We are seeking a Nurse Case Manager to direct, supervise, and manage the activities of the Case Management Team.
In this role, you will:
Facilitate the Hospital's goals of reducing length of stay, improving patient care, and efficient and effective utilization of resources to ensure an appropriate continuum of care for patients.
Function as a resource person and troubleshooter for the case management team and discharge planning regarding barriers to discharge.
Act as liaison with nursing and medical staff and other clinical departments to integrate the department's functions and services with other aspects of the patient care process.
We require:
Current and Valid NYS Licensure.
BSN required; MSN preferred. CCM preferred.
3-5 years Clinical Experience (Med/Surg acute care).
Minimum of 3 years of direct experience in utilization management, discharge planning, case management or home care.
Minimum of 2 years of leadership or managerial experience in a healthcare environment, preferred.
Proficiency with case management software and EMRs (e.g., Allscripts, Sunrise, or equivalent) preferred.
Current clinical and technical nursing skills.
Knowledge of rules and regulations of child abuse/neglect reporting as appropriate.
Knowledge of the requirements of regulatory agencies and third-party payors.
Demonstrated ability to use word processing, spreadsheet, and/or database programs as required by the position.
Excellent communication and interpersonal skills.
Good problem-solving, decision-making, and judgment skills.

As an RN at Our Lady of Peace, you'll join a team of professionals dedicated to person-centered, compassionate care.
As a leader of our care team, you'll provide direct nursing care to patients in the long-term care setting in accordance with established policies, procedures and protocols of the healthcare organization. In this role you will: Administer nursing care to residents under the direction and supervision of the Unit Manager and/or Nursing Supervisor. Under the supervision of the Unit Manager, the RN working in the facility provides direct resident care, evaluates outcomes and adjusts nursing care processes as indicated to ensure optimal resident care.
Administers and controls narcotics and medications in the facility as directed by nursing management.
Assists physicians with examinations, procedures and other processes related to direct resident care as directed.
Assists physicians, co-workers and families as needed in time of stress or crisis.
Monitors and ensures accuracy of recording on resident medical records. Maintains all required reports, records, statistics as directed by the Unit Manager.
Contributes daily to updating the nursing care plan. Documents significant assessments to update resident records and charts.
The RN monitors and supervises the performance and follows the implementation of resident care by the CNA's on his/her unit.
Enhances professional growth and development within the Nursing Home through participation in educational programs, current literature, in-service meetings and workshops.
Attends all in-services and meetings as required.
The following duties may be reassigned:
Perform supervisory responsibilities on a as needed basis.
Orients, instructs and trains assigned personnel.

Psychiatric Evaluator Emergency Department RN/LMSW
Our team of professional caregivers will do a thorough evaluation to help determine what is causing your distress and how we can best help you. We will check carefully for any underlying medical illness or biochemical origins to your symptoms. We will then work with you, your family, and other health-care professionals to develop a course of treatment that is right for you.
Roles and Responsibilities:
Responsible for collection of comprehensive biopsychosocial data, assessment/determination of lethality/risk, pre-certification of medical care with insurance/managed care entities.
Provide liaison services between emergency department providers and psychiatric providers in determining the disposition of clients in need of psychiatric evaluation
Position is responsible for determination of need for clinical consultation with psychiatrist and medical consultation with emergency department staff to allow timely patient disposition.
If RN, receive physician orders for disposition (If SW transfer to RN for receipt of orders).
Shift: Day/evening flex
Required Skills and Experience:
Previous experience in mental health field, preferably in an acute care inpatient psychiatric setting;
Provide psychosocial assessment and interview of patients, clients, family members and/or persons deemed necessary to obtain the most complete case history in the Emergency Department or ED Flex space;
Complete documentation of psychosocial evaluation to include a formulation of all data leading to a decision for disposition, legal status paperwork, insurance authorization, releases of information appropriate to data collection.
Physical Requirements:
Audio/visual acuity and physical dexterity to interact appropriately with patients, peers, visitors, staff, and outside customers/agencies;
Ability to do long periods of standing, walking, bending and stretching, and ability to change position and body motion quickly;
Ability to lift and assist in lifting, utilizing appropriate body mechanics, without weight restriction;
Ability to participate as a member of the team trained to physically subdue and restrain individuals in behavioral dyscontrol, of all sizes and strengths. This activity includes, but is not limited to, lifting, holding, straining and running.
