POSITION SUMMARY:
Community Health Action of Staten Island, in partnership with Sun River Health, has developed a collaborative program to enhance care engagement, improve HIV health outcomes, and promote the well-being of people living with HIV/AIDS (PLWH/A) in Queens and Staten Island. The Ryan White Support Services program addresses unmet social service needs that have significantly hindered their access to and engagement in HIV and other essential medical care. This program will expand services to Staten Island and establish community-based, non-Medicaid eligible services in Queens to assess, identify, and address unmet social determinants of health (SDOH) needs.
The Peer Advocate position was created as research and CHASI's values dictate that involving people who have their own lived experience with substance use, mental health conditions, and/ or HIV is invaluable in reaching and helping the population. The Peer Advocate mentors and guides individuals using their experiences to support people in a way that shows people that living a healthy life with HIV is not only possible but sustainable. The Peer Advocate maintains appropriate positive, professional interpersonal relationships with staff, participants, volunteers, and the community. They will be well versed in all CHASI and Sun River Health services to refer and link clients appropriate to services.
DUTIES & RESPONSIBILITIES:
Provide overview of program, department, and agency services through Orientation activities;
Accompany participants to provider sites to give support and navigation assistance;
Co-facilitate group activities that teach clients how to identify high risk thoughts or actions and slow down impulsive behavior to change their thought patterns for prosocial behavior;
Assist with community outreach and program promotion to inform the public of available agency resources, as needed;
Provide food and nutrition counseling/education;
Successfully complete required food and nutrition counseling and education training held by the College of Staten Island;
Conduct telephone and office-based follow up with individuals to identify and address barriers to care and treatment;
Provide reminder phone calls to reinforce retention in care and services;
Reach out to participants to follow up on referrals made and to engage them into programming;
Outreach individuals who miss appointments;
Attend and participate in staff development trainings as required;
Actively participate in regularly scheduled supervision and attend team meetings;
Complete all program documentation in a timely manner;
Perform all functions in alignment with CHASI's Mission, Vision, and Core Values;
Other duties as assigned.
Requirements:
QUALIFICATIONS:
Hold a NYS AIDS Institute Certified Peer credential, preferred. Other Peer certification from a reputable program can also be considered OR be able to obtain certification within first 6 months of hire.
Lived experience with HIV is highly preferred; candidates with lived experience with substance use and/or mental health conditions are welcomed also welcomed.
High school diploma or high school equivalency is highly preferred.
Bilingual Spanish is highly desired.
Possess an understanding of community-level work and the importance of collaborating and coordinating with other organizations.
Effective oral and written communication skills.

POSITION SUMMARY:
The Human Resources department provides comprehensive support to CHASI staff team to ensure smooth people operations of the organization and to enhance staff satisfaction and effectiveness. The department plays a crucial role in driving CHASI's culture in alignment with our mission and core values. CHASI has approximately 150 employees when fully staffed.
The Human Resources Manager is a unique generalist role that is responsible for managing a variety of day-to-day and ongoing tasks to support the life cycle of employees at CHASI. This includes, but is not limited to, leading and facilitating onboarding, offboarding, benefits administration, recruitment, staff engagement, staff relations, and special projects. The Human Resources Manager will be supervised by, and work as a collaborative partner with, the Vice President of Human Resources and will have the opportunity to help develop the department and agency-wide initiatives. This is a management-level position based on the scope of and competencies required for the role; it does not directly supervise any employees at this time.
This role is primarily in person, with the option of one remote work day per week following the onboarding period.
DUTIES & RESPONSIBILITIES:
Recruitment:
* Seek out and maintain relationships with appropriate venues and stakeholders to promote employment opportunities

Onboarding:

Requirements
QUALIFICATIONS:
* At least 4 years of experience in a Human Resources role and ability to demonstrate an understanding of the duties as described above
* A degree or certification in Human Resources preferred; additional experience may be considered in lieu of a degree or certification

Position Summary: Responsible for providing direct individual and group trauma-informed mental health counseling and other appropriate mental health services to men, women and young adults enrolled in CCA's Trauma Recovery Center (TRC). In collaboration with participants, develop a comprehensive and individualized treatment and service plan that integrates knowledge from a wide range of human service resources.
Performs extensive community outreach and home visits to participants.

