Intensive Case Manager REPORTS TO: OCI Coordinator POSITION OVERVIEW: The Intensive Case Manager provides intensive and short term case management to high risk behavioral health consumers with the overall objective of securing and maintaining housing, and access and engagement to mental health services. The Case Manager will assist the consumers to improve their independent living skills and manage the symptoms of their mental illness. The Intensive Case Manager is responsible for the assessment and collaborative development of the consumer's goals and aspirations and coordination of services to meet their goals. As a Mental Health/Disability Specialist, you will collaborate with classroom staff in identifying children in need of individualized developmental and behavioral services. You will recommend and demonstrate strategies to help children adapt and succeed in the classroom and at home. Additionally, you will refer children for additional evaluation by partner agencies for Part B and Part C services and mental health consultation. You will document and track screenings, individualized services, and referrals in our child-tracking software. Major Duties and Responsibilities Under the supervision of the Executive Director or Therapy Services Supervisor, the Licensed Therapist provides counseling and access services for assigned clients. Responsibilities include: Clinical Services Conduct intake assessments, biopsychosocial histories, mental status exams, suicide and risk assessments, and other required documentation. Provide accurate diagnoses using the current DSM. Develop individualized treatment plans with clients; update plans every 90 days or following sentinel events. Deliver individual, couple, family, and/or group therapy using a strengths-based, culturally humble approach. Provide referrals to internal and external services as needed. Coordinate care with outside providers to support continuity and progress. Maintain timely, accurate documentation in the Electronic Health Record (HER). Progress notes completed within 48 hours Assessments completed within 72 hours Treatment plans completed within 7 days of intake. Access, Engagement, and Support Services Provide screening, brief treatment, education, engagement, and advocacy. Support clients in navigating community resources and systems. Serve as a primary point of access for walk-in clients within assigned caseload. Professional Collaboration & Representation Maintain professional relationships with community partners and promote RFS services. Represent the program through presentations, psychoeducational seminars, or community outreach. Participate in clinical supervision, peer review, and case consultations, and record audits to ensure quality and compliance. Program Development & Organizational Support · Supervise and support practicum students and program-specific peer mentors.
