About the Company
CCPHP is seeking a full-time and highly skilled Exercise Physiologist / Performance & Wellness Health Coach to deliver data-driven performance testing and individualized coaching services to members of our concierge medicine organization. This position will begin as part-time and transition within three months to a full-time position. This role is New York City-based and requires travel throughout the United States to support on-site performance training, testing, assessments, and physician engagement, in addition to remote coaching conducted via a HIPAA-compliant audio and video platform. This position is ideal for a candidate with a strong background in exercise physiology, longevity, metabolic testing, and performance interpretation, who also enjoys working in a hands-on, client-facing environment with a discerning concierge population. Coaching and assessments are delivered within the scope of CCPHP's Wellness Program, which integrates sleep, exercise, nutrition, and stress management to support optimal health and performance, prevent and manage chronic disease, as well as promote longevity and healthspan.
About the Role
Essential Duties and Responsibilities
Conduct VO₂ max testing, metabolic assessments, and related performance evaluations for concierge members
Set up, calibrate, and break down performance testing equipment at clinical or on-site locations
Interpret performance data and translate results into individualized exercise, recovery, and lifestyle recommendations
Deliver personalized coaching sessions informed by objective testing data, member goals, and physician input
Conduct follow-up coaching sessions remotely or in person to support performance improvement, longevity goals and long-term behavior change
Track outcomes, maintain accurate documentation of all testing and coaching sessions and create timely summary reports for the National Director
Collaborate closely with the National Director, CCPHP staff, and members' physicians to ensure aligned, high-quality wellness support
Travel throughout the U.S. as needed to support performance testing events, on-site assessments, and member programming
Qualifications
Bachelor's degree in Exercise Physiology, Kinesiology, or a related field required
National Board Certification in Health Coaching (NBC-HWC) required (or eligible)
Master's degree in a health-related field preferred
ACSM-EP or ACSM-CEP Exercise Physiologist certification is preferred; may also be NSCA Certified Strength & Conditioning Specialist (CSCS)
Minimum of 2 years of experience in performance testing and health or performance coaching
Hands-on experience conducting VO₂ max testing and metabolic performance assessments required
Experience working in a clinical setting and collaborating with physicians strongly preferred
Proficiency with MS Office (PowerPoint, Word, Excel), Zoom, and healthcare platforms (EMRs, telehealth systems, client portals)
Required Skills
Strong foundation in exercise physiology, metabolic testing, and performance assessment methodologies
Proven ability to analyze and interpret VO₂ max and related performance data
Ability to communicate complex physiological concepts in a clear, motivating, client-friendly manner
Deep understanding of current longevity science, including emerging research in exercise physiology, metabolic health, sleep optimization, nutrition science, and stress management, with the ability to discuss these topics in a non-clinical, educational, and evidence-based manner to support member goals and the overall CCPHP Wellness Program
High level of professionalism, confidence, and presence in client-facing and clinical environments
Strong organizational skills and comfort working independently during on-site testing engagements

RESPONSIBILITIES
1. Serve as a Mentor for a team of approximately 20 youth (mentees):
● Serve as first point of staff level contact for the mentees.
● Help plan, set, and keep students on their program schedule.
● Maintain weekly contact with each mentee in person and via phone.
● Maintain regular contact with mentees' parent/guardian.
● Help SBU Education Director monitor student academic performance and assist
in the collection of student report cards, transcripts, and school documents.
● Work with SBU Youth Counselor and Education Director to identify social,
emotional, and academic issues or concerns.
● Check email daily and maintain good communication with SBU office staff
● Hold a weekly call with SBU office staff to discuss progress of each
student-athlete on the team.

● Complete progress reports for each mentee two times per year.
● Attend program orientation/training (2 trainings per year)
2. Serve as Head Coach a travel youth soccer team
● Plan and lead training sessions at least two times per week during the season and at least one time per week during the winter.

● Coordinate team travel and traveling with students to games and events.
● Track attendance data from soccer and academic sessions.
● Attend coach training sessions and coach meetings
● Communicate effectively with youth regarding soccer and academic events.

SCHEDULE

The Head Coach of our prestigious youth basketball program, with over 60 years of rich history in Harlem, will lead and inspire young athletes to reach their full potential both on and off the court. This role encompasses the development of a comprehensive coaching strategy that integrates high-level basketball training with a strong emphasis on academic enrichment, personal growth, and community involvement. The Head Coach will be a passionate leader dedicated to fostering a positive team culture and empowering players to become well-rounded individuals.
Essential Job Responsibilities:
Program Leadership: Develop and implement a cohesive vision for the basketball program that aligns with its mission of academic enrichment and community engagement. Provide strategic direction for all coaching activities and team initiatives.
Coaching & Training: Plan and lead effective practices that enhance players' basketball skills, teamwork, and game strategy. Prepare the team for competitive play while instilling a strong work ethic and a commitment to excellence.
Academic Integration: Collaborate with Basketball Director and Academic Coordinator to ensure that players prioritize their education. Develop programs and initiatives that support academic achievement, including tutoring sessions, study halls, and college readiness workshops and college visits.
Mentorship & Development: Serve as a mentor and role model for players, promoting values such as integrity, discipline, respect, and resilience. Provide individualized support to help athletes navigate personal challenges and set both athletic and academic goals.
Community Engagement: Foster strong relationships with players, parents, and community members. Actively participate in community events and outreach efforts to promote the program and its impact, and to encourage community support and involvement.
Player Evaluation & Development: Monitor player progress through regular assessments and feedback. Identify areas for improvement and develop tailored training plans to help each athlete reach their full potential.
Safety & Compliance: Ensure a safe and inclusive environment for all participants, adhering to program policies, safety guidelines, and best practices in youth sports.
Administrative Responsibilities: Oversee team logistics, including scheduling practices, games, and events. Manage communication with players and families, and coordinate with program staff on administrative tasks as needed.
Education and Experience Requirements:
Extensive coaching experience, preferably at the youth or high school level, with a strong background in basketball.
Proven track record of developing young athletes both in sports and academics.
Exceptional leadership, communication, and interpersonal skills, with the ability to connect with youth and their families.
Strong understanding of the importance of academic achievement in conjunction with athletic development.
Commitment to fostering a positive, inclusive, and supportive team culture.
First Aid/CPR certification is a plus.
Bachelor's degree in education, sports management, or a related field will be considered for greater leadership opportunities

Requirements
Minimum Qualifications
High School or Equivalent or better.

A commitment to the mission and programs of HCZ
Experience working with youth and adolescent population
College degree and at least two years of coaching experience, preferred
Must have strong knowledge and a keen understanding of football
Ensure the various athletics curriculums are structured and have clear learning goals
Solid understanding of youth sports and safety guidelines
Must be able to travel to away games or events
Must have strong leadership and organizational skills, excellent writing and communication skills
Positive, enthusiastic, collaborative, flexible, and approachable attitude
Ability to effectively engage and motivate students
What you'll do
Develop, implement, and execute the flag football athletics curriculum
Create a program that will include daily conditioning, fundamental drills, and a strategic playbook
Communicate and work effectively with students, colleagues, and parents
Monitor and report on student progress and performance both in the classroom and playing field
Collaborate and consult with staff on student performance in various athletic activities and schoolwork
Maintain ongoing communication with Athletics Program Director
Develop positive relationships with students that promotes wellness, growth, respect, and self-esteem
Act as a positive role model for students
Perform other duties as assigned
