Essential Functions:
Provide access to general information regarding CNS Healthcare, community mental health, and the services provided.
Provide support and collaboration between persons served, staff, and community members.
Monitor, assist, and resolve any customer service concerns, grievances, and any level of due process.
Receive, document, investigate, reply, formulate a solution, and resolve or escalate any customer service complaint.
Provide education and written materials regarding Recipient Rights.
Documents, reports, and coordinates with the Office of Recipient Rights.
Ensure that all Recipient Rights booklets and/or materials and required visual information is accessible in the lobbies.
Gather materials needed for the appropriate county's New Member/Intake packets.
Create and administer surveys for data gathering related to customer service and other related areas.
Monitor customer service and grievance logs.
Collaborate with the Total Quality Management (TQM) department to implement changes in standards of service.
Participate in community events, conferences, professional organizations, and educational programs to maintain up to date knowledge as assigned.
Ensure compliance with Limited English Proficiency Policy.
Act as a contact person for staff and persons served regarding interpreters, translated materials, and assistance with obtaining assistive technology, such as braille and voiced materials.
Provide forms and general information regarding CNS Healthcare services, local resources, and Advance Directives to staff and persons served.
Create, monitor, and analyze regular reports based on documented information from the Customer Service team and Strategic Plan for necessary committee, department, or contractual requirements.
Monitor, document, and respond to cards from Suggestion/Praises box.
Participate in accreditation reviews, initiatives, agency focus groups, work groups, and committees as assigned.
Support the development of agency staff and community partners by providing training (including Recipient Rights) as assigned.
Utilize motivational interviewing strategies and a trauma-informed approach to engage with and provide service to individuals.
Complete all required documentation completely, accurately, and in accordance with CNS
policy.
Meet direct service productivity and documentation standards per agency and program expectations.
Maintain compliance with quality care initiatives including Meaningful Use (MU), Medicare Merit Based Incentive Payment System (MIPS), Commission on Accreditation of Rehabilitation Facilities (CARF), and Certified Community Behavioral Health Clinic (CCBHC) requirements.
Ensure that the CNS Healthcare standards of service are applied to interactions with individuals served, guests, and staff.
Assume responsibility for professional growth by current reading and attendance at educational programs and conferences.
Maintain knowledge and compliance with established policies and procedures, corporate compliance program, code of ethics, applicable federal, state, and local laws and regulations, HIPAA standards, and other regulatory programs.
Perform other related duties as assigned.
Qualifications
At least a Bachelor's degree in Social Work, Human Services, or related fields.
Specialized training (including fieldwork and/or internships associated with the academic curriculum where the student works directly with persons receiving mental health services as part of that experience) ORone year of experience in treating or working with a person who has a mental illness.
Proficient with computers, Electronic Health Record (EHR) systems, and Microsoft office products including Word, Outlook, and Excel.
Experience with the application of the Health Insurance Portability and Accountability Act (HIPAA) in health care organizations
Ability to work closely with persons with mental or physical limitations.
Ability to format documents and use proper grammar, punctuation, and spell check.
Ability to work well as part of a multi-disciplinary team, communicate effectively, work collaboratively, and build positive working relationships
Ability to use discretion and judgment when handling matters of a sensitive or confidential nature.

The ICCW Care Coordinator convene the child and family team and guide members through the wraparound process, ensuring adherence to model fidelity and Wraparound values.
Maintain a caseload of 10-12 families with a minimum of one face-to-face contact per
Assemble a child and family team within two (2) weeks of enrollment by interviewing the family, identifying family members/natural supports/agency representatives and other significant persons.
Work with the family to uncover youths/family's strengths and needs; provide assistance with any immediate needs; and complete initial Strength Assessment and Needs and Priorities within 1 week of enrollment.
Work with the family and submit a comprehensive Support Plan (proactive and reactive crisis/safety plan) within 1 week of The plan must reflect the best possible fit with the family's strengths, culture, and beliefs.
Work with the Child and Family Team and submit an ICCW Plan of Care (POC) with services and resources that are community based and culturally relevant within 45 days of enrollment and an updated ICCW plan every 90 days thereafter.
Collaborate with other necessary individuals with whom the youth and family have contact, such as, CMO Workers, Teachers, Judges, District Attorneys, Mental Health Clinicians, DHHS, Foster Care workers, Physicians, etc. This means ICCW Care Coordinator maintain frequent contact, invites them to Child and Family Team meetings with adequate notice and provide copies of the ICCW Plan of Care within 2 weeks of completion to team members.
Monitor the provision of quality services provided through the Child and Family Team and CNS ICCW Care Coordinator when new services/resources must be sought or developed on behalf of the Child and Family Team. Community resources are sought first with the assistance of the Child and Family Team and the Community Team.
Make families aware of Youth Guided community activities and encourage youth and parent involvement in leadership and advocacy within their community (Wayne, Oakland or Macomb)
Assists the family's participation in family events by arranging transportation as
Maintain and update clinical knowledge by reading, in service training in designated areas, and other activities in consultation with supervisor.

Our Intake Analysts help new individuals seeking mental health and substance use services at CNS Healthcare. Our Intake Analyst team also provides program orientation and detailed information about services to new individuals.

Ensure that the individual's needs are identified and initially met through innovative and clinically effective methods.

Engage individuals into treatments that are frequently hospitalized, and those that continue to refuse treatment.
Utilize formal and informal assessment practices in order to respond to the needs, desires and interests of individuals.
Participate in outreach activities that target community agencies and facilities such as jails, shelters for homeless, hospitals, etc.
Perform intake office coverage duties as assigned.
Use motivational interviewing strategies to engage and provide service to individuals.

