The planning, implementation and supervision of individualized therapeutic activity programs as part of an overall “active treatment” for a patient diagnosed with a psychiatric illness, i.e., the use of sewing activities which require following a pattern to reduce confusion and restore reality orientation in a schizophrenic patient. d. The planning and implementation of therapeutic tasks and activities to restore sensory-integrative function, i.e., providing motor and tactile activities to increase sensory input and improve response for a stroke patient with functional less resulting in a distorted body image. e. The teaching of compensatory techniques to improve the level of independence in the activities of daily living, i.e., teaching a patient who has lost the use of an arm how to peel potatoes and chop vegetables with one hand, teaching an upper extremity amputee how to functionally utilize prosthesis, or teaching a stroke patient new techniques tot enable him/her to perform feeding, dressing and other activities as independently as possible. f. The designing, fabricating and fitting of orthotic and self-help devices, i.e., making a hand splint for a patient with arthritis to maintain one hand in a functional position or constructing a device which would enable an individual to hold a utensil and feed himself/herself independently. g. Vocational and prevocational assessment and training. 2. Initiates, interprets and makes necessary changes in the plan of care; promotes communication with the patient, physician family and other home health members. 3. Documents clearly and concisely care and services rendered on each visit, patient's reaction and therapist's observation. 4. Provides weekly progress notes prior to the required due to date. 5. Teaches home treatment program to patient and family. 6. Assists family in achieving safety techniques as indicated by Occupational Therapy program. 7. Documents and reports situations in which Occupational Therapists home health services should be discontinued. 8. Maintains records of charges for supplies utilized. 9. Communicates and cooperates with departmental personnel, supervisors, and physicians to promote effective working conditions and relations. 10. Reviews and revises the patient profile, treatment goals and plan of care. 11. Attends and participates in educational programs. May also be required to attend utilization review, case conferences and other committee meetings. 12. Adheres to agency policies and procedures. 13. Assists in fulfilling the philosophy and objectives of the agency. 14. Maintains strict confidentiality of patient information as stated in Agency policy. 15. Audits Occupational Therapy documentation in medical record for appropriateness.
Minimum Qualifications:
1. Graduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association; or has two years previous experience as an occupational therapist and has achieved a satisfactory grade on a proficiency exam conducted, approved or sponsored by the U.S. Public Health Service. 2. A minimum of one year experience is preferred.