POSITION SUMMARY:
CHASI's Prevention & Intervention Department focuses on seeking out and coordinating education and referrals for persons within priority populations to decrease acquisition of HIV and STIs through comprehensive effective strategies.
The Prevention Outreach and Education Specialist educates clients to increase their awareness of post & pre-exposure prophylaxis (PEP and PrEP) and discusses all relevant combination prevention measures according to client presentation. Outreach and Education Specialist covers a wide range of topics including reproductive health, mental health, substance use, and supportive services. Additionally, they provide information and resources on condoms, safer sex products, PrEP, Emergency PEP, iART, HIV care and treatment, as well as the undetectable = untransmitable (U=U) concept, and HIV/STI screening. Assists with client entry into healthcare and social services.
This position is full-time (40 hours/week) and requires a flexible schedule such that some of the job tasks will be conducted during evening and late evening hours, and on weekends.
DUTIES & RESPONSIBILITIES:
* Conduct street and facility-based (both within CHASI and at external partner locations) outreach to the target populations with the program's peers outreach workers and engage and navigate high risk persons to HIV counseling, education, and testing.
* Conduct risk behavior screenings to identify persons at high risk of HIV infection.
* Complete the appropriate documents and facilitate their enrollment into the program.
* Educate and counsel high risk persons on combination prevention strategies (e.g. correct and consistent condom use, the importance of frequent HIV testing and screens for other sexually transmitted infections, PreP, and medical interventions intended to assist efforts at staying HIV-negative).
* Ensure clients attend appointments for medical intervention and social services and secure documentation about the outcome of such appointments via phone or in person.
* Collect and submit to CHASI's Data Unit accurate data on persons contacted and engaged via both virtual and in-person outreach.
* Conduct presentations, and workshops on sexual health topics to increase program engagement.
* Work collaboratively with the whole staff to integrate cross program activities and functions.

Position Summary:
Reporting to the Registered Nurse will provide for medication dispensing and nursing assistance in the diagnosis, and continuous primary medical care to CCA participants. Will act and function as the HIV Coordinator of services for the clinic.

Assists the Registered Nurse in the performance of the Registered Nurse's duties.

Verifies the identification and observes the behavior of patients to whom medication or other medical services are provided.
Will act as the HIV Coordinator for the clinic. This includes the responsibility for tracking the HIV patients for medical care.
Must adhere to all procedures referable to accounting for medication dispensed to patients and reporting any deviations immediately to the Registered Nurse.
Maintains the medical charts, the medical unit appointment book and daily visit logbook.
Performs the appropriate independent nursing follow-up care of patients.
Assists in the identification and recall of patients for follow-up medical and nursing care. This includes the monitoring of urine toxicology.
Assists in the performance of admission and physical examinations.
Provides the physician or physician assistant with urine toxicology and other laboratory data as they become available.
Attends on a punctual basis clinic meeting.

Duties and Responsibilities:
Assist youth referred to CCA's Transition Advocacy Program in navigating academic, legal, social services, and medical/mental health systems.
Inform and ensure youth and parents/guardians facing a superintendent's disciplinary hearing are aware of their rights and responsibilities as outlined in Education Law 3214 and SCSD's Code of Conduct.
Provide support to youth who have an Individual Education Plan (IEP) or a 504 plan.
Participate in a Manifestation Determination Review as part of the disciplinary proceeding.
Address underlying issues regarding a youth's misbehavior in accordance with the SCSD Code of Conduct.
Assist parents, youth, hearing officers, and district staff (administrators, principals and teachers) in considering restorative interventions and supports that encourage educational success.
Provide services for the following: informal hearings, informal conferences, short-term suspensions, restorative meetings, local MDR/CSE meetings, transition meetings and family or criminal court proceedings related to school suspensions.
Support the transition for youth who are returning to mainstream school/community following an absence due to a long-term suspension, a return from juvenile detention, or a return from jail, prison or juvenile placement.
Conduct youth assessment through one-on-one meetings with the youth, family members and school staff and/or community supports to review pertinent records to develop an individualized Plan.

