Purpose
Under the purview of the Permanent Supportive Housing Department, the Rapid Rehousing Case Manager provides case management services and time-limited rental and utility assistance to individuals and families currently experiencing homelessness. The Case Manager assists with finding, securing, and maintaining housing and working with clients to establish financial independence. The Case Manager provides linkages to medical, mental health, and substance abuse resources, entitlement/state benefits assistance, and employment opportunities. The Case Manager monitors progress and elevates clients with higher needs to more intensive housing interventions such as Permanent Supportive Housing.
Duties and Responsibilities
Assist clients with finding, securing, and maintaining housing which includes conducting inspections.
Submit monthly fund requests for rental and utility assistance.
Provide linkage to resources for clients who may be at risk of eviction, in addition to community resources for social, mental health, educational, vocational, entitlements/benefits, and or substance abuse services.
Ability to communicate efficiently and effectively with individuals served.
Able to advocate for clients as well as support self-advocacy.
Must be able to set and maintain boundaries and adhere to confidentiality.
May provide transportation to appointments for clients at the discretion of the case manager on a case-by-case basis.
Coordinate and collaborate with other agencies and social service providers.
Develop mutually reinforcing relationships with landlords and assist clients to become financially independent.
Ability to understand and accept different backgrounds while providing a non-judgmental approach to rendering services.
Attending weekly and monthly agency, program, and network meetings.
Provide temporary caseload coverage for vacant positions as determined by program needs.
Monitor all cases assigned and complete all documentation/charting required by Columbus House
Expected to complete a variety of high-quality documentation (CARF Accredited, Department of Housing, Etc.)
Experience with HMIS (Homeless Management Information System) or ability to become proficient in database systems.
Create strength-based, person-centered service plans.
Complete all required assessments both electronic and in written form.
Other related duties as assigned.
Qualifications
High School graduate required, bachelors preferred
Excellent organizational and time management skills with a deadline-oriented mindset
Strong written and oral communication skills employing the English language
Proficient in Microsoft Office and other common desktop and online applications
Knowledge of and ability to effectively navigate substance abuse, mental health, and social services networks
Familiar with criminal justice barriers and able to engage, motivate, and support the individuals serviced
Familiar with mental health, substance abuse and dual diagnosis issues and group dynamics
Able to work with multiple populations including youth (18-24 years old), adults, and families
Basic knowledge of all aspects of the delivery of person-centered case management
Able to meet all documentation requirements within set timeframes
Knowledgeable of crisis intervention, boundaries, confidentiality, universal precautions, harm reduction and motivational interviewing
Understanding of housing first principles, housing law, tenant rights/responsibilities
Ability to create professional and respectful relationships with clients, outside providers, landlords and property managers
Empathetic and compassionate, with individuals that have experienced homelessness and factors that lead to homelessness
Valid drivers' license with a good driving history
Supervised by: Rapid Rehousing Manager
Schedule: Monday- Thursday 8:30am- 4:30pm

Objective
To support and link consumers to housing, educational, employment, vocational, spiritual, recreational, medical, and treatment resources; to reduce the numbers of men and women in Middlesex County who struggle with mental illness and/or substance abuse which has or can lead them to homelessness.
Qualifications
BA degree required, Masters preferred or four years plus experience working as a case manager in outreach/substance abuse position.
Experienced individual to work with people who are homeless and who have a history of mental health issues and substance abuse and who are currently disengaged from services.
Knowledgeable of outreach activities, engagement skills, basic entitlements, housing, state and community resources and mental health issues.
Must have organizational and network skills to create linkages to medical, mental health, substance abuse treatment, employment, and educational opportunities;
Sensitivity to working with people with various cultural backgrounds and must have knowledge of Trauma Informed and Gender Specific care services.
Computer literate with the ability to learn new software programs required.
Must have valid Driver's License and a good driving record. Harm Reduction skills and PTSD knowledge a plus. Bi-lingual English/Spanish preferred.
Responsibilities
Engage in street outreach to consumers who have histories of homelessness, mental illness, substance abuse, unemployment, incomplete educations, lack of employment skills and are disengaged from services.
Must be comfortable providing a full range of case management services to individuals in the community, i.e. in soup kitchens, shelters and on the streets.
Must team with and support all other outreach case managers and shelter case managers as appropriate.
Attend all required network and staff meetings; meet with Middlesex and Transitional Programs Manager for regular supervision; review caseload on a weekly basis; complete required monthly, weekly and daily charting and movement reports.
Position will be required to collaborate with the CAN Entry and Exit process in Middlesex region.
Must be able to work one evening a week at the local Warming Center during winter season after 9PM.
Complete all required documentation accurately and in timely fashion.
Enter all required client data completely, accurately and timely into Case Worthy computer program.
Other related duties as assigned.

Purpose

Duties and Responsibilities
General: Complete daily, weekly and monthly checklists, maintain records of scheduled procedures, and identify and recommend improvements pursuant to maintenance, security, and programs and operations.

