are effective for nine months, but may be extended by the Human Resources Director for a period which shall not exceed a total of three years from the date established.
Certification/Referral: Names from the eligible list will be referred to the hiring department by the following methods.
Rule of the Rank: The top rank or ranks of eligibles will be referred for hiring interviews. The minimum number of names to be referred will be equal to the number of positions plus nine, or 10% of the eligible list, whichever is higher. When filling nine or more positions in a department at the same time, the top rank or ranks will be referred and the minimum number of names shall be two times the number of positions to be filled or 10% of the eligible list, whichever is higher. Pre-Employment Drug Screening Exam: Some classifications require a new employee successfully pass a pre-employment drug screen as a condition of employment. Final appointment cannot be made until the eligible has passed the drug screen. The County pays for the initial drug screen.

Under supervision of the Program Manager, the Mental Health Clinician provides mental health assessment, treatment, and case management services to justice-involved adults under California AB 109 Public Safety Realignment. The AB109 Mental Health Clinician works collaboratively with probation, community partners, and treatment providers to support client stabilization, reduce recidivism, and promote successful community reintegration.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Adheres to El Concilio California policies and procedures per the agency employee manual.
Clinical Services
Conduct intake assessments for AB 109 clients, including criminogenic risk, mental health needs, substance use, housing stability, and functional impairments.
Provide individual, family, and group psychotherapy using evidence-based practices.
Develop, monitor, and update treatment plans that reflect client goals and clinical needs.
Complete court-, probation-, and grant-required assessments and reports within mandated timelines.
Respond to crises and provide risk assessments, safety planning, and crisis intervention as needed.
Deliver evidence-based interventions targeted at reducing recidivism, such as Cognitive Behavioral Therapy (CBT), Motivational Interviewing (MI), Moral Reconation Therapy (MRT), and trauma-informed care
Documentation & Compliance
Complete timely, accurate clinical documentation in accordance with agency policy, funding requirements, and legal/ethical standards.
Maintain confidentiality and comply with HIPAA and state regulations.
Adhere to confidentiality laws while appropriately sharing information with justice partners under signed releases.
Participate in quality assurance activities, audits, and utilization reviews.
Follow safety guidelines when working in field-based, probation, or community settings.
Collaboration & Coordination
Participate in multidisciplinary team meetings (MDTs) with probation, reentry staff, substance use providers, housing coordinators, and community partners.
Attend multidisciplinary team meetings, case consultations, and clinical supervision.
Collaborate closely with Probation Officers to support client compliance with supervision conditions and treatment mandates.
Provide referrals and linkage to community-based mental health, medical, housing, and vocational services.
Conduct outreach and engagement for hard-to-reach or resistant AB 109 clients.
Maintain professional boundaries and uphold client confidentiality at all times.
Required to work at El Concilio California for special events and meetings.
Other duties as assigned by immediate supervisor and administration.
