Direct Clinical & Case Management Services * Provide ongoing individual counseling and crisis intervention for clients experiencing psychological or adjustment difficulties. * Develop and facilitate group counseling programs. * Conduct telephone, face-to-face, and home visits as required for monitoring and support. Local travel is required based on caseload complexity and state regulations. * Employ Motivational Interviewing and clinical guideposts to educate, support, and motivate clients toward positive change and personal independence, assisting with the development of daily living skills. * Assess and address barriers to care and facilitate linkage with crisis/emergency services when necessary. * Maintain an active, ongoing caseload for regular outreach, management, and monitoring of care plan effectiveness. * Deliver services including CPST (Community Psychiatric Support Treatment) and TBS (Therapeutic Behavioral Services). Coordination & Collaboration * Ensure the provision of care is safe, timely, effective, efficient, and client-centered. * Work closely with therapists, case managers, CHW supervisors, clients, and family members to coordinate and integrate services, including behavioral health and long-term services and supports (LTSS), to enhance continuity of care. * Participate in regular clinical supervision for case direction and ISP updates. * Act as a resource for partner agencies by providing clinical input, assessment, education, and consultation on mental/behavioral health issues. * Offer consultation, education, and outreach to community agencies, parents, teachers, and other groups to improve skills in working with children, adolescents, adults, and families. Administrative & Professional * Maintain accurate and timely clinical records and ensure proficiency in electronic record keeping systems. * Ensure confidentiality and security of Protected Health Information (PHI) in accordance with established agreements and regulations. * Attend and actively participate in administrative staff meetings, trainings, and conferences. * Perform other clinical and administrative assignments as directed by the supervisor, including providing clinical outpatient services to children and adolescents when needed. Education & Credentialing * Degree in Human Services or a closely related field is required. * Must be credentialed and in good standing in a behavioral health domain, such as: *
SW-T (Social Worker Trainee) * MFT (Marriage and Family Therapist) * LISW (Licensed Independent Social Worker) * LPCC (Licensed Professional Clinical Counselor) * Must possess or be eligible to obtain an NPI number and Ohio Medicaid ID. * Background check/Credentialing checks are required prior to employment. Experience & Knowledge * Behavioral Health (BH/SUD) experience (1 year preferred). * Knowledge of Medicaid billing and coding is essential. * Familiarity with the delivery of CPST (Community Psychiatric Support Treatment) and TBS (Therapeutic Behavioral Services). * Demonstrated knowledge of Crisis/De-escalation techniques. * Proficiency in using electronic record keeping systems.
