YAI co-facilitator job description
Location: Queens
● Co-Facilitate a weekly course with ILSP group members (12-16 people) once a week for 12 weeks and complete progress notes contemporaneously
● Ensure all required documents (i.e. waivers) are signed for each member upon initial group attendance.
● Group lasts for 2.5 hours with members and requires staff to arrive 15 minutes early to set up and half hour clean up afterwards. For example a group that runs 4PM-6:30PM would require staff to be on hand from 3:45pm to 7PM.
● Help create and support a group setting that allows for members to feel comfortable and open in sharing with the process of becoming more independent
● Support individuals with any necessary collateral contacts which may include family members/caregivers, service coordinators or mental health professionals.
● Provide neutral feedback and discussion around ILSP curriculum topics during sessions for members to make educated decisions for themselves that focuses on self-direction and self-determination (see curriculum list)
● Participate in meetings with program supervisor as needed.
● Ensure the mental and physical health and safety of all group members
● Attend all mandatory YAI staff training and OPWDD mandated trainings (annual)
● Help maintain an accurate attendance record
Compensation
● Co-Facilitators are compensated $87.50 per group session
● Additionally, you will also be compensated as per the number of progress notes you complete per group session: For 1-6 notes, you will be paid $26.25. For 7-10 notes you will be paid $43.75. For 11-16 notes you will be paid $70 and for 17-20 notes, you will be paid 87.50.
● You will also be compensated at hourly wage ($35/hr) for any facilitator meetings and mandatory trainings.
● If you are a current YAI non-exempt staff, you will be paid time and a half for supporting groups. You will also be paid time and a half for facilitator meetings.

In this role, you will will serve as Co-trainer primarily focusing on training for the UCLA Health Ambulatory Practices. You will serve as a resource for new and existing staff for seeking out information related to front and back-office duties. Monitor new hires progress throughout training by direct observation and routinely reviewing new hires progress throughout the Simulations. Conduct in-service and customized training session to address quality issues or changes in operating procedures. Assist in clinic on boarding process and expansion of Ambulatory Re-design initiatives thru the implementation of customized training programs for assigned staff. Must be willing and able to travel to multiple Ambulatory sites as needed.

Qualifications

+ Knowledge of UCLA systems is strongly preferred
+ Experience with various computer software and hardware including word-processing, knowledge of Microsoft Word, Excel, Powerpoint, Outlook, and the internet. Typing skills to prepare forms and correspondence with speed accuracy.
+ Skill in adapting to various learning styles and training methodologies.
