Position Summary Infant and Early Childhood Mental Health (IECMH) Consultation is an indirect, multilevel service in which consultants' partner with the adults in young children's lives to build their capacity to foster healthy social-emotional development.[1] In this program, consultants partner with and support early childhood care and education providers, in center- and home-based settings. IECMH Consultants receive specialized training to provide IECMH consultation through an evidence-based framework. This project's broad goals include reducing rates of suspension/expulsion from ECE settings, and more specifically to reduce race disparities in the rates of suspension/expulsion. THIS WORK MATTERS! Corrections Health provides medically necessary health care to Adults in Custody (AICs) and Youth in Custody (YICs) in Multnomah County jails. Behavioral Health Assessment and Planning Clinician - FLOAT provides services of assessment, crisis intervention, counseling and care coordination for identified individuals in jail. Other services include case consultation, group facilitation, education, attention to evidence-based practices and selected assigned administrative and client care duties. This assignment may float to any of the three locations within Corrections Health. The three locations would include: 1. The Multnomah County Detention Center (Downtown- adult population) 2. The Juvenile Detention Center (NE Portland-youth population) 3. Inverness Jail Detention Center (Near PDX airport) This position is essential to the public health and safety, and required to report for duty regardless of County decisions to close or curtail other county operations. Depending upon assignment, you will be performing some or all of the following duties, which are a representative sample of the level of work appropriate to these positions: Triage/Direct Care: Conduct initial mental health assessments using current DSM-5-TR diagnostic criteria. Facilitate coordination and referrals for subsequent care. Provide direct client care through crisis intervention, counseling, and client education, including guidance on accessing community resources. Initiate, monitor, and discontinue suicide watch as clinically indicated. The Behavioral Health Assessment and Planning Clinician - FLOAT primarily focuses on providing a high volume of accurate bio-psycho-social, diagnostic assessments, risk assessments, treatment planning, and safety planning to clients entering mental health services. This role typically will not carry a caseload. All other aspects of triage/direct care are assigned as needed such as urgent intervention and caseload coverage. Documentation: Documents client care with precision, adhering to national evidence-based clinical standards. Utilizes Electronic Health Records and Multnomah County Sheriff Office databases for clinical note and encounter documentation. Care Coordination: As the designated Responsible Mental Health Authority, Behavioral Health Assessment and Planning Clinician - FLOAT are responsible for providing and receiving clinical consultation. This includes consultation with other Corrections Health staff, Multnomah County Sheriff's Office (MCSO) staff, and external entities such as outpatient providers, partner agencies, and legal agencies. Such consultations are to be conducted as clinically indicated for care coordination, ensuring that appropriate releases of information are in place. Behavioral Health Assessment and Planning Clinician - FLOAT are also required to attend and actively participate in site-specific staff meetings, clinical huddles, and any other meetings mandated by mental health leadership. Training, Development, Supervision and Consultation Behavioral Health Assessment and Planning Clinician - FLOAT must complete 12 hours of annual professional training and continuing education, per NCCHC standards and Corrections Health leadership. They are also required to complete all assigned management training. Behavioral Health Assessment and Planning Clinician - FLOAT may develop and deliver training to Corrections Health, MCSO, and external partners. They must attend and participate in individual and group clinical and administrative supervision and consult with the Corrections Health mental health team and leadership on clinical issues. Through this program, Crisis Response Specialists assist police officers with situational de-escalation and crisis intervention as co-responders. BHU resources also provide emergency responders with additional options to divert individuals to behavioral health resources as an alternative to incarceration which have not been readily available in the past. BHU members cultivate and maintain professional relationships with organizations in the community that provide behavioral health resources to direct and refer individuals to the appropriate community resources. This role has no on-call duties and the ability to work a 4/10's schedule. You will also receive a CME budget and reimbursement for license renewal. The Behavioral Health Clinician is a member of the Metropolitan Pediatrics clinic staff, working in tandem with the clinic Medical Home care team to conduct assessments to evaluate the problem and utilize psycho/social health integration tools appropriate for the patient's illness, injury, disability, or current health status. The Behavioral Health Clinician will help patients and families solve and cope with problems in their everyday lives to improve overall health outcomes in the integrated care setting. The diverse responsibilities of this role include timely provision of behavioral health therapy and care coordination through direct contact with patients and their families, assisting with behavioral health referrals and prior authorizations, providing patient and team education and community outreach. This critical role has diverse responsibilities, which may include: Act as a member of a robust multidisciplinary team to provide crisis intervention, brief therapy, case consultation, resource connection, care coordination, and program development. Provide support for patient procedural distress. Identify and assess behavioral health needs as part of a multidisciplinary primary care team, billing appropriately for services rendered. Engage and activate patients and families in their care. Work collaboratively as a primary care team member both preventatively and to meet acute patient needs, providing ongoing support as needed. Ability to quickly build rapport, assess patient needs and provide brief clinical interventions. Help observe and improve care team function and relationships via ongoing professional education, collaboration, and contribution. Communicate effectively with other providers, staff, and patients. Provide efficient and evidence-based care that meets the needs of our primary care population. Provide culturally responsive, whole person, and family-oriented care. Understand, value, and adapt to the diverse professional cultures of an integrated care team. Provide timely behavioral health care coordination through direct contact with patients, their families, and community partners. Provide patient, parent and team education and community outreach. Contribute to ongoing program development and outcome research. Assist in practice improvement efforts through participation in committees and provider meetings. Metropolitan Pediatrics participates in E-Verify to confirm employees' eligibility to work in the U.S. Qualifications Experience and education required for this position includes: Current licensure as a Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT) or Licensed Professional Counselor (LPC) in the state of Oregon required. 1-3 years of experience in a clinical setting required. Transferrable experience considered. Knowledge and understanding of community resources, and effectively accessing them. Understanding of and experience working with PCPCH (Patient-Centered Primary Care Home) standards. Proficiency in Microsoft Excel and Word required. Experience working with an Electronic Medical Records system required with preference for Epic-specific experience. Experience with interpretation of a variety of patient population reports.