Maintenance: Assist the Manager of Facilities and perform minor repair and maintenance of all physical assets, buildings, facilities, grounds, and utilities.
Inspect physical assets, buildings, facilities, grounds, and utilities and identify items in need of maintenance or repair.
Security: Provide onsite security. Inspect physical assets, buildings, facilities, and grounds to ensure such assets and building occupants are safe and secure.
Minimum Job Requirements
Must have a basic understanding of Columbus Houses' mission, homelessness, substance abuse, and mental health, and the unassisted and unrestricted ability to: move independently throughout Columbus House and Middletown.
Perform manual labor in varied assignments; lift and carry as much as 25 pounds safely; do extensive walking, bending, stooping, reaching; use hands to perform fine manipulation and safely use maintenance tools and equipment.
Must be reliable and capable of accepting supervision.
Knowledge, Skills and Abilities Required
Requires skill, knowledge, and experience, pursuant to: safe work practices, occupational hazards, and safety precautions involved in general property security, grounds and maintenance work.
Safe and efficient operation, use, and maintenance of a variety of basic hand tools and equipment; chemicals and other products used to perform routine maintenance and custodial work.
Recognize failures pursuant to electrical, plumbing, HVAC and other equipment and building components.
Requires skill, knowledge, and experience to execute oral and written instructions; read, write, make, and maintain legible reports and records.

Purpose
Under the direction of the Recovery House and Respite Manager, the Resident Support Specialist provides supervision, safety, organization, and program management support to up to 19 homeless clients in need of 24/7 shelter including those in transitional housing and those in need while they recuperate from recent medical procedures. The Resident Support Specialist networks with the greater New Haven shelters and social service system and collaborates with other Columbus House residential staff in order to provide an environment that reflects the core values of Trauma Informed Service: Safety, Trustworthiness, Choice, Collaboration and empowerment.
Qualifications, Knowledge, Skills and Abilities

Duties & Responsibilities:
Conduct and coordinate Job Readiness Workshops, employment assessments, job placement, job development, and job search activities that are individualized to the interest and uniqueness of clients.
Conduct field work, phone calls and meetings to develop employment opportunities.
Provide professional leadership in organizing, administering, supervising, and evaluating the vocational needs of participants.
Develop strong employer relationships to gain new resources and/or continue existing resources to create new works sites and resources.

Purpose
Under the direction of the Director of Facilities, the Facilities Technician will be the point person in the absence of the Facilities Manager. The Facilities Technician collaborates with the Facilities Technicians, Environmental Service Workers and with Program Staff to safely implement the repair and maintenance of physical assets, grounds, facilities, equipment and utilities; to monitor the security of occupants and property, and to supervise building occupants and others pursuant to programs and operations to ensure goals are achieved and that the property and improvements are clean, safe, healthy, and may be fully used productively, comfortably, economically and safely, at all times by residents, staff, volunteers, visitors, and others. Duties and Responsibilities
Complete daily, weekly, and monthly checklists, maintain records of scheduled procedures, and identify and recommend improvements pursuant to maintenance, security, and programs and operations.
Complete all required safety training on time as assigned by the company.
Replace and repair building finishes such as ceiling tiles, door hardware, and can paint walls, piping, floors, and equipment etc.
Perform minor repair and maintenance of all physical assets, buildings, facilities, grounds, and utilities.
Inspect physical assets, buildings, facilities, grounds, and utilities and identify items in need of maintenance or repair; ensure assets and facility occupants are safe and secure.
Clean and maintain grounds including but not limited to grass cutting, shrub pruning, snow removal and de-icing.
Reliably and safely transport other staff, materials, supplies and tools as required throughout the state of Connecticut.
Cleanup after yourself when completing a project.
Assist with Environmental Services when required.
Use the EMAINT work order system.
Complete all required documentation and reporting.
Respond immediately to emergency situations and customer concerns.
Monthly on-call responsibilities.
Use your own vehicle and have your own tools.
On-call responsibilities.

Knowledge, Skills and Abilities Required
High School Graduate or equivalent.
Able to follow directions and accept supervision.
Skill, knowledge, and experience, pursuant to safe work practices, occupational hazards, and safety precautions involved in general property security, grounds, and maintenance work; safe and efficient operation, use, and maintenance of a variety of basic hand tools and equipment; chemicals and other products used to perform routine maintenance and custodial work.
Assist with troubleshooting and repairs of buildings and installed systems to include HVAC, plumbing systems, kitchen equipment, roofs, drains, shop, and grounds equipment.
The interpersonal skills required to maintain effective working relationships with supervisor, co-workers, building occupants, volunteers, public officials, visitors, etc.; and the ability to work independently or cooperatively as a member of a team.
Excellent organizational and time management skills with a deadline-oriented mindset.

Valid Driver's license with a good driving record.

