Essential Functions
Implement and manage health education strategies, interventions and programs.

Facilitate group counseling and education sessions within the scope of secondary prevention and document in Electronic Health Record.
Re-engage CompDrug patients who are not actively participating in treatment. Utilizes evidence-based outreach and engagement strategies and ensures swift connection to appropriate CompDrug staff to support the patient's return to treatment
Manage and maintain program inventory and supplies.

Required Experience:

Bridges is a voluntary program that provides in-home guidance and support for 18-21 year-old young adults who have transitioned out of foster care. Participants attend school, work, participate in an employment program, or have a medical condition that prevents them from such activities.
This position requires local travel and flexibility to work a non-traditional schedule.
Our Nationally recognized Treatment Foster Care Program was developed almost 40 years ago to provide intensive treatment services for children and adolescents in a family setting. Identifies members that would benefit from an alternative level of service or other waiver programs.
May also serve as mentor, subject matter expert or preceptor for new staff, assisting in formal training of associates and may be involved in process improvement initiatives.
Responsible for reporting critical incidents to appropriate internal and external parties such as state and county agencies (Adult Protective Services, Law Enforcement).
Assists and participates in appeal or fair hearings, member grievances, appeals and state audits.

INTERNSHIP SUMMARY: Under the administrative/clinical supervision of the Director of Clinical Services/ Field Instructor, the Social Work intern will be responsible for providing social service functions in a healthcare setting. The intern will complete assessments and case management to patients and families. Field hours are flexible, some evenings and/or weekends may apply. SWT or LSW required.
RESPONSIBILITIES INCLUDE:
Perform psychosocial assessments on all patients to determine social service needs, and plan of care.
Evaluate patient data, develops and implement a plan of care for the patient.
Communicate with physicians regarding social service needs of the patient.
Participate in plan of care for each patient; work cooperatively as a member of the interdisciplinary treatment team; completes progress notes and discharge plans in a timely fashion.
