Initiates, interprets and makes necessary changes in the plan of care; promotes communication with the patient, physician family and other home health members. 3. Documents clearly and concisely care and services rendered on each visit, patient's reaction and therapist's observation. 4. Provides weekly progress notes prior to the required due to date. 5. Teaches home treatment program to patient and family. 6. Assists family in achieving safety techniques as indicated by Occupational Therapy program. 7. Documents and reports situations in which Occupational Therapists home health services should be discontinued. 8. Maintains records of charges for supplies utilized. 9. Communicates and cooperates with departmental personnel, supervisors, and physicians to promote effective working conditions and relations. 10. Reviews and revises the patient profile, treatment goals and plan of care. 11. Attends and participates in educational programs.
