Purpose
Under the direction of the Employment and Benefits Manager, the Rep Payee & Financial Coach will manage the finances of 20- 40 clients who are unable to do so in their own best interests due to a disability or other life circumstances. They will work to empower clients on how to manage their finances with the goal of clients becoming financially independent and self-sufficient. The Representative Payee also serves as liaison to the Social Security Administration, Veteran's Administration, and other financial institutions. The Rep Payee & Financial Coach will work with clients in the greater New Haven and Middlesex areas.
Duties and Responsibilities
Collaborate with case managers and Veterans Administration staff as they refer clients who need assistance with managing their finances
Provide guidance with established monthly budgets, provide timely payments and maintain the representative payee software and associated charts
Work directly with clients to assist in credit building, banking, and other financial resources as it relates to their successful development
Coach clients about financial literacy such as budgeting, increasing credit scores, and other aspects of financial management with the goal of the client becoming financially independent
Collaborate closely with the local Social Security Office and the Veterans Administration on specific cases
Attend regularly scheduled supervision, team, shelter or staff meetings as requested
Complete SSA and other reports as requested and in a timely manner
Attend required agency, program and network meetings
Monitor all cases assigned and complete all documentation/charting required
Collaborate with other case managers and work to promote the Representative Payee program city-wide
Maintain strict confidentiality concerning both program applicants and accepted clients while carrying out all duties
Other related duties as assigned
Qualifications
Familiarity with Social Security and Veterans Administration processes
Ability to teach clients to increase their wealth and become financially independent
Knowledge of basic tax preparation and budgeting
Knowledge of government assistance programs and basic child credits
Extremely detail-oriented, punctual, financially literate
Possesses superior interpersonal skills
Excellent organizational and time management skills with a deadline-oriented mindset
Strong written and oral communication skills employing the English language
Proficient in Microsoft Office and other common desktop applications
Able to work with diverse populations
Empathetic and compassionate towards individuals that have experienced homelessness and factors that lead to homelessness
Possesses a strong desire to promote equal access and opportunity for individuals experiencing homelessness and those with disabilities
Familiar with mental health, substance abuse and dual diagnosis issues and dynamics

Purpose
Under the purview of the Permanent Supportive Housing Department, the Permanent Supportive Housing Case Manager provides case management services to adults experiencing homelessness who will be living in long-term supportive housing arrangements. The case manager may assist with finding, securing, and maintaining housing while also providing linkage to medical, mental health, and substance abuse resources, entitlement/state benefits assistance, and employment opportunities.
Duties and Responsibilities
Assist clients with finding, securing, and maintaining housing.
Provide linkage to resources for clients who may be at risk of eviction, in addition to community resources for social, mental health, educational, vocational, entitlements/benefits, and or substance abuse services.
Ability to communicate efficiently and effectively with individuals served.
Able to advocate for clients as well as support self-advocacy.
Must be able to set and maintain boundaries and adhere to confidentiality.
May provide transportation to appointments for clients at the discretion of the case manager on a case-by-case basis.
Coordinate and collaborate with other agencies and social service providers.
Develop mutually reinforcing relationships with landlords and assist those clients that would like to move on from supportive housing with linkage to community housing resources.
Ability to understand and accept different backgrounds while providing a non-judgmental approach to rendering services.
Attend weekly and monthly agency, program and network meetings as well as facilitating tenant meetings with other case managers.
Provide temporary caseload coverage for vacant positions as determined by program needs.
Monitor all cases assigned and complete all documentation/charting required by Columbus House
Expected to complete a variety of high-quality documentation (CARF Accredited, DHMAS funded, Etc.)
Experience with HMIS (Homeless Management Information System) or ability to become proficient in database systems.
Create strength-based person-centered service plans.
Complete all required assessments both electronic and in written form.
Conduct general health and safety checks minimally every 6 months.
Other related duties as assigned.
Qualifications

Purpose

Duties and Responsibilities

Perform manual labor in varied assignments; lift and carry as much as 25 pounds safely; do extensive walking, bending, stooping, reaching; use hands to perform fine manipulation and safely use maintenance tools and equipment.
Must be reliable and capable of accepting supervision.
Knowledge, Skills and Abilities Required

