The Senior Utilization Management Consultant is a highly experienced Registered Nurse responsible for ensuring the compliant, efficient, and high-quality operation of Utilization Management (UM) programs. This role serves as the internal expert on federal and state regulatory requirements, translating medical policy into actionable operational procedures. The Consultant drives quality assurance, performs audit oversight of UM processes, and acts as the primary clinical liaison between the organization and external clients or Third-Party Administrator (TPA) vendors. This critical role is responsible for maintaining clinical integrity, ensuring policy adherence, and performing targeted quality audits across our diverse client base with an additional focus on regulatory consistency.This position does not involve direct staff management but requires strong cross-functional leadership and client communication skills.
What you'll do:
Clinical and Operational Oversight & Policy
Operationalize medical policy by collaborating with the UM program manager, clients, UM vendor(s) and internal CH teams to formally adopt, operationalize, and update policies.
Drive process improvements and efficiencies within UM operations, ensuring standards are consistently met.
Work with cross-functional teams to integrate new policies or regulatory changes into existing technological systems and workflows.
Maintain and manage the accuracy of the prior authorization code list, ensuring alignment with current medical policy and regulatory requirements
Maintains documentation for audits, annual program evaluation to ensure policies and practice meet regulatory and clinical integrity requirements.
Quality Assurance & Regulatory Compliance
Clinical Audits: Conducts ongoing clinical file reviews of the UM vendor's medical necessity decisions. Audits adverse determination notices for clinical accuracy, ensuring they correctly cite the specific utilized clinical criteria (MCG code, Plan Policies) as required by state and accreditation standards.
Utilization Review: Identify trends, patterns of over or under-utilization, and potential areas for intervention. Monitor prior authorizations, concurrent reviews (inpatient case management), and retrospective reviews to ensure appropriate care, setting, and length of stay.
Clinical/Quality Review: Review provider, member, client, and internal queries related to prior authorizations.
Ensure regulatory compliance: Maintain up-to-date knowledge of and adherence to federal, state, and CMS regulations, as well as accreditation standards. Prepare documentation and process narratives to support NCQA delegation reporting for UM
Stay up-to-date with relevant UM regulations and industry best practices.
Ensure UM operations are in compliance with all applicable laws and regulations.
Assist in the development and implementation of policies and procedures to mitigate regulatory risk.
Stakeholder & Client Communication
Act as the primary clinical and UM operations liaison for clients and the TPA UM vendor, addressing complex questions and resolving regulatory or policy discrepancies.
Facilitate cross-functional communication and collaboration with departments such as Clinical Policy, Legal, Compliance, and IT to ensure seamless UM operations. Facilitates consistent patient transitions between internal UM and Care Management teams and ensure linkage between teams.
Participate in client-facing meetings
Subject matter expertise: Serve as an internal SME on medical necessity criteria, evidence-based guidelines, accepted standards of medical practice, and prior authorization coding
To be successful in this role, you'll need:
Education: Current, active Registered Nurse (RN) license required. Bachelor of Science in Nursing (BSN) or related field preferred.
Experience: 5-7 years of progressive experience in Utilization Management within a health plan, Managed Care Organization (MCO), or TPA setting. Minimum of 5 years of direct clinical patient care experience.
Expertise: In-depth knowledge of UM regulatory requirements, including state/federal mandates, accreditation standards (e.g., NCQA, URAC), and medical necessity criteria. Mastery of coding for prior authorizations
Skills: Demonstrated ability to translate complex regulatory language into clear, executable operational steps and policies. Excellent written and verbal communication, presentation, and auditing skills.

What You'll Do:
Leading outreach efforts to high-risk individuals through telephonic or electronic outreach
Completing telephonic assessments and creating goal oriented care plans for members enrolled in the program, and supporting the members as they work to achieve those goals.
Use evidence-based medicine and professional guideline recommendations to improve members' health through interventions including but not limited to:
Suggesting formulary alternatives to ask about
Coaching on medication adherence strategies
Coordinating receiving medications through lower cost of care settings
Working with providers to address drug interactions or adverse drug reactions
Recommending dosage adjustments to optimize therapy
Identifying opportunities for preventive care like immunizations and cancer screenings, etc.
Educating and empowering members using decision support through multiple modalities including telephone and email
Provide accurate documentation of member assessments and follow up member interactions
Identification of opportunities for maximizing value on medical and pharmacy health plan benefits
Collaborate with other clinicians on the Care Navigation team on a daily basis and presentation of complex cases to the medical director on a monthly basis
Provide subject matter expertise to support the Collective Health operations teams in response to complex or escalated pharmacy inquiries

To be successful in this role, you'll need:
A PharmD
Minimum of 2 years experience in a PBM, hospital or clinic, retail pharmacy, health plan, or other managed care setting
2-3 years experience in Medication Therapy Management, Clinical Specialty Programs, experience at a payer (PBM, TPA, health plan, etc.), or residency training is preferred

