Community Outreach Liaison Key Responsibilities:
Generates referrals and admissions using the four-cornerstone approach to business development, within the assigned territory.
Develops and implements territory sales plan which will meet the goal of expanding the customer base in the market areas.
Monitors and recognizes changing referral patterns in assigned area and modifies strategies to meet goals.
Documents all sales related activity within the CRM daily.
Responsible for meeting monthly and annual referral and admission goals in the assigned territory.
Identifies, develops and maintains key relationships in the territory, by participating in various functions sponsored in the medical community.
Promotes Ohio's Hospice through educational opportunities and community events in assigned territory.
Assists in other activities and departments when requested.
Participates in Continuous Quality Improvement activities to support the quality of Ohio's Hospice services.
Precepts new staff to support professional relationships with newly hired team members as required.
Community Outreach Liaison Qualifications:
Must have a bachelor's degree or equivalent experience in healthcare or business development
Preferred two years in a sales/marketing role within the healthcare industry with a proven record of accomplishments in direct sales and marketing.
Computer skills sufficient to properly document services and communicate effectively, CRM experience preferred.
Ability to drive during daytime, nighttime, or inclement weather.
Valid Driver's License with Safe Driving Record

Housing Outreach Specialist

Location : Candidate must reside in the state of Ohio. The Housing Outreach Specialist is responsible for developing and implementing the health plans housing strategy and programs. This includes the approach to assisting individuals in programs in securing accessible, affordable housing through Federal and local programs. The Specialist will possess knowledge of housing programs and services, focusing on homelessness and at-risk populations, and specific accessibility needs as related to LTSS populations.
How you will make an impact :
* Liaise between housing agencies and the health plan and partner with these agencies to develop and access affordable housing services for members.
* Partner with the Health Plan trainers to develop and implement training specific to affordable housing services in the state of Ohio.
* Work under the Housing First model, honoring Member choice.
* Responsible for working with housing agencies and other housing programs to help develop and access affordable housing services for Members receiving LTSS.
* Responsible for education and supporting Care Coordinators and Support Coordinators in identifying housing options for members.
Minimum Requirements :
* Requires a minimum of 3 years experience in developing housing and/or housing policy at a local, state or federal level for special populations such as the elderly, people with disability and/or people with mental health challenges to secure accessible, affordable housing through Federal and local programs; or any combination of education and experience which would provide an equivalent background.
Preferred Skills, Capabilities & Experiences :
* Bachelor's degree in Social Services or related field highly preferred.
* Case management experience with older adults or individuals with disabilities.

Job Description:
The Transplant Outreach Coordinator will be responsible for the departments Outreach efforts. Position is responsible for developing the departments' image and maintaining relationships with the Primary Care Physicians, Dialysis Units and Staff, Nephrologists and Social Workers. Develop individualized service plans and goals in collaboration with clients to address behavioral health, healthcare, employment, housing and other essential needs.
Connect clients with appropriate resources and services, including emergency shelter, transitional housing, healthcare providers, mental health and substance use treatment and social services.
Advocate for clients to ensure access to benefits and community-based supports.
Provide crisis intervention and supports to clients experiencing mental health crises, substance use issues, or other emergencies.
Collaborate with crisis response teams, law enforcement, emergency medical services as needed to ensure client safety and well-being.
Build partnerships and collaborate with community organizations, homeless shelters, healthcare providers, law enforcement, and local government agencies to coordinate services and support for individuals experiencing homelessness.
Participate in community meetings, task forces and committees focused on homelessness prevention and solutions.
Maintain accurate and up to date documentation of client interactions, assessments, service plans, referral and outcomes in accordance with agency policies and regulatory requirements.

SUMMARY
The mission of the Greater Cleveland Food Bank is to ensure that everyone in our communities has the nutritious food they need every day. The Community Outreach Coordinator supports this mission by assisting eligible neighbors in completing applications for SNAP (food stamps) and other public benefits through the Ohio Benefits self-service portal (OB). This position requires the person to travel to various community locations to educate the community about SNAP and other public benefits and screen neighbors one-on-one for these benefits. This position also requires the Community Outreach Coordinator to build partnerships with community organizations and work with staff and volunteers to connect eligible community members to benefits.
Requirements
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. This list is not all-inclusive and other duties may be assigned.
* Work as part of a team to find and reach community members who are eligible for but not receiving SNAP benefits. This individual is responsible for submitting on average 15 SNAP applications per month.
* Identify and reach out to agencies and organizations to partner on SNAP and public benefit enrollment events.
* Publicize and market the Help Center via flyers and other means.
* Engage the community on site at community events, Mobile Pantry distributions, pop-up clinics, libraries, hunger centers and other available venues.
* Maintain required job knowledge and skills and core professional competencies. Attend and participate in required educational programs and staff meetings.
* Read, respond to and be responsible for information relayed via e-mail, text message and instant message communications from staff and supervisors on all assigned work days.

