The Registered Medication Associate (RMA) role provides high quality care and services to meet the overall needs of residents in a caring, consistent, individualized, and efficient manner. He or she ensures that residents receive comprehensive care and services by assisting them in the administration of medications, in accordance with state licensing and regulatory standards. The Registered Medication Associate is a Culture Ambassador who will model the core values of the company, leading by example, and ensure all team members exemplify the core values of the company.

Qualifications
• This employee must have successfully completed course work and examinations to obtain certification as a Resident Care Associate.
• The RMA must obtain continuing education credits as required by the state to maintain certification as an RMA and must provide evidence of continued certification to the community in a timely and accurate manner.
• The RMA must possess knowledge of safe medication administration techniques and demonstrate this knowledge through a competency skills evaluation.
• CPR and First Aid certified or willing to obtain as per company policy.
• The RMA must understand and practice resident confidentiality.
• The RMA must have the ability to work weekends and flexible hours as needed for resident care and services.
• The RMA must have the ability to show sincere compassion and demonstrate genuine concern for the physical and emotional needs of seniors and their families.
• Must be proficient in basic office equipment, including a personal computer, for the purpose of accomplishing and maintaining a high level of job performance.

Areas of Primary Responsibility
• Assists in monitoring and safe delivery of the Medication Management Program.
• Adheres to all established policies and procedures of Commonwealth Senior Living and state regulatory standards when administering medications.
• Administers and accurately records the administration of medications for residents as prescribed by the physician.
• Presents medication to residents and observes ingestion or completion of other application or administration techniques.
• Observes, records, and reports complications, symptoms of adverse actions or side effects of medications and obtains input from the Resident Care Director, as indicated, to ensure the optimum health and safety of the resident.
• Determines and records effectiveness of medications in a timely and accurate manner.
• Documents refusals of medications and reasons why prescribed drugs are not administered as per physician order.
• Stores medications in a safe and accurate manner.
• Monitors medications to ensure adequate accountability measures are taken when medications are ordered, received into the community, administered, and reported off at change of shift.
• Documents on the Daily Shift to Shift Communication Log each shift with resident and medication updates, changes, concerns, and follow up.
• Demonstrates safe handling of controlled substances including appropriate shift counting and reporting of discrepancies or concerns to the Resident Care Director.
• Routinely inspects, cleans, audits, and monitors equipment and supplies to ensure resident safety. Reports any equipment or supply issues to the Resident Care Director for correction or repair.
• Obtains, records, and reports vital signs as indicated during the administration of medications.
• Records and restocks medication inventories, as needed, and requested.
• Re-orders resident medications from pharmacy, as needed, and requested.
• Documents pertinent information in the resident record, completes incident reports as indicated.
• Demonstrates awareness of proper infection control practices by using Universal Precautions and following policies and procedures of the company, including OSHA's Blood-Borne Pathogen Standard.
• Communicates with residents, staff and others in an accurate and factual manner and seeks clarification from or refers questions to Resident Care Director, when indicated.
• Prioritizes and provides verbal encouragement and support to residents.
• Assist in maintaining a safe and clean environment.
• Maintains a positive and professional demeanor toward residents, families, and associates.
• Participates in and attends all required in-service training and education programs as scheduled.
• Acts as a shift supervisor when appropriate or as directed.
• Maintain regular communication with the Resident Care Director.
• Perform other duties as assigned or as listed in the Resident Care Associate job description.
Physical/Sensory Requirements
The RMA must be able to lift, stoop, carry, turn, and assist high-risk residents, push medication carts and be willing and able to work a flexible work schedule. The RMA must be able to sit, stand, bend, and move continuously during work hours. Must be able to lift and/or carry 50 to 75 pounds. This position is considered a medium risk for exposure to COVID, as it provides direct care to potential COVID-positive residents.

