Purpose of Job:
Improve the overall health of the communities we serve by expanding access quality health and wellness services by providing administrative support services as follows:
Essential Duties and Responsibilities:
* Provides excellent customer service as point of contact for department.
* Organizes large volumes of information, enters needed information into appropriate system in efficient and timely manner.
* Responsible for administrative tasks including but not limited to meeting material preparation and distribution, scheduling meetings, and taking meeting minutes.
* Files correspondence and other records.
* Makes copies of correspondence or other printed matter.
* Monitors surrounding area for cleanliness and security.
* Maintains confidentiality and professional attitude.
* Performs duties of a Courier as needed.
* Performs additional duties depending on department need, i.e. Answering phones, mailing weekly statements, managing calendars, making travel arrangements, posting payments from calls, etc.
* Performs other duties assigned, including supporting the CHAS Health Mission and Core Values.
Qualifications:
Education/Experience: Two years receptionist and/or administrative assistant experience preferred.
Skills: Computer skills required. Effective oral and written communications skills, including excellent telephone etiquette required. Exceptional customer service, time management, and multi-tasking skills required. Valid driver license, insurance and vehicle required. Commitment to supporting a safe and respectful environment is required.
Physical Demands:
Required to stand, sit, and be mobile one-third to two-thirds of the time. They are required to use hands to finger, handle or feel over two-thirds of the time; while reaching with hands and arms occurs one-third of the day. Climbing or balancing, stooping, kneeling or crouching occurs less than one-third of the time. Communicating by talking / hearing occurs over two-thirds of the day. Purpose of Job:
Improve the overall health of the communities we serve by managing patient care at assigned medical clinic as follows:
Essential Duties and Responsibilities:
·Provides direct supervision of medical support staff.
·Interviews and selects new candidates with Clinic Administrator.
·Responsible for training, orienting and mentoring all medical support staff.
·Coordinates and/or participates in staff in-service training as directed.
·Supervises development of continuing education plans.
·Ensures that medical support staff is meeting or exceeding quality goals.
·Orders medical supplies, drugs, and immunizations.
·Verifies that all medical equipment is consistently in good working order.
·Performs essential duties of a Medical Assistant - Certified or Registered Nurse, as licensed to support team.
Performs other duties assigned, including supporting the CHAS Health Mission and Core Values.
Qualifications:
Education/Experience: Two years' experience as a medical assistant or registered nurse required. Registered nurse required in urgent care setting. Management experience in healthcare setting preferred. Previous experience in a community health care setting preferred. Experience with EHR preferred.
Medical Assistant - Certified: Completion of medical assistant training through an apprenticeship program registered and approved by the WA Department of Labor & Industries or college program accredited by the Accrediting Bureau of Health Education School (ABHES) or the Commission of Accreditation of Allied Health Education Programs (CAAHEP) required. Certification through the American Association of Medical Assistants (AAMA), National Healthcareer Association (NHA) or National Center for Competency Testing (NCCT) required within 120 days of hire. Washington locations also require a Medical Assistant - Certified credential issued by Washington State Department of Health.
Registered Nurse: Valid RN license in the state of Washington or Idaho required.

Overview:
On-the-Job Training: Completion of 2000 hours with the support and guidance of an experienced MA coach (paid).
Supplemental Coursework: Completion of 410 hours of online coursework (unpaid).
In-Person Lab Days: Attend 3 lab days with an expert instructor to practice and demonstrate mastery of hands-on technical skills.
Reporting: Log and report on-the-job hours and skill practices as required.
Skill Mastery: Successfully perform a required number of skills and be signed off to complete the program as directed by the program requirements.
What You'll Do:
Maintain and update patient medical records, including medical history, current status, vital signs, treatments, and medications. Review and update immunization records.
Prepare for patient visits by reviewing files, ensuring necessary reports are present, and providing updates to the Provider team.
Work with Providers to ensure efficient schedules and continuity of care. Discuss patients scheduled for the day, including pre-visit preparation.
Prepare and maintain patient exam rooms, ensuring appropriate supplies and sterilized equipment are available.
Prepare patients for examinations, explain medical treatment procedures, and provide limited education and coaching within scope. Instruct patients about medication and possible allergic reactions.
Respond to patients' questions and concerns in person and via email. Direct inquiries outside of scope to the appropriate person.
Assist physicians during examinations, procedures, treatments, and minor office surgeries. Anticipate Providers' needs and manage their requests.
Collect and prepare laboratory specimens, complete screenings, and review patient data per protocol. Follow up on test results as directed by healthcare providers and document them in the electronic medical record (EMR).
Prepare and administer medication as directed by a physician.

