POSITION SUMMARY: As the Case Manager, you are a member of a multidisciplinary team, utilizing your skill and experience to assess, plan, develop, coordinate, and provide treatment, rehabilitation, and support services to Adult Community Clinical Services (ACCS) clients under the clinical supervision of the team leader, assistant team leader, and other mental health professionals. Along with the team, on an ongoing basis, reviews the required outcomes to evaluate community tenure and independence.
Provides service coordination (case management) for an assigned group of clients, including coordination and monitoring of activities, assumes responsibility for, implementing, evaluating, and revising the overall Community Service Plan in collaboration with the client and team; symptom management, works with the clinical team, ensuring immediate changes are made in the Community Service Plan as clients' needs change; educate and support clients' families, and advocate for clients rights and preferences.

POSITION SUMMARY: A Unit Coordinator will be responsible for development, organization, paperwork, and service delivery of one or more assigned projects and/or core program services.

Elwyn is currently seeking a Vocational Specialist to assist individuals in navigating their career options. The focus is on providing intensive assessments to identify clients' strengths. This role engages in conversations with clients to understand their goals and preferences, analyzes their physical and emotional conditions, evaluates their milestones, and creates reference reports. Additionally, this position prepares clients for interviews and new jobs by offering effective coaching and counseling.
DUTIES AND RESPONSIBILITIES:
Conduct thorough assessments to evaluate clients' skills, abilities, and interests
Develop individualized employment plans based on clients' goals and aspirations
Provide career counseling to enhance clients' employability
Assist clients in exploring career options and identifying suitable job opportunities
Offer vocational training to enhance clients' job-related skills
Collaborate with educational institutions to coordinate transitional services for students
Conduct workshops on topics such as resume building, job search strategies, and interview skills
Help clients access community resources, including job fairs and job search websites
Connect clients with potential employers
Conduct employer outreach to establish partnerships and secure job opportunities
Assist clients with disability disclosure and request reasonable accommodations from employers
Advocate for clients' rights in the workplace
Monitor clients' progress throughout the employment process
Provide ongoing support and guidance
Help clients develop effective workplace skills, including time management and problem-solving
Work with counselors, psychologists, and social workers to ensure holistic support
Responsible for on-call coverage including being responsible, on a rotating basis, for program coverage when staff members call out and no other coverage is available and promptly responding to the manager's call within 15 minutes and being prepared to report to the program within one hour
QUALIFICATIONS, EDUCATION AND EXPERIENCE:
A bachelor's degree required
A degree in psychology or human services is preferred
At least two years in delivering personalized job development and supported employment services for individuals with physical or mental disabilities preferred
Exposure to counseling work or relevant experience preferred
Must possess a CEP (Certified Employment Professional) certificate or obtain it within 1 year of hire
Ability to develop solutions for job seekers
Deep understanding of counseling and guidance techniques
Ability to work independently and as part of a team

Job Description As the Program for Assertive Community Treatment (PACT) Program Nurse, you are a member of a multidisciplinary team, utilizing your nursing skill and experience to assess, plan, develop, coordinate, and provide treatment, rehabilitation, and support services to (PACT) clients under the clinical supervision of the team leader and the prescriber.

Scope of Work: The primary objective of the SAFEPLAN Advocate is to assist survivors of domestic violence, sexual violence, and stalking who are seeking help from the judicial system primarily within the Worcester area New Hope, Inc. court houses. These include E. Brookfield, Dudley, Uxbridge, Westborough, and Milford District Courts. The role of the advocate is to provide safety planning, information about the law and referral services to survivors. It is expected that the SAFEPLAN Advocate's job performance will incorporate the practice and promotion of New Hope's core values, which include respect, teamwork, empowerment, and professionalism. Assist survivors of domestic violence, sexual assault, and stalking who come to court seeking protective orders by: providing crisis assessment and intervention, assisting survivors in completing protective order application forms, providing information for survivors on legal options, and the process to obtain a protective order.
Screen potential clients for appropriateness and eligibility to receive SAFEPLAN services.
Conduct comprehensive safety planning for survivors and their children that is individualized for each client and consists of ways for the client to remain safe in different situations.
Accompany, support, and advocate for survivors in court during civil proceedings.
Make referrals and connections for appropriate follow-ups to domestic violence services, other social service programs, and the District Attorney's Office as necessary including counseling, group support, legal services or District Attorney Victim Witness Advocates.
Maintain appropriate and professional boundaries with individuals receiving SAFEPLAN services.
Establish professional and collaborative working relationships with court personnel and others including, but not limited to, clerks, judges, attorneys, prosecutors, security officers and victim witness advocates.

Discuss the dynamics of abuse with survivors of domestic violence.

Provides ongoing assessment of client's symptoms and response to treatment. Make appropriate changes in Community Service Plans to ensure immediate and appropriate interventions are provided in response to changes in mental status or behavior, which puts clients at risk.

