We are looking for a dynamic individual who can for provide services and support to the post-pandemic recovery effort for the residents within the city of Albany by actively linking them to needed government benefits, services, and/or goods. With the support of the team leader and other Wellness Advocates, we will remove barriers and achieve positive outcomes for the health and well-being of marginalized, disadvantaged, and underserved city of Albany residents by actively linking them to needed government benefits, services, and/or help them obtain essential goods. This is a short-term service delivery model with the goal of resolving referrals within six to eight weeks.
Essential Duties And Responsibilities include the following. Other duties may be assigned. Employees are expected to be awake and alert while working.
Maintain a caseload of 10 12 clients and meet service needs within a six to eight week time frame.
Receive referrals and collaborate with referral source, as needed.
Rapidly engage clients and complete intake paperwork including the Social Determinants of Health questionnaire.
Effectively interface with government offices, social service providers and other community resources to actively link clients to services, assist in their application for benefits, and to obtain concrete goods.
Maintain positive communication with clients and provide active assistance including transportation, attending meetings, complete applications, etc.
Advocate for client with providers, as well as troubleshoot barriers that impede or otherwise negatively impact the ability to access services/resources.
Assist clients by providing education, information and support.
Complete documentation and data collection, as needed.
Become familiar with government benefits, community and neighborhood resources, etc.
Participate in and contribute to team meetings. Attend relevant program development activities, trainings, and conferences.
Other duties as assigned.
Education and/or Experience
Requires a High School Diploma and equivalent of at least two years of relevant experience.
Certification/Licensure
Requires a valid drivers license.

Dynamic Opportunity ~ As a full-time Spanish speaking employee, working 40 hours a week, the Peer Advocate for the Program for Assertive Community Treatment PACT program, assess service needs and stage of change and participate in development of individualized recovery focused service plans.
The Guidance Center ~ is a primary mental health support agency, providing compassionate, professional care for individuals and families with mental health needs in the greater Cumberland County, NJ area. For more than 50 years, our non-profit organization of professionals, doctors, nurses, therapists, and support staff, has assisted the community in the diagnosis and treatment of all types of mental illness.
What does this position look like ~ A Peer Advocate uses their experience and education to provide counseling, guidance, role-modeling and advocacy to PACT participants. The Peer Advocate will participate in team meetings, treatment plan building and share in the implementation of those treatment plans. They assist in completing social and daily living skills assessments and provide resources, education and linkages to address any concerns. The Peer Advocate will provide transportation, as needed to support the persons served in meeting their basic needs as well as accomplishing personal goals. The peer advocate will promote the use of community recreational and social activities and utilize individual and group modalities to help support persons served in integrating and socializing within their community of choice. The Peer Advocate will be a part of a multidisciplinary team and will utilize the strengths of the other team members while enhancing the team with their personal experience and strengths. They will participate in on-call response to service requests after standard operating hours and provide crisis de-escalation to those persons in need.
What you need to apply ~
- (1) Master's degree in a behavioral health science from an accredited institution and one year experience in the provision of mental health services;

(2) Bachelor's degree in a behavioral health science from an accredited institution and two years experience in the provision of mental health services;
- (A) Primary consumer may substitute demonstrated volunteer or paid experience working with individuals with serious and persistent mental illness in lieu of a bachelor's degree;
- (B) Primary consumer, who does not possess a bachelor's degree as required in this section for the mental health specialist position, shall be regarded as a full, professional member of the clinical team, function under the same job description as other mental health specialists, and receive salary parity;
- (3) Associate's degree in psychiatric rehabilitation and two years experience in the provision of mental health services.