Purpose

Duties and Responsibilities
In collaboration with CDO create and implement donor stewardship and annual fund plan including events and engaging communications through targeted donor segments for establishing and cultivating donors throughout the giving cycle.
Identify and manage annual fund donor prospects with support from the Donor Relations Specialist focusing on elevating giving (major gifts).
Develop individual cultivation and solicitation strategies, create relevant engagement opportunities that support the progression of the relationship.
Establish planned giving prospect outreach and education through direct mail, newsletters, and direct contact.
Identify potential planned giving prospects and create the individual plan.
Track progress to goal on annual fund.
Conducts client, volunteer, and donor interviews as directed by CDO for inclusion in external communications.
In collaboration with CDO identifies and maintains an updated list and relationships with media and other partners
Assists in the preparation and dissemination of all media alerts and press releases. Coordinates press events. Creates media packets
Assists in preparing staff for speaking engagements and community events

Demonstrates understanding and evaluates clients by acknowledging individual psychological needs, developmental disability, ethnic consciousness, and trauma history.
Assist clients with the development of individual treatment plans and goals to provide support for achievement to maintain housing, mental health treatment, and/or other services provided.
Collaborate with Housing Specialist to assist with client during housing search, recertification and housing inspections.
Serve as a liaison to provide mediation and advocacy with client/landlord on behalf of the client to develop a workable plan to obtain and/or maintain housing.
Provide crisis intervention to ensure the clients well-being and stability to remain in the community.
Monthly management of updating data into Smartsheet and the Homeless Management Information System HMIS for data input/integrity.
Assist clients with the coordination of health services and, as necessary, schedules client appointments for and provides transportation to physicians, clinicians, and other health care professionals.
Maintain complete and up-to-date client files to assure overall compliance with agency standards utilizing the assigned programs Master List.
Act as liaison with other agencies, i.e., DSS, SSI, HOPWA/ACT, and other social and vocational agencies to advocate for and assist clients.
Attend monthly HOPWA CAN, Greater Hartford CAN, and MMW CAN meetings and participate in Case Management meetings, conferences, seminars, and trainings as required.
Mediate conflicts between providers, family/guardians, clients, and the agency, per the client's needs.
Develop or maintain familiarity with community resources and referral processes necessary to access these resources.
Advocate and/or assist client in receiving services identified in the planning process. This includes working closely with the Housing Specialist (internal) and assisting as needed with housing search and completing housing applications.
Follow proper intake & discharge procedures, when a client is discharged, with attention to recordkeeping and referrals.

Purpose

Knowledge base of all aspects of the delivery of residential services for the homeless population preferred.
Must be aware of mental health, substance abuse, and dual diagnosis issues; knowledgeable of the area network providers; computer literate.

Duties/Responsibilities:
Enter client case notes using the GIRP format, within the 72-hour time frame, complete six month and annual assessments and service plans to be submitted every six (6) months.
Perform lead-based paint assessments on behalf of each client as required.
Provide comprehensive planning and active support for the care, treatment, and safety of the agency's clients.
Demonstrates understanding and evaluates clients by acknowledging individual psychological needs, developmental disability, ethnic consciousness, and trauma history.

Attend monthly HOPWA CAN, Greater Hartford CAN, and MMW CAN meetings and participate in Case Management meetings, conferences, seminars, and trainings as required.
Mediate conflicts between providers, family/guardians, clients, and the agency, per the client's needs.
Develop or maintain familiarity with community resources and referral processes necessary to access these resources.

Duties/Responsibilities:
Establish a positive rapport with the client/consumer to assess and build on the strengths and resources to build an effective network of supports to draw upon during their recovery journey.
Caseload of 15-20 clients who are currently experiencing homelessness or at risk of homelessness and have an Opioid Use Disorder diagnosis, or of another substance use disorder with a high risk of overdose.
Conduct home and community visits to assist the client/consumer with finding adequate housing, connect with community services/providers and benefits.
Develop and implement a recovery plan of short and long-term goals with each client/consumer that addresses identified needs in areas such as housing and lease, arrears and debts, income and benefits, support services and resources, health, parenting and child services to work on strategies and identify resources.
Evaluates the clients/consumers psychological or behavioral needs, developmental abilities, cultural influences, and trauma history which would be related to their recovery.
Monitor and assess documented recovery plans quarterly to guarantee that the goals of the recovery plans are achieved.
Connect clients/consumers with identifying services and supports to increase participation in community activities, interpersonal, family, and social engagements.
Enter client/consumer case notes into HMIS to document encounters/engagements within the 72-hour time frame, complete the HERO acuity and assessment to guide the recovery plan to be submitted every six (6) months.
Collaborate with property management, landlords and/or rental assistance administrator to assist with completion of recertifications, housing inspections, along with identify and reduce risks to housing stability, providing interventions to remedy any lease violations.
Linkage with eviction prevention to support the client/consumer with maintain housing.
Address crises and coordinating emergency interventions.
Collaborate with treatment teams to ensure coordination of care to assess if it is best suited for client/consumers recovery.
Attend initial appointments with clients/consumers when with new service providers to support established effective working relationships. Seen as a “Warm Hand-off”.
