Responsibilities About the Role During a visit that can last up to one hour, Matrix providers review and observe a member's current health, medical history, medication adherence, social environment and other risks. This provides unmatched insight into a member's overall health and well-being that can be difficult to capture during routine office visits. The Matrix Comprehensive Health Assessment (CHA) helps to improve quality of care and allows us to potentially close multiple care gaps with a single visit. Our providers: * Conduct adult / geriatric assessments to include medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up and clear documentation according to Matrix guidelines and protocols * Administer and order point of care and lab-based testing as needed * Work with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate * Collaborate with Primary Care Physician (PCP) on patient education and follow up as necessary * Provide services at the member's place of residence which can be in their homes or Skilled Nursing Facilities Benefits of Working at Matrix * Flexibility - Visits are scheduled based on YOUR availability. We conduct assessments Sunday - Saturday from 8 am - 7 pm. You can work during the day, evening, and/or weekend based on your schedule. * Competitive Pay - Earn income based on the number of assessments you complete. Increase your earning potential by offering greater availability and traveling to see members outside your home location. Those completing a monthly average of 20 - 100 visits earn $1,800 - $11,500 per month. This is based on the per assessment completion rate range of $90 - $115 depending on your state of residence. * Benefits - Including mileage reimbursement, 401(k), employee referral bonuses, and limited voluntary benefits. * Education and Support - We invest in YOU! Job Description FLEX PROVIDER POSITION SUMMARY: The Flex Provider (fP) provides clinical assessment and care to assigned patients including assessments of health plan members in their homes and in facilities. The responsibilities include: adult/geriatric assessment, medical history, physical exam, medication reconciliation, diagnosis, and health education. Additionally, patient education, communication to the PCP, follow-up facilitated with the PCP, and case management referrals may also be part of any assessment. Clear documentation according to Matrix Medical Network (MMN) guidelines and protocols is required. Flex Providers will use previously acquired skills, education, medical training and experience as a medical professional in performing assessments. In the performance of the assessments, patient safety and well-being is of the highest priority. Flex Providers are expected to maintain MMN standards for quality and customer service. Accordingly, the assessments performed by Flex Provider will be performed in accordance with currently approved and generally accepted professional medical standards and with MMN policies and procedures. From time to time, MMN will provide to Flex Providers copies of those policies and procedures. Other clinical care services may be required within the NP scope of practice. FLEX PROVIDER RESPONSIBILITIES: Performs in-depth health risk assessments on all assigned members in patient homes or facilities Works collaboratively with physicians, case managers, social workers, family members, key care givers and any appropriate ancillary medical personnel as appropriate Responsible for identifying overall quality improvement activities through assessing and planning health care which is individualized to the member and consistent with Matrix Medical Network policies and protocols Reviews and interprets the medical and social history of members Performs appropriate screening procedures for members Interprets laboratory data, recommends appropriate follow-up, reconciles medications Refers members to PCP, case management and disease management programs as necessary.