Follow-up to engage youth for up to 6 months following their transition back to mainstream school and/or community, track progress and provide referrals to the school district and/or other community supports to ensure a successful transition.
Participate in ongoing professional development and training opportunities.

Qualifications:

The Alternative to Incarceration (ATI) program provides an opportunity for those who are involved with the criminal justice system, charged with an offense and exhibit substance use and/ or mental health treatment needs, to access care and treatment with the long-term goal of managing their lives in the community and preventing future arrests and returns to incarceration. The Peer Mentor position was created using the research and belief that involving people who have their own lived experience with substance use, mental health conditions, and/ or criminal justice involvement is invaluable in reaching and helping the population.
The Peer Mentor role mentor will guide individuals using their experiences to support people in a way which shows people that recovery is possible and does happen - a peer mentor becomes a role model. The Peer Mentor provides services that are strength-based and trauma-informed. The Peer Mentor provides outreach, engagement, and accompaniment to individuals enrolled in the program programs on-site and in the community.
DUTIES & RESPONSIBILITIES:
* Provide overview of program, department, and agency services through Orientation activities;
* Accompany participants to provider sites to give support and navigation assistance;
* Co-facilitate group activities that teach clients how to identify risky thoughts and slows down impulsive behavior to change their thought patterns for prosocial behavior;
* Assist with community outreach and program promotion to inform the public of available agency resources, as needed;

CHASI's Next Step Resource and Recovery Center provides opportunities for people in all stages and forms of recovery to deliver and attend individual and group support activities, initiate and conduct outreach, and events celebrating recovery and reducing stigma associated with substance use. It hosts several services, including Recovery Center peer-driven individual and group recovery services, counseling and case management, overdose prevention services, harm reduction, and recovery-oriented community outreach, education, and the CHASI Peer Training Institute. The Peer Training Institute is designed to promote professional growth and development for people in recovery from substance use and support the institutions that hire them. Currently, the Peer Training Institute includes training opportunities in the areas of Peer Recovery Coaching (CRPA - Certified Recovery Peer Advocate), Administration, and Janitorial. This position also includes supervision of at least one Training Specialist.
The Training Institute Coordinator will serve as the lead trainer and coordinator of the CHASI Peer Training Institute and the lead organizer and point of contact for the newly established CHASI Alumni Association.

DUTIES & RESPONSIBILITIES:
Conduct street and facility-based (both within CHASI and at external partner locations) outreach to the target populations with the program's peers outreach workers and engage and navigate high risk persons to HIV counseling, education, and testing.
Conduct risk behavior screenings to identify persons at high risk of HIV infection.
Complete the appropriate documents and facilitate their enrollment into the program.
Educate and counsel high risk persons on combination prevention strategies (e.g. correct and consistent condom use, the importance of frequent HIV testing and screens for other sexually transmitted infections, PreP, and medical interventions intended to assist efforts at staying HIV-negative).

Position Summary:

The Information and Referral (I&R) Specialist assists with CHASI's Community Food Pantry operations and conducts outreach to substance users to engage persons in services, provides a wide range of linkage & navigation services, provides individual education to reduce health-related harm from injection and other drug use, conducts sterile syringe exchange, trains on opioid overdose reversal, and delivers HIV and hepatitis C (HCV) counseling and testing. The I&R Specialist primarily works at the community access center to reach long-term and newer substance users, to partner with them as they set their own goals for improving their health and well-being, and to help them navigate service systems to achieve those goals.
DUTIES & RESPONSIBILITIES:
Conduct group presentations to engage and inform substance users about harm reduction interventions.
Conduct harm reduction-oriented individual and group education encounters with substance users to reduce the transmission of blood-borne diseases, other infections, and drug overdose.
Enroll individuals in State-authorized syringe exchange program and provide exchanges that promote the consistent use of sterile syringes and safe collection of used syringes and provide safer injection counseling.
Distribute Naloxone kits to reverse opioid overdose and provide group and individual training on their correct use of Naloxone.
Conduct HIV and HCV counseling, testing, partner notification and linkage services, in accordance with standards set by NYS Department of Health/AIDS Institute and CHASI.
Deliver community-based education and risk reduction education and materials to targeted populations and the community.
Provide information, referral, navigation, and coordination assistance that links individuals to health and human services responsive to their individually expressed needs and interests.
Collaborate with harm reduction and community access staff on the identification of service provision, trends in local drug use, and service providers that are substance user friendly.
Support the food pantry staff and volunteers during operations when needed.
Complete accurate documentation service provision using hard copy tools and electronic data systems.
Track inventory of harm reduction program supplies.
Complete internal and external training to maintain knowledge and skills related to harm reduction, culturally competent service delivery, and other job-relevant topics.
Actively participate in staff meetings and other agency activities.
Encourage a sense of commitment, camaraderie, and accountability among team members by modeling these qualities.
Screen incoming phone calls and provide administrative support on a periodic basis.
Provide services on the Mobile Health Unit as assigned.

Requirements:
QUALIFICATIONS:
High school diploma or GED required.
Experience working with substance using persons and diverse populations strongly preferred.
Conversant with harm reduction practices, philosophy, and interventions.
Bilingual English/Spanish strongly preferred.
Ability to work effectively independently and as part of a team with demonstrated ability to contribute positively to a team-based work environment.
Demonstrated proficiency in the use of office computer applications, including Microsoft Word, Excel, and Outlook.
Strong oral and written communications skills; ability to manage communications tactfully and professionally with a high attention to detail and accuracy.
Driver's license and clean driving record preferred.
Must have flexible schedule and be able to work weekends.

POSITION SUMMARY:
The Senior Peer Mentor (SPM) will support the Transportation Coordinator in managing a grant-funded rideshare transportation program for individuals with a history of substance use disorder. The SPM will be responsible for documenting all transportation services utilized on a weekly and monthly basis and escorting participants to necessary locations across Staten Island when warranted. This role is ideal for a compassionate and detail-oriented individual committed to supporting people in recovery by ensuring they have reliable access to essential services.
DUTIES & RESPONSIBILITIES:
Transportation Support & Documentation:
Maintain accurate records of all rides provided through the program, including dates, times, destinations, and participant details.
Prepare and submit weekly and monthly transportation utilization reports to the Transportation Coordinator.
Assist with tracking program performance and identifying trends in transportation needs.
Communicate with participants about their scheduled rides, ensuring they understand pickup times and locations.
Peer Support & Escorting Services:
Provide in-person escort services for participants who require additional support traveling to appointments, treatment programs, or other essential locations across Staten Island.
Offer peer-based support and encouragement during transit, helping participants navigate barriers to accessing care.
Act as a liaison between participants, transportation providers, and the program team to ensure smooth service delivery.
Collaboration & Advocacy:
Work closely with the Transportation Coordinator and case management staff to address transportation-related concerns.
Advocate for participant needs and provide feedback on ways to improve the program's accessibility and effectiveness.
Uphold confidentiality and ethical standards while supporting individuals in their recovery journey.

Requirements:
QUALIFICATIONS:
New York State Certified Recovery Peer Advocate-Provisional (CRPA-P), at minimum.
Prior experience in peer support, case management, or human services preferred.
Strong attention to detail with the ability to maintain accurate records and reports.
Excellent interpersonal skills with the ability to engage and support individuals in recovery.
Must be dependable and able to meet participants at designated locations on time.
Familiarity with Microsoft Office (Excel, Word) or Google Docs/Sheets for reporting.
PREFERRED:
Experience working with individuals in recovery and/or navigating transportation challenges.
Knowledge of Staten Island's public transit system and local recovery resources.
Valid driver's license and reliable transportation (not required but a plus).