To support the team, provides service coordination for a small group (2-4) clients, including coordination and monitoring the activities of the individual treatment team (ITT); assumes primary responsibility for developing, writing, implementing, evaluating, and revising overall Community Service Plan in collaboration with the client and ITT; provides therapy utilizing evidence-based practices (i.e., CBT-P, CBT, DBT, CAMS, MI), symptom management, ensuring immediate changes are made in the Community Service Plan as clients' needs change; educate and support clients' families, and advocate for clients rights and preferences.

Job Description As the Program for Assertive Community Treatment (PACT) Lead Nurse, you are a member of a multidisciplinary team, utilizing your nursing skill and experience to assess, plan, develop, coordinate, and provide treatment, rehabilitation, and support services to (PACT) clients under the clinical supervision of the team leader and the prescriber.

Responsibilities
POSITION SUMMARY: As a member of the Interdisciplinary team, the Licensed Practical Nurse (LPN) outreaches individuals receiving crisis supports in coordination with other members of the mobile crisis team. The LPN provides basic medical assessment and care within the scope of their training/practice and function as a link to Crisis Response Centers and Emergency Departments as needed. The LPN plays an integral role in all services provided by the mobile crisis team, including rapid response to community crises, assessments, crisis stabilization planning, trauma-informed approach, social determinants of health screening and referrals, community engagement, and linking individuals to other services to meet their immediate and long-term needs.
The LPN practices within the scope of Pennsylvania's Nurse Practice Act and in accordance with Elwyn's policies and procedures and those of Elwyn's regulatory agencies.
DUTIES AND RESPONSIBILITIES:
Complete a focused nursing evaluation at the site of the encounter, when dispatched with the Mobile Crisis Team, which includes but is not limited to attaining the client's medical history, current medical concerns, medications, and recent hospitalization and share all pertinent information with the Registered Nurse (RN); if there is an escalation or imminent danger during the initial encounter, immediately engage emergent resources, e.g. contact 9-1-1 to determine if hospitalization is warranted; document all details of the encounter to the client's chart and to the electronic record as soon as possible after the incident
Facilitate transfer to the hospital/Emergency Department by communicating details of the encounter with emergency services, updating the RN on the situation, and providing needed documentation to the emergency services team as follow up regarding the encounter, as needed
Collaborate with client's current mental health resources and/or primary care resources, therapists, and other members of the Interdisciplinary Team (IDT) to plan and implement the client's therapeutic program of care; document the treatment plan to the client's chart and electronic record
Present pertinent data obtained through direct observation during the client encounter including but not limited to client interview, obtaining and sharing client history, physical evaluation and other reports as received by external resources to the IDT to communicate client progress and ensure continuity of care, and present information in a concise and organized manner
Report any observed medication and treatment side effects or abnormal laboratory values to the Registered Nurse (RN) immediately for further observation and follow up care, if needed
Administer and document medications given, e.g. Narcon or EpiPen, as indicated; ensure that necessary consents have been completed and documented prior to administration
Document all behavioral and physical changes in the client's status and other pertinent information in the nursing progress notes and in the electronic health records; communicate all client information to the incoming crisis mobile staff through verbal and/or written methods
Order medical and nursing supplies; restock supplies when needed
Maintain medical equipment in proper working order; clean equipment and facilitate equipment repair as necessary; wipe down work surfaces at the end of every shift to ensure infection and disease transmission control
Demonstrate flexibility, adaptability, and team work when nursing assignments must be modified to ensure program compliance and quality of care to clients
Adhere to Elwyn's policies and procedures related to HIPAA and the disclosure of health protected information by maintaining confidentiality of client records, being respectful of the client by speaking to them in private during interviews and updates, whenever possible
Complete all Elwyn mandatory educational requirements by the annual due date and attend nursing meetings and in-services, as required, to ensure program compliance, and learn nursing best practices to provide quality client care
Submit new/current nursing license, cardiopulmonary resuscitation (CPR) card, driver's license, physical exam, and contact sheet on or before the actual due dates
Maintain valid certifications in cardiopulmonary resuscitation (CPR) and first aid to ensure safe and efficient client care and to be in program compliance
Perform cardiopulmonary resuscitation (CPR), first aid, crisis intervention and medication administration using agency-trained protocols
Perform other duties as assigned
EDUCATION/EXPERIENCE/SKILLS REQUIREMENTS:
Licensed as a Licensed Practical Nurse in the Commonwealth of Pennsylvania
One (1) year of psychiatric, medical surgical or chemical dependency nursing experience
Knowledge and understanding of program regulating bodies (e.g. Office of Mental Health & Substance Abuse Services OMHSAS, Department of Behavioral Health) preferred
Ability to work in a fast-paced environment and manage and prioritize multiple, often competing, priorities
Demonstrated strong attention to detail
Demonstrated strong time management and organizational skills
Demonstrated ability to anticipate future needed changes or identify problem areas and take effective actions