Duties & Responsibilities:
Conduct and coordinate Job Readiness Workshops, employment assessments, job placement, job development, and job search activities that are individualized to the interest and uniqueness of clients.
Conduct field work, phone calls and meetings to develop employment opportunities.
Provide professional leadership in organizing, administering, supervising, and evaluating the vocational needs of participants.
Develop strong employer relationships to gain new resources and/or continue existing resources to create new works sites and resources.
Develop an individual employment plan with tenant that is updated quarterly.
Provide follow-along support to assist clients in maintaining employment.
Establish and operate Computer Lab hours onsite to assist with resume writing, completing job applications, job search, etc.
Maintain client files, employment logs and employer contacts for follow up.
Serve as liaison with outside agencies, i.e., CT Works, Chamber of Commerce, Metro Alliance, and other social and vocational agencies on behalf of clients.

Education and Experience:

The Facilities Technician collaborates with the Facilities Technicians, Environmental Service Workers and with Program Staff to safely implement the repair and maintenance of physical assets, grounds, facilities, equipment and utilities; to monitor the security of occupants and property, and to supervise building occupants and others pursuant to programs and operations to ensure goals are achieved and that the property and improvements are clean, safe, healthy, and may be fully used productively, comfortably, economically and safely, at all times by residents, staff, volunteers, visitors, and others. Duties and Responsibilities
Complete daily, weekly, and monthly checklists, maintain records of scheduled procedures, and identify and recommend improvements pursuant to maintenance, security, and programs and operations.
Complete all required safety training on time as assigned by the company.
Replace and repair building finishes such as ceiling tiles, door hardware, and can paint walls, piping, floors, and equipment etc.
Perform minor repair and maintenance of all physical assets, buildings, facilities, grounds, and utilities.
Inspect physical assets, buildings, facilities, grounds, and utilities and identify items in need of maintenance or repair; ensure assets and facility occupants are safe and secure.
Clean and maintain grounds including but not limited to grass cutting, shrub pruning, snow removal and de-icing.
Reliably and safely transport other staff, materials, supplies and tools as required throughout the state of Connecticut.
Cleanup after yourself when completing a project.
Assist with Environmental Services when required.
Use the EMAINT work order system.
Complete all required documentation and reporting.
Respond immediately to emergency situations and customer concerns.

Purpose

Duties and Responsibilities
In collaboration with CDO create and implement donor stewardship and annual fund plan including events and engaging communications through targeted donor segments for establishing and cultivating donors throughout the giving cycle.
Identify and manage annual fund donor prospects with support from the Donor Relations Specialist focusing on elevating giving (major gifts).
Develop individual cultivation and solicitation strategies, create relevant engagement opportunities that support the progression of the relationship.
Establish planned giving prospect outreach and education through direct mail, newsletters, and direct contact.
Identify potential planned giving prospects and create the individual plan.
Track progress to goal on annual fund.
Conducts client, volunteer, and donor interviews as directed by CDO for inclusion in external communications.
In collaboration with CDO identifies and maintains an updated list and relationships with media and other partners
Assists in the preparation and dissemination of all media alerts and press releases. Coordinates press events. Creates media packets
Assists in preparing staff for speaking engagements and community events
Other related duties as assigned by the Chief Development Officer

Qualifications
Bachelor's Degree and 2+ years of experience in a non-profit Development Office

client requests with patience and respect. Serve as a point of contact for information

Purpose
Under the direction of the Recovery House and Respite Manager, the Resident Support Specialist provides supervision, safety, organization, and program management support to up to 19 homeless clients in need of 24/7 shelter including those in transitional housing and those in need while they recuperate from recent medical procedures. The Resident Support Specialist networks with the greater New Haven shelters and social service system and collaborates with other Columbus House residential staff in order to provide an environment that reflects the core values of Trauma Informed Service: Safety, Trustworthiness, Choice, Collaboration and empowerment.
Qualifications, Knowledge, Skills and Abilities
High School Graduate or equivalent; Demonstrated the ability to manage, motivate and direct consumers.
Complete knowledge base of all aspects of the delivery of residential services for the homeless population.
Excellent organizational and time management skills with a deadline-oriented mindset
Proficient in Microsoft Office and other common desk top applications.
Able to effectively use the local network of substance abuse, mental health, and social services providers.
Familiar with mental health, substance abuse and dual diagnosis issues and group dynamics.

