This role is responsible for conducting comprehensive diagnostic assessments, determining appropriate levels of care, and ensuring a smooth transition into services. This position is vital to ensuring clients feel heard, supported, and connected to the right resources from day one. Key ResponsibilitiesClinical Assessment & Intake Conduct initial mental health and substance use diagnostic assessments using evidence-based tools. Evaluate client history, presenting concerns, risk level, medical needs, and social determinants of health. Determine level-of-care recommendations and collaboratively develop initial treatment plans. Facilitate crisis assessments as needed, including safety planning and referrals for higher-level care. Care Coordination & Communication Communicate assessment results and clinical recommendations with clients, families, and treatment teams. Complete referral coordination for internal services (therapy, groups, psychiatry) and external supports when indicated. Ensure accurate, timely documentation in accordance with Medicaid, HIPAA, state, and federal guidelines.
