Within Aeroflow, the Mom and Baby Education Provider Group is composed of over 50 providers who provide support for patients through group classes.

Providing education and care through our virtual platform improves mothers' and babies' health outcomes, including increasing rates of breastfeeding, and enhances satisfaction in our patients' mothering experiences.

WHAT YOU'LL DO
Be Patient-Focused
Teach and facilitate classes on a broad range of topics in the pregnancy and postpartum period.

Chart visits and maintain accurate records according to payor requirements, per guidelines provided by Aeroflow.
Support and advocate for the normal physiologic functions involved in the maternal-infant process.

As a Speech Language Pathologist (SLP), you are key to helping us provide superior patient care. You will work to provide pathology service on an intermittent basis to clients in their homes in accordance with the physician's orders.
We are looking for a team player who is eager to learn, passionate about providing quality care, and has a proven track record of compassionate care.
What you will do in this role:
Evaluate the client's level of functioning and recommend mechanisms to enhance the client's hearing ability.
Provide initial and ongoing comprehensive assessments of the client's needs, including Outcome and Assessment Information Set (OASIS) assessments.
Establish a Speech Therapy treatment plan in consultation with the physician and revise it with the approval of the physician as necessary.
Assist in development and implementation of the interdisciplinary client care plan as it pertains to Speech Therapy.
Assure that the physician's orders are appropriate and discuss necessary changes.
Communicate with other team members and, when appropriate, instructs them in speech pathology techniques that they may use while working with the client.
Teach, supervise, and counsel the family and client in the total Speech Therapy program and other related problems of the client at home.
Evaluate the client, his/her significant other(s) and the home situation to determine what instruction will be required, what assistance will be available for the family in caring for the client, and what other agency and community services will be required.
Make arrangements for out‐client services for procedures which cannot be given in the home.
Record and report to the physician the client's reaction to treatment and /or any changes in the client's condition.
Participate in the development of periodic revision of the physician's plan of treatment keeping him/her informed of the client's status, and obtaining additional orders if necessary.
Communicate with the physician at least every 30 days or whenever changes occur regarding the client's status and obtain additional orders, if necessary.
Evaluate the effectiveness of speech pathology intervention and update the client care plan as needed.
Confirm, on a weekly basis, the scheduling of visits with the Director of Nursing in order to coordinate the client's visit schedule.
Participate in agency activities, case conferences and meetings, when appropriate.
Write clinical/progress notes on the date of the visit and incorporate the same in the client's record within 72 hours.
Qualifications
Master's Degree in Speech‐Language Pathology with a Bachelor Degree in the Field of Speech and Hearing Therapy preferred

Evaluate the home environment and make appropriate recommendations.

Qualifications

Minimum two years of varied work experience as a registered nurse with preferably one of the two years in either an acute, oncology, home health and/or hospice setting.

Job Summary
As an Occupational Therapist (OT), you are key to helping us provide superior patient care. You will work to provide skilled Occupational Therapy services in accordance with the client's plan of care to provide skilled occupational therapy service on an intermittent basis to clients in their homes in accordance with the physician's orders.

Instruct clients in care and use of wheelchairs, braces, splints, and prosthetic and orthotic devices.

Job Summary
As a Physical Therapist (PT), you are key to helping us provide superior patient care. You will work to provide skilled Physical Therapy services in accordance with the client's plan of care to include helping clients to relieve pain, develop or restore functional and maintain maximum performance, using physical means, such as exercise.

What you will do in this role:
Provide initial and ongoing comprehensive assessments of the client's needs, including Outcome and Assessment Information Set (OASIS) assessments
Establish a treatment plan in consultation with the physician and revise it with the approval of the physician as necessary
Assist in development and implementation of the interdisciplinary client care plan as it pertains to Physical Therapy
Assist the physician in evaluating clients by applying diagnostic and prognostic muscle, nerve, joint and functional ability tests
Treat clients to relieve pain, develop or restore functional and maintain maximum performance, using physical means, such as exercise
Consult and familiarize staff regarding the established plan
Teach, supervise, and counsel the family and client in the physical therapy program
Instruct client in care and use of wheelchairs, braces, crutches, canes and prosthetic and orthotic devices
Provide the equipment required to carry out the plan. Arrange for out‐client service for procedures which cannot be given at home
Record on a timely basis all evaluation data, treatments, and client's response to therapy intervention; Also records quality assurance data
Prepare an assessment of the client's condition and periodic evaluations and summaries. Supervise Physical Therapy Assistants and paraprofessionals as per agency policy
Record and report to the physician the client's reaction to treatment and /or any changes in the client's condition
Communicate with the physician at least every 30 days or whenever changes occur regarding the client's status and obtain additional orders, if necessary.
Communicate with other team members (including Home attendants) and, when appropriate, instructs them in certain phases of physical therapy that they may use while working with the client
Write clinical/progress notes on the date of the visit and incorporate the same in the client's record within 72 hours.
Participate in case conferences, physical therapy staff meetings, agency activities and in‐service programs as necessary
Qualifications
Must be a graduate of a Physical Therapy school approved by the Council of Medical Education of the A.M.A. in collaboration with the American Physical Therapy Association
Must be licensed or registered in Virginia
Must have at least one year of institutional/home health experience
Must have current CPR certification
Working knowledge of Medicare regulations and guidelines is a plus
You must have a minimum of one year experience as an PT, home health experience is strongly preferred
Must be proficient using PT equipment
You must be able to provide two references
You must be able to pass OIG checks, Virginia State Police background check, and national background checks, and able to verify employment eligibility with necessary documentation
Reliable transportation, proof of valid car insurance, and a valid driver's license