Position Summary:
Under the supervision of the Emergency Department Pharmacist, the Medication History Technician will obtain medication histories through documentation review and patient/family/significant other interviews for patients arriving via the Emergency Department to be admitted to the inpatient units.
Key Duties and Responsibilities:
Obtain home medication histories by interviewing the patient/family/significant other for patients arriving through the Emergency Department to be admitted to the inpatient units.
Utilize the external prescription history, patient's electronic medical record (EMR), and the patient's pharmacy to verify the correct medication history. This includes but is not limited to prescription, non prescription, herbals, and supplements.
Compile all of the information obtained to determine a complete medication history including the medication name, dose, frequency, and adherence.
Document the correct medication history in the EMR.
Positively interacts with patients, other hospital guests, and hospital staff to enhance the quality of service provided.
Minimum Requirements:
High School Diploma or equivalent
CT Pharmacy Technician registration. CPhT is preferred but not required.
1 - 2 years of retail or medication reconciliation experience preferred
An appropriate level of understating and training in medical terminology and medication related terms. Current/fluent in pharmacy terminology, medications and dosages.
Proficient with using a computer, typing, and organization.
Required to concentrate for extended periods of time, provide high output of work on short notice, establish priorities, make quick accurate decisions.
Preferred Skills:
Be able to prioritize competing demands, work independently, and show flexibility and teamwork.

Under the direct supervision of a pharmacist, the Pharmacy Technician II prepares and fills routine orders for prescribed medications; restocks automated dispensing machines; repackages medications; compounds both sterile and non-sterile medications.
Primary Duties and Responsibilities
Abides by recommendations from regulatory and advisory groups regarding safe medication principles including look alike sound alike and high alert drugs. Responds appropriately to medication occurrences. Understands and takes appropriate action when a medication occurrence is identified. Complies with internal reporting requirements for medication occurrences using the Patient Safety Event system. Participates in meetings and on committees and represents the department and hospital in community outreach efforts.
Assists pharmacists in obtaining patient specific information as appropriate for safe medication management. Under direct supervision participates in the exchange of information with appropriate healthcare professionals and staff.
Ensures that medications are properly and safely stored throughout the hospital. Stores medications under conditions necessary to ensure stability. Secures medications in accordance with the hospital's policy law and regulation. Refills and replaces medications in automated medication dispensing machines throughout the hospital. Troubleshoots and services automated medication dispensing machines as needed. Appropriately labels all medications and chemicals used to prepare medications with contents expiration dates and appropriated warnings. Routinely inspects all medication storage areas according to hospital's policy. Adheres to all laws regulations and hospital policies regarding the management and handling of recalled discontinued unused expired or returned medications. Inspects returned medications for integrity before returning to pharmacy stock. Segregates outdated or recalled medications from pharmacy stock.
Participates in the evaluation of the hospital's medication management system for risk points and identifies areas to improve safety.
Safely and accurately prepares all medications in the most ready to administer form (including hazardous medications and sterile products) using proper techniques calculations and appropriate precautions. Labels and packages medications according to hospital policy applicable laws and regulations and standards of practice to ensure safe accurate administration (i.e. labels medications with the drug name strength quantity beyond use date patient name patient location and directions for use). Maintains documentation for all repackaged medications as required by departmental policy.
Selection and Procurement: Ensures that medications are selected and procured based on departmental criteria. Demonstrates a working knowledge of the formulary list and the formulary process. Adheres to policies and procedures regarding the procurement of non-formulary medications. Adheres to policies and procedures that address medication shortages outages emergency and disaster situations. If applicable adheres to policies and procedures regarding the 340B Drug Pricing Program.
Stores controlled substances in such a manner to prevent diversion in accordance with state and federal laws and regulations. Accounts for all unused expired or returned medications according to law. Utilizes automation where applicable.
Transports medication to patient care units placing in the designated delivery locations within established departmental standards for turnaround times.

The Specialty Pharmacy Technician assists the Pharmacist(s) in all areas of pharmacy operations including data entry product dispensing and packing of prescription orders in a high-volume fulfillment environment. The position requires routine interaction with clinic liaisons, pharmacists, and providers as well as specialty pharmacy patients with complex medication needs requiring high touch care and follow up. Primary Duties and Responsibilities
Contributes to the achievement of established department goals and objectives and adheres to department policies procedures quality standards and safety standards. Complies with governmental and accreditation regulations.

Assists with placing the daily order for drugs and supplies; receives checks and puts away the daily order; follows established inventory management procedures; participates in physical inventories and cycle counts.

