The Chronic Disease Prevention Program ( CDPP ) is an 8 module educational online course that aims to help unemployed individuals utilizing employment services at Orange County DSS to enter the workforce in a healthy manner. The program instructs participants on such topics as weight maintenance, diet, exercise, stress management, and problem solving. The program involves 4 coach visits during the first 10 weeks while the participant is completing the course, and then 5 phone check-ins over the next 30 weeks after the course has been completed. The LifeStyle Coach will guide participants through the CDPP while counseling individuals on such topics as exercise, nutrition, stress management, problem solving. The LifeStyle coach will primarily be responsible for scheduling and attending coach sessions, monitoring participants progress, and actively fielding questions or concerns from assigned participants. The ideal candidate will display a robust knowledge of diet/nutrition, exercise, stress management, and problem solving. A transformational leadership style will be crucial to success in this role. Positivity, encouragement, and motivation will be needed when advising participants. Candidates will need strong communication skills in order to coherently articulate instructions to participants. In addition, the ability to actively adjust coaching techniques based on participants individual needs is vital.
