This position works under the support and direction of the Area/Regional VP, Operations and onsite Program Directors to enhance the academic, social, emotional, and behavioral outcomes of students through use of evidence-based interventions and in collaboration with the operations and educational staff from each campus within assigned area. ‖ Responsibilities Include: Delivering comprehensive training and continuous guidance to teachers and educational staff in designing impactful Functional Behavior Assessments (FBAs) and Behavior Intervention Plans (BIPs). Monitoring and evaluating the effectiveness and quality of academic instruction and behavioral support services provided to students, ensuring exceptional outcomes. Supporting Spectrum staff by modeling, reinforcing, and enhancing instructional best practices to promote student growth aligned with Individualized Education Plan (IEP) goals. Implementing evidence-based interventions by applying scientifically proven behavioral and educational strategies, promoting positive outcomes in academic, social, emotional, and independent functioning. Providing crisis management intervention and support for students exhibiting aggressive or self-injurious behavior and developing strategies to reduce the need for 1:1 staffing. Collaborating closely with on-site education teams to collect and analyze data on student behavior, monitor student progress, and ensure appropriate intervention responsiveness and consistent progress toward goals. Conducting regular quality checks for each campus within assigned region at the student, staff, classroom, and site levels; making recommendations for improvement based on analysis of data. Licensed currently or in the process of obtaining a board-certified behavior analyst (BCBA) credential. This program is expanding the locations where it provides patients with medical and social support when and where they need it, with the goal of preventing unnecessary hospitalizations, health complications, and unmanaged disease progression that can occur when timely clinical interventions are not provided or are not accessible. Our home-based programs are offered to eligible patients at no cost to them. Immediate opening for a Nurse Practitioner or Physician Assistant to travel to local skilled nursing facilities. You would see approximately 10-15 patients a day depending on the daily census. Position Summary: An exempt clinical position where the nurse practitioner (NP) or Physician Assistant (PA) is responsible for providing direct patient care. The NP participates as a leader of the skilled nursing facility (SNF) care team. Visits Alignment members/patients at skilled and long-term levels of care in designated SNFs facilities. Provides appropriate evidence-based geriatric medicine. Coordinates care with hospitalists, primary care physicians and care managers. Makes home visits as directed by the medical staff to meet patient needs and provide continuity of care. The primary responsibilities of the Admissions Clinician include providing direct participant care in a manner that exemplifies HealthRIGHT 360's philosophy, goals, policies, mission and vision through the provision of all aspects of residential entry services. Admissions Clinicians conduct level of care assessments, treatment planning, determining medical necessity and evaluations to identify behaviors and problems related to a participant's addiction and potential co-occurring disorders. S/he acts as an administrative liaison between the agency and outside organizations. Admissions Clinicians will be responsible for entering data into multiple databases and signing off on the LPHA portion of other intakes being performed.