Required Qualifications

This is a role for a hands-on, highly technical engineering manager who is passionate about building resilient, scalable, and automated observability and monitoring systems. The person will be tasked with a "greenfield" opportunity to support the architect, build, and manage our enterprise Network Operations Center (NOC). The primary objective is to move beyond traditional, reactive monitoring and support the creation of an automation-driven, proactive, and predictive technical operations function.
The ideal candidate possesses deep technical expertise in modern observability and automation stacks and is driven to eliminate manual toil and improve reliability through code.
The successful candidate will be responsible for supporting the design of the operational framework, participating in selecting and implementing monitoring tools, hiring and training a team of skilled engineers, and establishing the processes necessary to ensure 24/7/365 availability, performance, and security of our critical infrastructure.
The primary goal of this role is to participate in the creation of a proactive, best-in-class NOC that minimizes downtime and service disruptions, thereby directly supporting exceptional patient care and healthcare systems operations. An essential skill for this position is to have a deep understanding of the regulatory and operational demands of the healthcare industry, including HIPAA compliance.
What you'll do:
You will be the principal architect and engineer for our new NOC, participating in the building of a highly automated, resilient, and insightful operations function from the ground up.
Participate in leading the end-to-end design and greenfield implementation of the NOC. Define and document all operational policies, SOPs, and data-driven performance standards (KPIs, SLOs, SLAs, and Error Budgets) to establish a foundation of excellence.
Deploy a sophisticated observability platform using tools like Prometheus, Grafana, Honeycomb, the ELK Stack, etc., to provide deep, actionable insights into our complex AWS and GCP cloud environments.
Evangelize and develop a framework for automated diagnostics and remediation. Drive the adoption of Infrastructure as Code (IaC) with Terraform and Ansible to eliminate manual toil and ensure consistent, repeatable environments.
Develop custom code that may be needed from time to time using technologies such as Python, Javascript, Java, Database, etc.
You will help build and cultivate a world-class technical team, fostering a culture that attracts and retains top engineering talent.
Lead the full-cycle recruitment, hiring, and onboarding process for the founding team of NOC engineers (L1/L2), ensuring they are equipped for success from day one.
Actively lead, mentor, and develop your team's talent. Foster a culture of technical curiosity and accountability through continuous feedback and clear career pathing.
Oversee all aspects of team management, including creating fair and effective 24X7 shift schedules and on-call rotations, to guarantee complete operational coverage at all times.
As the operational leader, you will own the real-time stability of our platform and command the response when issues arise, with a specific focus on the unique demands of the healthcare insurance TPA space.
Oversee the 24/7 monitoring and health of all critical infrastructure and applications, especially claims adjudication systems, member portals, and provider data networks.
Act as the ultimate point of escalation for major incidents. Lead the incident response and management process with precision, ensuring rapid technical resolution, conducting blameless post-mortems, and communicating clearly to all business and technical stakeholders.
Champion ITIL best practices to move beyond reactive fixes. Analyze incident trends to identify root causes, drive a proactive problem management process, and prevent future occurrences.
You will ensure the NOC operates with the highest standards of security and compliance, providing transparent visibility into performance for leadership and stakeholders.
Ensure end-to-end compliance by acting as the primary owner for NOC compliance, partnering closely with Cybersecurity and Data Governance teams to ensure all tools and procedures are architected to protect PHI/PII and adhere strictly to HIPAA and HITECH regulations.
Deliver actionable insights by developing and maintaining a suite of real-time dashboards for technical teams and automated executive reports for leadership, providing clear visibility into system health, SLO adherence, and overall operational performance.
To be successful in this role, you'll need:
A seasoned leader with a proven track record of building and managing high-stakes technical environments.
A minimum of 8 years in IT infrastructure or operations, with at least 4 years in a leadership capacity (Manager) within a NOC, SOC, and/or SRE team.
Verifiable, hands-on experience architecting, building, and scaling a technical operations center from the ground up in a modern cloud environment.
Bachelor's degree in Computer Science, Information Technology, or a related technical field, or equivalent, demonstrable professional experience. Military experience can substitute for a degree.
Expert-level knowledge of ITIL and SRE principles, with a proven ability to blend both frameworks to optimize operations (Incident/Problem Management, SLOs/SLIs, Error Budgets).
Technical Expertise & Proficiencies
The ideal candidate is a hands-on technical leader with deep, current skills across the modern tech stack.
Expert-level scripting skills (Python, Go), full stack Java and Javascript Framework, and deep experience with Infrastructure as Code and automation frameworks (Terraform, Ansible).
Hands-on architectural expertise with public cloud platforms (AWS, GCP) and extensive experience with containerization and orchestration (Docker, Kubernetes).
Deep understanding of enterprise networking fundamentals (TCP/IP, BGP, DNS, VPNs) and expert-level systems administration skills (Linux, Windows).
Proven experience engineering and utilizing modern observability stacks (e.g., OpenTelemetry, Honeycomb, Prometheus, Grafana, ELK Stack) and managing ITSM platforms (Jira Service Management, PagerDuty, etc).
Preferred Qualifications
Prior experience in the healthcare IT industry.
In-depth knowledge of HIPAA/HITECH regulations and their application to IT infrastructure.
Relevant certifications such as ITIL Foundation/Practitioner, CCNA/CCNP, PMP, or CompTIA Network+.
Relevant AWS Certified Cloud Practitioner/Architect and GCP Cloud Digital Leader/Architect.
