Services include a full range of DHS 75 and DHS 83 license requirements and include a trauma informed approach, with gender specific services for men.

Role Description
The Fleet PM and Parts Coordinator is responsible for developing and maintaining comprehensive Equipment Maintenance Plans (EMPs) to ensure the reliability and performance of all site assets. Major Responsibilities:
Conducts a physical therapy evaluation of each patient and establishes a plan of care and treatment goals. Interprets physician referrals and completes musculoskeletal, neuromuscular, cardiopulmonary, and/or integument evaluations utilizing appropriate methods. Evaluates outcomes within an appropriate time frame.
Provides on-going assessment of patient response to treatments and teaches and applies interventions as appropriate. Updates and revises plan of care as indicated and provides physician with status report on a regular basis.
Communicates problems to physicians and management and clearly states plans, actions and goals so changes are reflected as they occur. Completes a plan of care as indicated and provides physician with status report. Maintains and updates clinical and patient records according to state and federal guidelines.
Manages assigned caseload including scheduling and informing site leadership of coverage needs and ability to provide coverage. Manages patient's medical condition including medications. Acts as a patient advocate to insure optimal outcomes. Teaches appropriate treatment procedures and supportive activities to patients and caregivers and develops plan for obtaining adaptive devices as needed.

Licensure, Registration, and/or Certification Required:

We're looking for Telemetry RNs for an immediate travel nurse opening in Marshfield, WI. The right RN should have 1-2 years' recent acute care experience. Read below for more requirements. As a TELE Travel Nurse, you will provide care by connecting patients to machines that measure heart rate, blood pressure, breathing rate, as well as blood-oxygen levels and electrocardiogram information. These machines then send data to computer screens for RNs to monitor. Telemetry RNs read and interpret the data to better determine patient care.

This position involves carefully reviewing financial and medication assistance applications to ensure compliance with grant and other program funding requirements. Additionally, the Patient Assistance Counselor maintains a comprehensive understanding of healthcare coverage options available through local, state, and federal programs. ESSENTIAL JOB FUNCTIONS
Documents interactions with patients via phone or in person and defines the action(s) taken for the purpose of recording these encounters for future reference.
Conducts outreach and enrollment activities to bridge patients to available health care and medication programs/grants and interacts with various internal and external resources to ensure patient referral needs are met.
Reviews Financial Assistance and medication assistance applications to ensure completeness and program requirements are met.
Assists with the approval or denial of Financial Assistance applications within a specified dollar amount to help facilitate timely processing of applications.
Reviews applicants' outstanding account balances to determine which balances can be included in the Financial Assistance award, including balances referred to other agencies.
Calculates income and asset totals to determine the Financial Assistance award level.
Reviews applicants' accounts for existing payment plans and terminates the plan to avoid further collection measures while the Financial Assistance application is being reviewed.
Reviews accounts to determine individual patient eligibility for assistance through outside financial programs and monitors for ongoing assistance.
Maintains financial and social case management programming statistics for grant program purposes.
Regular attendance is required in order to carry out the essential functions of the position.

JOB QUALIFICATIONS
EDUCATION

EXPERIENCE

CERTIFICATIONS/LICENSES

Employees hired less than three months before the start of the CAC training period may defer their attendance and certification exam until the following year.