Prepares prescriptions for packaging for daily shipment. Creates invoices and shipping labels; ensures compliance with HIPAA requirements; and follows all cold chain shipping standards.
Processes refills and performs data entry of new prescriptions including demographics insurance plans and prescription information under the supervision of a pharmacist; brings to the attention of a pharmacist any claims requiring overrides or interventions; troubleshoots insurance claim rejects; and follows up on problem prescriptions.
Communicates regularly with the teams in the clinics and the support center about the status of benefits pre-authorizations and financial assistance; as well as copay or other issues to facilitate timely processing of prescription orders and deliveries.

Prepares prescriptions for dispensing: counts and verifies medication labels vials appropriately and prioritizes waiters.
Processes cash check credit card and payroll deduction transactions; secures appropriate signatures; ensures compliance with HIPAA requirements; prepares delivery tickets; reconciles delivery transactions; performs end of day register procedures; runs daily cash report and completes deposits as assigned.
Processes refills and performs data entry of new prescriptions including demographics insurance plans and prescription information under the supervision of a pharmacist and according to policy; brings to the attention of a pharmacist any claims requiring overrides or interventions; troubleshoots insurance claim rejects; and follows up on problem prescriptions.
Receives checks and puts away the daily order; returns medication to the shelf after filling; follows established inventory management procedures; participates in physical inventory process at intervals determined by management.
Greets customers immediately either in person or within three rings via telephone; performs intake of refill requests and new prescriptions gathers patient demographics and insurance information; clearly marks and prioritizes waiting customers. Monitors the waiting room by making sure they know who is waiting and whom they are waiting for.

• Adheres to all established policies and procedures of Commonwealth Senior Living and state regulatory standards when administering medications.
• Administers and accurately records the administration of medications for residents as prescribed by the physician.
• Presents medication to residents and observes ingestion or completion of other application or administration techniques.
• Observes, records, and reports complications, symptoms of adverse actions or side effects of medications and obtains input from the Resident Care Director, as indicated, to ensure the optimum health and safety of the resident.
• Determines and records effectiveness of medications in a timely and accurate manner.
• Documents refusals of medications and reasons why prescribed drugs are not administered as per physician order.
• Stores medications in a safe and accurate manner.
• Monitors medications to ensure adequate accountability measures are taken when medications are ordered, received into the community, administered, and reported off at change of shift.
• Documents on the Daily Shift to Shift Communication Log each shift with resident and medication updates, changes, concerns, and follow up.
• Demonstrates safe handling of controlled substances including appropriate shift counting and reporting of discrepancies or concerns to the Resident Care Director.
• Routinely inspects, cleans, audits, and monitors equipment and supplies to ensure resident safety. Reports any equipment or supply issues to the Resident Care Director for correction or repair.
• Obtains, records, and reports vital signs as indicated during the administration of medications.
• Records and restocks medication inventories, as needed, and requested.
• Re-orders resident medications from pharmacy, as needed, and requested.
• Documents pertinent information in the resident record, completes incident reports as indicated.
• Demonstrates awareness of proper infection control practices by using Universal Precautions and following policies and procedures of the company, including OSHA's Blood-Borne Pathogen Standard.
• Communicates with residents, staff and others in an accurate and factual manner and seeks clarification from or refers questions to Resident Care Director, when indicated.
• Prioritizes and provides verbal encouragement and support to residents.
• Assist in maintaining a safe and clean environment.
• Maintains a positive and professional demeanor toward residents, families, and associates.
• Participates in and attends all required in-service training and education programs as scheduled.
• Acts as a shift supervisor when appropriate or as directed.
• Maintain regular communication with the Resident Care Director.
• Perform other duties as assigned or as listed in the Resident Care Associate job description.

Physical/Sensory Requirements
The RMA must be able to lift, stoop, carry, turn, and assist high-risk residents, push medication carts and be willing and able to work a flexible work schedule. The RMA must be able to sit, stand, bend, and move continuously during work hours. Must be able to lift and/or carry 50 to 75 pounds. This position is considered a medium risk for exposure to COVID, as it provides direct care to potential COVID-positive residents.
