Provide education and support to clients in recovery for substance use disorder. Participate in outreach to community partners to help coordinate services for clients in order to help sustain sobriety. Maintain regular contact with clients to monitor and assess progress.
Position Description:
Assist staff with caseloads through regular client contact in community and program setting. Track and assess progress as determined by program guidelines
Help to develop and implement Individual Service Plans
Efficiently operate personal computer to enter, edit and/or retrieve data using agency electronic health record
Assist clients in accessing community resources, including, but not limited to: food, clothing, supportive housing, Medicaid, job readiness and/or placement, public assistance, etc.
Facilitate site-based daily support/peer groups
Visit, support, and transport clients as needed upon discharge from program
Collaborate with family members, internal and external case managers, and other providers while supporting client's recovery
Prepare documentation in compliance with contract, regulatory and Agency requirements
Other duties as assigned
Required Knowledge, Skills and Abilities:
Knowledge of substance use disorders and community resources to assist with maintaining sobriety
Effective verbal and interpersonal communication skills
Ability to operate personal computer to enter, edit and/or retrieve data and to produce reports and correspondence as necessary
Beginner level problem-solving skills and ability to multi-task while prioritizing tasks
Job Requirements:
High School Diploma or High School Equivalency
Must be in recovery for minimum 12 months; preferably 2 years
Peer Support Specialist certification or ability to obtain within first three months of employment
