DEPARTMENT DESCRIPTION
The Rehabilitation teams at Samaritan evaluate and treat patients with injuries or conditions that have caused either a decline in physical mobility or functional independence. In addition to acute and swing bed rehab, we offer a variety of outpatient services including sports and orthopedic rehab, pediatric rehab, neurological and stroke rehab, balance and dizziness rehab, wheelchair seating, and neck, back and extremity care. At SNLH, we have a large and well equipped outpatient therapy department located within a 25 bed critical access hospital. Enjoy a dynamic and supportive therapy environment, professional camaraderie and 45 minute treatment times.

Current unencumbered Oregon Physical Therapy license required.
Healthcare Provider Level BLS required.

Clinical Psychologist Requirements:

Qualifications

About the Position
We have an exciting career opportunity for an Intensive Supportive Housing Therapist who possesses a combination of skills and QMHP eligibility to provide clinical services for our Southern Oregon supportive housing programs in Medford, Oregon. The Intensive Supportive Housing Therapist position requires the ability to conduct mental status examinations and DSM-5 diagnoses. They will conduct comprehensive psychosocial mental health assessments and complete assessment reviews and updates every six months or as clinically indicated. The Intensive Supportive Housing Therapist will perform certain mental health functions, such as consultation, crisis management, and individual and group therapy, or delegate these functions to appropriately qualified clinical staff. They will work collaboratively with community agencies to ensure appropriate admission to the program and provide warm hand-off to behavioral health services aftercare.

Swing Lane Supportive Housing is an exciting, newly constructed housing program in Medford, Oregon that provides permanent, intensive Supportive Housing, as well as transitional Room & Board units for adults who have a serious mental illness.
Rogue Ridge Apartments is a brand-new, 28-unit, permanent Supportive Housing program in Ashland, Oregon. Rogue Ridge serves adults who are houseless and living with a mental illness. The program is in a beautiful three-story apartment complex with elevators, ADA units, and a community/activity room for residents to enjoy. Supportive services include on-site staff who provide peer support, skills training, tenancy assistance, community navigation, and more; 24/7 access to housing support; and tenancy services and support around move in and out, rent collection, maintenance, and repairs.
ColumbiaCare Services is a qualifying employer for the Public Service Loan Forgiveness (PSLF) Program.
What You'll Need
Must be eligible for Qualified Mental Health Professional (QMHP) registration. Eligibility requires:
Master's Degree in Psychology, Counseling, Social Work, or related field Unlicensed Therapists MUST be actively seeking or willing to obtain licensure.

Key Responsibilities
Provide outpatient behavioral health services in a Rural Health Clinic setting.
Collaborate closely with primary care providers as part of an integrated behavioral health model.
Maintain manageable caseloads, averaging 8-10 patients per day.
Participate in a light, phone-only call rotation (1:8).
Deliver compassionate care using Epic EMR for documentation and communication.
Qualifications
Master's degree in Social Work (MSW) required.
Current LCSW license (or eligibility for Oregon licensure).
Previous experience in integrated or rural behavioral health settings preferred.
Strong interpersonal and communication skills with a collaborative mindset.
Schedule
Four 10-hour days per week (no weekends).
Full-time, permanent position based in La Grande, Oregon 97850.

About the Position
We are looking for a Residential Treatment Therapist to join our team for our Southern Oregon region. The Residential Treatment Therapist will serve as a role model through the provision of recovery-oriented interventions to promote and nurture a healthy treatment culture. In collaboration with the Clinical Manager and Residential Therapist, they will provide individual and group treatment services such as, but not limited to, skills training, activity therapy, group therapy, and case management as indicated in the treatment plan.
The Residential Treatment Therapist will develop the competency necessary to perform comprehensive mental health assessments, including diagnosis and mental status exam. They will also develop treatment plans which center individuals' stated goals, address needs identified in the assessment, and prescribe appropriate interventions. The Residential Treatment Therapist will document treatment interventions provided, including rationale for services and progress towards intended outcome. They will also complete all documentation, including assessments, treatment plans, and individual service notes in a timely fashion in adherence with Federal standards.

Work Schedule: Monday through Friday 8:30am - 5:00pm (Full Time, Day). This position requires flexibility in schedule based on regional need.

What You'll Need
Must be eligible for Qualified Mental Health Professional (QMHP) registration. Eligibility requires:Master's Degree in Psychology, Counseling, Social Work, or related field Unlicensed Therapists MUST be actively seeking or willing to obtain licensure.
If not already registered as a QMHP through the Mental Health & Addiction Certification Board of Oregon (MHACBO), we will assist you in registering within 30 days of hire.
Driving is an essential function of this position. Successful applicants must provide a valid driver's license from their state of residence or obtain and submit one within 30 days of hire, maintain a safe driving record, and have the ability to pass a DHS criminal background check.
Physical Requirements: This position requires the ability to frequently sit, talk, listen, and use hands and fingers. It may require the ability to occasionally walk, climb stairs and ladders, bend, stoop, squat/kneel, and perform other physical tasks as applicable; as well as lift, carry, push, and pull up to 20 pounds.
