**About the Role:**
The Forklift Operator role at the Faison location provides support by driving and supporting key initiative to improve performance throughout the plant. Perform with efficiency, all phases of work available in throughout the facility. You'll add value to this role by performing various functions including, but not limited to:
+ Unloading material and stock from incoming vehicles and move to proper staging location based on product (hydro-cooling, cooler, tanks, etc.)

+ Simple maintenance on the forklift such as recharging its battery and refueling.

+ Being responsible for recognizing and reporting people safety, food safety, and/or quality issues.

Develops and implements the patient care plan in cooperation with other Agency professionals and paraprofessionals who will be providing care to the patient in MatrixCare.
Manages assigned cases and provides skilled nursing care as outlined on the physician's plan of treatment and in accordance with the Agency's policies and procedures.
Observes signs and symptoms and reports to the physician and other applicable personnel, reactions to treatments, including drugs and changes in the patient's physical or emotional condition and outcomes of intervention. Ensures on-going evaluation of patient / family response to care.
Adequately educates, teaches, supervises, consults, and counsels the patient / family members regarding the nursing care needs, other related problems of the patient at home, the disease process, self-care techniques, end-of-life care, and processes for dealing with issues of ethical concern. Also provides specialized hospice training to other staff, family members and informal caregivers to ensure adequate care.
Initiates appropriate preventative and rehabilitative nursing procedures, as appropriate, for the patient's care and safety.

Summary:
The Speech Pathologist plans, directs, and delivers language-communication services to Agency patients in coordination with the Agency staff so the stated goal and objectives can be achieved.
SPEECH THERAPIST (ST) ESSENTIAL JOB FUNCTIONS:
1. Provides evaluation of patients with speech and communication disorders when referred by Agency staff and/or patient's physician according to Agency policies. Develops and revises a plan of treatment/care in collaboration & consultation with the physician, IDG member, and healthcare team and revising it as appropriate. Consults and coordinates services with other disciplines and plans discharge, as appropriate.
2. Determines and recommends the best functional communication system for use by the patient in his/her unique activities of daily living and recommends inter and intra-agency referrals, documenting via the plan of treatment and the patient care plan. Provides treatment according to plan of care.
3. Ensures ongoing assessment, evaluation and documentation of patient's level of functioning and hearing in response to therapy.
4. Tests and recommends mechanisms, which focus on interventions as alternative methods of communication/speech and swallowing exercises to help with nutrition.
5. Plans, implements, and teaches patients and family members to provide for continuous rehabilitative and habilitative services for speech and communication disorders after the patient is discharged under Medicare and other insurance reimbursements guidelines.
6. Assesses, reports, and records patient's status, progress and changes in the client's needs to the physician, health care team and other appropriate personnel.
7. Ensures initial and ongoing assessments to identify the client's level of functioning and ability to communicate, including OASIS assessments at appropriate time points.
8. Educates, consults and teaches other agency personnel, family members, designated facilitators, and other organizational personnel in methods of assisting the patients according to a plan of treatment.
9. Evaluates the effectiveness and outcomes of care and identifies barriers to desired outcome. Set goals based on the needs of the patient/family.
10. Participates in continuing education programs to share and stimulate the health care team in speech therapy involvement, in team conferences for certification and recertification of plans of treatment. Maintains current healthcare information per McKesson.
11. Displays a high degree of courtesy, tact, and knowledge of services provided by the agency in all contact with staff, patients, and visitors.
Requirements:
Master's Degree in Speech Pathology.
Certificate of Clinical Competence in Speech Pathology or in CFY.
Speech Pathologist currently or temporarily licensed to practice speech pathology in North Carolina.
Maintain current CPR certification (BCLS).