High school diploma or general education degree (GED) required. Experience working at a nonprofit social service agency, either as a volunteer or employee, is required. Must possess outstanding communication skills with strong attention to detail. Position Summary
The Community Coordinator plays a pivotal role in managing a designated area within the shelter. This position is responsible for providing comprehensive case management and connections to housing resources and supportive services to assist clients transitioning out of homelessness. This position aims to empower homeless individuals with the necessary tools to achieve housing stability and increase income, aligning with the overarching goal ensuring homelessness is a brief, infrequent, and one-time experience for those served.
Some of the duties include:
Collaborate with clients to devise Exit Plans that accelerate their transition out of homelessness, connecting them with suitable destination options based on eligibility
Facilitate referrals and connections to community partners, as appropriate
Assist shelter clients in enhancing their income prospects by guiding them towards job training programs, employment opportunities, or facilitating SSI/SSDI applications
Align clients' trajectories with their Exit Plans by coordinating their engagement with other Communities within the Shelter
Ensure client's integration into Cuyahoga County's Coordinated Entry system
Arrange client transportation to critical destinations outlined in their action plans
Adapt to fluctuating needs by floating between Communities, as needed
Maintain participant data and statistical information in the client database system
Engage in regular caseload reviews with supervisor(s)
Assist in managing Shelter emergencies in adherence to program policies, prioritizing the safety of both clients and staff
Conduct bi-weekly reviews with each community participant to monitor progress
Apply best practices in case management (e.g., Motivational Interviewing, Critical Time Intervention) and shelter operations (e.g., Housing First, Low Demand, Harm Reduction).
Qualifications

Proven experience identifying, accessing, and coordinating with community partners and social service resources; experience working with vulnerable populations and people with lived experience; and the ability to uphold and support the Housing First Philosophy.

SUMMARY
The mission of the Greater Cleveland Food Bank is to ensure that everyone in our communities has the nutritious food they need every day. The Community Outreach Coordinator supports this mission by assisting eligible neighbors in completing applications for SNAP (food stamps) and other public benefits through the Ohio Benefits self-service portal (OB). This position requires the person to travel to various community locations to educate the community about SNAP and other public benefits and screen neighbors one-on-one for these benefits. This position also requires the Community Outreach Coordinator to build partnerships with community organizations and work with staff and volunteers to connect eligible community members to benefits.
Requirements
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. This list is not all-inclusive and other duties may be assigned.
Work as part of a team to find and reach community members who are eligible for but not receiving SNAP benefits. This individual is responsible for submitting on average 15 SNAP applications per month.
Identify and reach out to agencies and organizations to partner on SNAP and public benefit enrollment events.
Publicize and market the Help Center via flyers and other means.
Engage the community on site at community events, Mobile Pantry distributions, pop-up clinics, libraries, hunger centers and other available venues.
Maintain required job knowledge and skills and core professional competencies. Attend and participate in required educational programs and staff meetings.
Read, respond to and be responsible for information relayed via e-mail, text message and instant message communications from staff and supervisors on all assigned work days.
Maintain professionalism in all interactions with social service agencies, community partners, neighbors, volunteers, and coworkers in order to uphold the exceptional reputation of the Greater Cleveland Food Bank and the Benefits Outreach Department.
Work directly with neighbors to assist them in applying and obtaining primarily SNAP, and other public benefits programs using the Ohio Benefit State-wide software.
Educate neighbors about State and Federal regulations about documentation, ABAWD requirements and what they can expect in an interview with their county's Job and Family Services.
Inform neighbors of follow up processes within their county and empower them to advocate for themselves in order to receive their benefits.
Refer neighbors to other social service agencies as needed, in order to meet their needs as expressed to the Community Outreach Coordinator. Form and maintain relationships with other social service agencies.
Track applications completed daily on the department's shared programs; Ohio Benefits and Cap60.
Support all Outreach team functions at the Community Resource Center and provide direct neighbor engagement a minimum of 20 hours per week at the Community Resource Center.
Actively support and champion initiatives aimed at driving continuous improvement throughout the organization. May participate in one of the following: LEAN council, a kaizen, 5S campaign or other process improvement efforts to support continuous improvement and improved processes within the food bank.

Qualifications
High school diploma or general education degree (GED) required. A Bachelor's degree in related field (such as education, social work, psychology, etc.) or one year of experience in related field (such as community organizing, customer service, case management in a nonprofit setting, or social work) is required. Experience in public benefit management software preferred but not required. Experience in training, teaching, and/or public speaking is preferred but not required. Experience working at a nonprofit social service agency, either as a volunteer or employee, is required. Must possess outstanding communication skills with strong attention to detail. Must be able to effectively work in a team atmosphere with highly diverse people in order to achieve the goals and objectives of the Outreach Department and carry out the mission and values of the Greater Cleveland Food Bank. Must be able to work independently towards personal and team goals, as well as prioritize own work. Must be available to work some evenings and weekends, as needed, as part of a 40 hour work week. Must be able to maintain own schedule and balance flexible hours. Must be oriented towards efficient problem solving, multi-tasking, conflict management, and teamwork. Must possess basic understanding, familiarity, and sensitivity to hunger and poverty issues. Proficiency in Windows based computer software required. A valid Ohio Driver's License and proof of automobile insurance are required (mileage is reimbursed). Unencumbered driver's license is required. Willingness to drive the Greater Cleveland Food Bank's Food Truck and/or support events where the Food Truck is present is required. Physical Requirements and Environment
The physical requirements for this position include: Ability to frequently remain in a stationary position. Occasional ability to move about inside the office to access file cabinets, office machinery, etc. Ability to operate a computer or other office productivity machinery frequently. Occasional ability to ascend/descend stairs to access work spaces. Ability to position or move body to access filing cabinets or other work equipment frequently. Ability to constantly exchange accurate information and communicate in person or via telephone with neighbors, partners, donors, coworkers and other members of the community. Must be able to frequently view a computer screen. Ability to move office equipment/supplies constantly. Ability to lift 25-50 lbs on a regular basis. Travel is constantly required for this position. While performing the duties of this job, the employee is frequently exposed to outside weather conditions. The noise level in the work environment is usually moderate. Employee will be required to travel to various locations in the community.