Provide integrated behavioral and physical healthcare, including medication management, the assessment and coordination of physical/medical care, and the provision of direct services to the beneficiary in the community.
Complete a thorough and accurate nursing assessment to evaluate current medical/physical symptoms and conditions, including substance use, and provide recommendations to applicable care providers as appropriate.
Contribute to the person-centered planning process for individuals served as a member of team-based service model and identify and use effective advocacy strategies to address and overcome diverse challenges facing individuals served.
Routinely collect accurate vital signs including, but not limited to, blood pressure, pulse, height, weight, body mass index (BMI), waist circumference, pulse, oxygen, glucose, pulse oximetry, and carbon monoxide testing.
Perform Point of Service testing including, but not limited to, HGBA1C, Drug Screening, Lipids, Glucose, and pregnancy testing as appropriate.
Assess and recommend lab work based on the individual's medications and health conditions, collecting samples as required. Communicate with the individual, care team, and lab to ensure that lab work is successfully completed.

Act as a triage nurse to assist in responding to medical emergencies, triage emergent situations, and assist in problem-solving regarding medications, medical issues, or access to prescriber services as needed.
Provide education regarding chronic health conditions, disease process, healthy lifestyle management, vaccinations, and medication management including risks, benefits, and side effects.
Routinely review current medications with persons served and provide medication continuity until the individual is seen by an agency provider.
Recommend and encourage the use of increased prescriptions for Medication Assisted Treatment (MAT) for tobacco, alcohol, and opioid use disorders when clinically appropriate.
As a team-based service, perform a minimum of 80% of ACT contacts in the beneficiary's home or other agreed upon community location, with at least 70% of the nursing contacts being performed in the beneficiary's home or other agreed upon community location.
Provide transportation assistance to individuals served as needed.
Attend ACT team meetings at least weekly, or more frequently at the discretion of the ACT Team Supervisor, to review status of all beneficiaries, updates from on-call, clinical and case/care management needs, crisis management, schedule organization, and finalized plans for ACT staff deployment in the community.
Participate in on-call rotation as assigned, which includes completing pre-hospital admission assessments as needed.

PSR Generalist plays a key role in supporting adults experiencing mental health symptoms by providing direct rehabilitative services. Generalists engage members in meaningful, growth-focused activities in alignment with MDHHS guidelines and Clubhouse International Standards for psychosocial rehabilitation. Compensation Range:
The starting pay for this position is $17.00 - $21.00 hourly rate, and is based on non-discriminatory factors such as skills and experience.

General Responsibilities:
Participate as part of a treatment and rehabilitation team to support effective program operations and service delivery.
Work collaboratively within Clubhouse Units such as Food Service, Janitorial, Clerical, Employment, Transportation, and Member Services during the work-ordered day.
Engage members 1:1 as partners in meaningful work; provide coverage at Transitional Employment sites when a member is unavailable.
Take initiative in identifying and facilitating work opportunities for members within the Clubhouse.
Provide transportation to members as needed.
Ensure adherence to fidelity-based Clubhouse International Standards, including participation in weekend and holiday programming as required.
Maintain a working knowledge of the Michigan Mental Health Code and Recipient Rights.
Always uphold professional boundaries and consumer confidentiality.
Complete all required documentation accurately and within established timelines.
Work effectively both independently and as part of a team.
Seek clarification or direction when needed to support program success.
Demonstrate strong customer service skills when interacting with members, staff, partner providers, Home Operators, and community groups.
Assist the Clubhouse Supervisor and PSR Generalists with special projects as assigned.
Support and model customer service expectations.
Utilize de-escalation skills to respond appropriately to confrontational, antagonistic, or hostile behavior.
Participate in required staff development and mandatory CNS educational programs.
Demonstrate competency in key dimensions for this classification, as defined by agency standards.
Maintain proficiency in MS Office (Word, Excel, PowerPoint, Outlook) and navigate electronic medical records effectively.
Rotate with staff for evening/Saturday programs and designated holiday shifts.
Perform additional duties as assigned.
Position Qualifications:
High School Diploma or Equivalent. Some college level courses are preferred.
Minimum one (1) year experience in working with adults with serious mental illness.
Valid Michigan Driver's license and good driving record.
Transportation or automobile accessibility with current auto insurance.
Basic understanding of principles of psychosocial rehabilitation.
Understanding of Michigan Mental Health Code and Recipient Rights.

Use evidence-based practices to provide individual, family, and group therapy
Conduct face-to-face comprehensive biopsychosocial assessments
Formulate and coordinate person-centered treatment plans
Collaborate and participates with treatment team, community partners, and other natural supports as identified by the family
Assist families with referral, linking, and coordination to community resources, and provide education on how to access and utilize supports independently.
Assist families with accessing and utilizing community supports
Act as liaison between the child/family and their natural support system, primary care physicians, and other community agencies identified by the family
Coordinate care with families, schools, DHHS/foster care, and other community agencies.
Initiate referrals for psychiatric evaluations when deemed clinically appropriate
Provide case management support for children and families as needed
Participate in case conferences and staff meetings
Utilize clinical supervision, respond to directives, and implement feedback.
Prepare reports for other agencies, DHHS, courts, schools, etc. as needed
Obtain and maintain CAFAS rater-reliability and conduct quarterly CAFAS assessments
Utilize a trauma-informed approach to services