Located in Toppenish and Spokane, WA, this program provides advanced training that surpasses traditional academic learning, undergraduate pharmacy clerkships, and general community pharmacy practice.
Residents will develop and deliver patient care services, focusing on managing chronic diseases such as diabetes, hypertension, cardiovascular risk, hepatitis C, and anticoagulation. They will also contribute to setting and achieving departmental goals to enhance pharmaceutical services and patient care standards.
To meet these objectives, residents will participate in both required and optional experiences within the YVFWC organization. These experiences include primary care services, clinic-based pharmacy management, central fill and specialty pharmacy operations, care for patients with HIV, refugee services, and administrative practices. Additionally, residents will have opportunities to engage in research and gain teaching experience through our partnership with the WSU College of Pharmacy and Pharmaceutical Sciences.
Overall, this program emphasizes delivering pharmaceutical care to improve patient outcomes, regardless of the patient's background or circumstances.

What You'll Do:
Provides care during patient visits, discussing chief complaint and obtaining vital signs and complete health history for the patient. Develops a baseline physical assessment.
Performs phone triage with patients to determine chief complaint and assessment of next steps and follow up care needed.
Documents health history, assessments, actions and medications in the Electronic Medical Record (EMR) system.
Assists providers in diagnostic procedures and treatments.
Performs IV Starts when ordered. Provides information to patient regarding the procedure, including site selection and possible complications.

Collaborates with provider in implementing the patient plan of care including providing education and identifying additional services needed for the patient. Initiates follow-up care plan as needed.
Collaborates with ancillary staff to process patients based on providers' schedule, maintaining smooth patient flow.
Performs clinical responsibilities in alignment with The Joint Commission (TJC), Health Resources and Services Administration (HRSA), and YVFWC requirements.
Qualifications:
Associate's Degree in Nursing. Bachelor of Science Degree in Nursing is preferred.
One year's experience providing clinical or outpatient care as an Registered Nurse (RN) is preferred.
RN license required for the state of practice.
Dual licensure in Washington and Oregon is required within 120 days of hire
If RN license is in a participating Nurse Licensure Compact (NLC) state, the following requirements apply: Must submit proof of license in state of residence. Must submit proof of completion for the required suicide prevention training during 1st week of onboarding. If relocating to Washington State, must obtain WA RN license within 6 months of hire. Must update primary state of residence within 60 days (applies to anyone relocating to Washington State)
Military Nurses are not required to obtain Nurse Licensure Compact (NLC)
BCLS certification within 90 days of hire; ACLS & PALS are preferred

Purpose of Job: Improve the overall health of the communities we serve by expanding access to quality health and wellness services by opening channels to resources and access to health care for those least served as follows:
Essential Duties and Responsibilities
* Counsels and aids patients and their families to understand healthcare recommendations and options by providing education and resources. Supports patients in navigating the healthcare system.
* Conducts comprehensive and on-going identification of social drivers of health (SDOH) needs and barriers to effective patient care. Advocates for and refers patients to appropriate community resources.

* Supports equitable distribution of supplies, clothing, hygiene items, seasonal needs, and education to community members.
Population and Location-Based
* Provides and advocates for culturally competent care and support for patients from specific patient populations.
* Supports creation of educational materials and resources that are tailored to specific patient populations.

