The Support Worker observes and assists clients in a residential setting. HIGHLIGHTS * Person centered collaborative approach, designed to provide knowledge, skills, and resources to residents to ensure living in the most independent setting available, and address any emergent issues to ensure their self-care. * Maintaining necessary casework records to document the provision of case management services for Medicaid reimbursement. * At least 55% of each day is spent in direct face-to-face contact with persons served.     Contract Description The PER DIEM Master Level Supervisor will oversee the Mobile Crisis Outreach Response Teams (MCORT) in Ocean, Monmouth, Middlesex and Union counties. The team will consist of Peer Support Specialists & Bachelor's Level Crisis Counselors. This position involves progress note review, crisis response decision making, technical assistance provision, and continuous staff development. MCORT provides 24/7 mobile community outreach services to adults in crisis, including mental health, substance use, and suicidal emergencies. Calls are dispatched through the 988 Suicide Crisis Lifeline Call Center. Mobile Crisis Outreach Teams will be available to help individuals de-escalate their crisis and access needed resources. Highlights Manage and supervise direct care staff, including regular supervision, managing schedules, and addressing any performance issues. Review individual care plans developed by the staff to ensure that the appropriate level of care is being utilized and that the treatment plan is clinically supported. Evaluate staff productivity, documentation of service delivery and billing. Be available for on-call clinical consultation or in-person case reviews with to determine the most appropriate intervention or if a higher level of care is necessary. Provide consultation, training, and technical assistance as needed and clinical oversight for day-to-day operations. Work with community referral sources and stabilization provider networks regarding case management and linkages to stabilization and aftercare services.