Perform hourly wellness rounds on the floor.

Observe and document medication calls.
Help move client belongings as necessary.
Clean and disinfect rooms as clients leave ensuring rooms are well-prepared for new admissions.
Collaborate with other agencies and social service providers.
Attend agency and program meetings as scheduled.
Complete all required reports/incident logs.
Enter data into computer as directed.
Provide shift coverage to other RS staff during their break times.
Remain on-site to perform essential functions during emergency situations as designated by the Chief Executive Officer.

Purpose

Identify and manage annual fund donor prospects with support from the Donor Relations Specialist focusing on elevating giving (major gifts).
Develop individual cultivation and solicitation strategies, create relevant engagement opportunities that support the progression of the relationship.
Establish planned giving prospect outreach and education through direct mail, newsletters, and direct contact.
Identify potential planned giving prospects and create the individual plan.
Track progress to goal on annual fund.
Conducts client, volunteer, and donor interviews as directed by CDO for inclusion in external communications.
In collaboration with CDO identifies and maintains an updated list and relationships with media and other partners
Assists in the preparation and dissemination of all media alerts and press releases. Coordinates press events. Creates media packets
Assists in preparing staff for speaking engagements and community events

Purpose
Under the purview of the Permanent Supportive Housing Department, the Permanent Supportive Housing Case Manager provides case management services to adults experiencing homelessness who will be living in long-term supportive housing arrangements. The case manager may assist with finding, securing, and maintaining housing while also providing linkage to medical, mental health, and substance abuse resources, entitlement/state benefits assistance, and employment opportunities.
Duties and Responsibilities
Assist clients with finding, securing, and maintaining housing.
Provide linkage to resources for clients who may be at risk of eviction, in addition to community resources for social, mental health, educational, vocational, entitlements/benefits, and or substance abuse services.

The Team Supervisor is responsible for the overall function and implementation of the case management programs, ensuring the highest quality of service within the mission of My Sisters' Place (MSP). This individual also provides guidance, assistance, and oversight to all Community Social Workers, and Peer Specialist, in addition to serving as support to the Director of Program Services. These responsibilities include but are not limited to, maintaining cooperative relationships with community agencies and partners, participating in community efforts such as Greater Hartford CAN meetings, and facilitate case management meetings on a regular basis, managing on-call duties, including scheduling and monitoring data entry.
Supervisory Duties and Responsibilities:
Manage workflow of Community Social Workers and Peer Support Specialists including defining goals, communicating objectives and monitoring performance.
Train new hires Community Social Workers and Peer Support Specialists in all aspects of case management.
Review Orientation Checklist with new hires during Onboarding process.
Train all new Community Social Workers in Smartsheet, DDAP, and/or HMIS.
Oversee and approve individual timecards, including approving PTO and CT Sick Leave requests.
Provide weekly/monthly documented supervision to Community Social Workers and peer specialists and provide supervision feedback.
Monitor and update Smartsheet, Department Mental Health, and Addiction Services information technology database (DDAP) and the Homeless Management Information System (HMIS) for data input/integrity.
Enroll new referrals/matches into HMIS, enter Smartsheet along with assign cases/referrals to CSWs. This would include Discharging clients (as needed) from program rosters in HMIS and Smartsheet.
Attend bi-monthly matching meetings; Greater Hartford CAN as well as attend case conference meetings to provide updates to funders on clients.
Oversee completion of required documentation such as assessments and service plans within the required timeframe per funder and agency policy, including case notes within the 72-hour timeframe and review of case files for updated documentation and accuracy of paperwork filed.
Audit case files as needed per funder requirements; CARF, etc.
Complete formal evaluation on an annual basis and 90-day evaluations for new hires.
Work with Director of Program Services and HR on staff morale and office environment issues/improvement.
Complete and submit documentation of any employee disciplinary/performance issues.
Experience with Conflict Resolution and Mediation.
Assist with Housing Search including not limited to viewing units on client's behalf, transporting clients to view units, etc.