Qualifications
Education/Experience: One year of health and/or social experience required. What You'll Do:

Purpose of Job:
Improve the overall health of the communities we serve by providing individualized nutritional support to patients for their particular situation as follows:
Essential Duties and Responsibilities:
Meets directly with patient to identify dietary needs through nutritional assessment based on nutrition-focused physical findings and patient history.
Provides nutritional counseling either individually or in a group setting; identifying goals and supporting education with evidence-based care.
Develops detailed care plans and documents as part of the electronic health record in accordance with established program standards.
Assesses, plans and implements the delivery of medical nutrition therapies in collaboration with the patients inter-disciplinary team.
Monitors and evaluates patient's progress through routine nutrition interventions to support positive health outcomes.
Participates and supports patients in various program committees; including developing educational programs for various ethnic, cultural and social groups to teach nutrition education.
Remains up to date on evidence-based research and recommends ways to expand and improve nutrition programs.
Participates in quality improvement of the program in conjunction with other services CHAS Health offers.

Qualifications:
Education/Experience: Bachelor's degree in Nutrition or related field required. Registered Dietitian credentialed with the Commission on Dietetic Registration and eligible for state licensure. Experience in chronic disease including HIV and Diabetes and/or maternity nutrition counseling preferred.
Skills: Excellent oral and written communications skills required. Computer skills required. BLS (CPR/AED)

Overview:
On-the-Job Training: Completion of 2000 hours with the support and guidance of an experienced MA coach (paid).
Supplemental Coursework: Completion of 410 hours of online coursework (unpaid).
In-Person Lab Days: Attend 3 lab days with an expert instructor to practice and demonstrate mastery of hands-on technical skills.
Reporting: Log and report on-the-job hours and skill practices as required.
Skill Mastery: Successfully perform a required number of skills and be signed off to complete the program as directed by the program requirements.
What You'll Do:
Maintain and update patient medical records, including medical history, current status, vital signs, treatments, and medications. Review and update immunization records.
Prepare for patient visits by reviewing files, ensuring necessary reports are present, and providing updates to the Provider team.
Work with Providers to ensure efficient schedules and continuity of care. Discuss patients scheduled for the day, including pre-visit preparation.
Prepare and maintain patient exam rooms, ensuring appropriate supplies and sterilized equipment are available.
Prepare patients for examinations, explain medical treatment procedures, and provide limited education and coaching within scope. Instruct patients about medication and possible allergic reactions.
Respond to patients' questions and concerns in person and via email. Direct inquiries outside of scope to the appropriate person.
Assist physicians during examinations, procedures, treatments, and minor office surgeries. Anticipate Providers' needs and manage their requests.
Collect and prepare laboratory specimens, complete screenings, and review patient data per protocol. Follow up on test results as directed by healthcare providers and document them in the electronic medical record (EMR).
Prepare and administer medication as directed by a physician.

High School diploma or GED
CPR certification is required within 90 days of hire

Note: Applicants currently in their final year of pharmacy school are encouraged to apply. However, it is expected that all applicants will have graduated from an accredited pharmacy program before the start of the residency in July.

Yakima Valley Farm Workers, in collaboration with Washington State University (WSU) College of Pharmacy and Pharmaceutical Sciences, offers a one-year post-graduate primary care pharmacy residency program. Located in Toppenish and Spokane, WA, this program provides advanced training that surpasses traditional academic learning, undergraduate pharmacy clerkships, and general community pharmacy practice.
Residents will develop and deliver patient care services, focusing on managing chronic diseases such as diabetes, hypertension, cardiovascular risk, hepatitis C, and anticoagulation. They will also contribute to setting and achieving departmental goals to enhance pharmaceutical services and patient care standards.
To meet these objectives, residents will participate in both required and optional experiences within the YVFWC organization. These experiences include primary care services, clinic-based pharmacy management, central fill and specialty pharmacy operations, care for patients with HIV, refugee services, and administrative practices. Additionally, residents will have opportunities to engage in research and gain teaching experience through our partnership with the WSU College of Pharmacy and Pharmaceutical Sciences.
Overall, this program emphasizes delivering pharmaceutical care to improve patient outcomes, regardless of the patient's background or circumstances.
