PHP Telehealth is currently recruiting Nurse Pracitioners with a passion for delivering high quality, virtual care. Candidates must be licensed in one or more of the following states: Ohio, Indiana, and/or West Virginia.
This is a remote position, working part time from anywhere in the United States. As an Advanced Practice Provider, you will be part of an exciting new model of care utilizing innovative digital health technologies to achieve better outcomes.
The Advanced Practice Provider provides an enriched level of medical care with a goal of increasing wellness, preventing illness, and improving outcomes through early detection, wellness visits, and timely diagnosis and/or treatment. The APP utilizes innovative technologies to treat in place when clinically appropriate.
JOB DUTIES & RESPONSIBILITIES
Provide remote care to patients from the comfort of their own residence/facility
Utilize advanced digital health technologies to diagnose and care for patients
Work closely with client staff to ensure proper care and handoffs are provided
Identify/anticipate key health issues for patients in multiple specialties like: Cardiology, medication management and reconciliation, Pulmonary and Diabetes.
Record visit information directly into client EMR systems
Provide remote employee health evaluations for minor ailments
QUALIFICATIONS KNOWLEDGE/SKILLS & ABILITIES
Must have Advanced Practice Certification
Must be licensed in one or more of the following states: Ohio, Indiana, West Virginia
Excellent communication skills
Good technology aptitude
3+ years of experience
PREFERRED
Primary care/internal medicine
ED or Urgent Care experience
SNF experience
Telehealth experience

The Pediatric NP Provider Hospice/Palliative Care has two primary responsibilities.
He/she will provide NP-appropriate service to patients enrolled in hospice and may also assist in staffing pediatric palliative care consultation services that function as a program of MJHS Hospice and Palliative Care
He/she may work in a community, long-term care environment, or hospital
He/she has specialist skills in symptom management, communication including goal setting discussions, and hospice eligibility review, and he/she may work independently or as part of a treatment team
In all settings, he/she will provide in-person visits or telehealth visits, as appropriate. Care will consistently comport with best practices, including history-taking, appropriate examination, formulation and care planning, treatment recommendation or implementation, communication, and documentation
In the role as hospice NP, he/she will coordinate with the team physician and manager and will be responsible for face-to-face evaluations for hospice eligibility, and the assessment and management of hospice patients in need of exigent medical care
He/she will also contribute to the training of the hospice team, providing support for the interdisciplinary team, as needed
He/she will also help build a teaching curriculum on pediatric aspects of hospice care that will be of use to our hospice workers who are engaged in pediatric care through cross coverage or for when young adolescents are incorporated into adult hospice teams
In providing both direct care and support to other clinicians, the NP will use an evidence-based approach to management and to the use of materials and policies/procedures
Other activities related to quality improvement projects may also be required, as appropriate
Master's degree in nursing and graduate of an approved NursePractitioner
Program required
License/Certification: NYS Certified NursePractitioner (NP)
Minimum of 1-year clinical experience as a NP provider preferred
