    Job Description This is an at least part-time position of minimally 8 - 10 hours a week that will provide and oversee comprehensive psychiatric evaluation and medication management services to an especially diverse population within a primary care setting. As a member of the behavioral health team at Community Health Centers of Greater Dayton (CHCGD), this position will function as a collaborator for the PMHNP and PCPs while working collaboratively with the Medical Director, Behavioral Health Director, Behavioral Health Consultants, Primary Care Physicians, and other administrative and operational staff to ensure continuity of care for patients meeting criteria for mental health, substance use, and other related psychiatric illnesses. This position will provide services and collaborative support for patients across the lifespan. The CHCGD organization currently has six site locations and services can be provided as office-based services and telehealth services as well. Principal Duties and Responsibilities Provides psychiatric evaluations and medication management services Provides diagnosis and medication education to patients and their families Functions as a collaborative support for PMHNP, PCPs, APNs, etc. as needed Provides Behavioral Health consultations to Primary Care Providers and other member of the Behavioral Health team as needed Facilitates the hospitalization of patients served who require inpatient care Maintains agreed upon level of productivity Complete or oversees completion of disability questionnaires for behavioral health patients who apply for disability if appropriate Refers patients to appropriate resources in the community as needed for extended or higher level of care Attends regular meetings with the team for coordination of care Attends regular meetings and expected. Formulates plan of care for clients. Documents patient assessment/intervention data in medical records using established medical record forms and documentation practices. Provides patient education to individuals and groups about health maintenance and management of chronic disease process and mental health conditions. Communicates facts, ideas, and skill that change knowledge, attitudes, values, beliefs, behaviors, and practices if individuals and families. Responds to crisis situations according to policy, protocol, and site/organizational need. Demonstrates communication skills that are clear, effective, facilitate patient care and team functioning, and is respectful of differences. SUPERVISORY RESPONSIBILITIES None ESSENTIAL DUTIES AND RESPONSIBILITIES * Oversee day-to-day operations of the 340B program within the organization. * Ensure compliance with all federal 340B program requirements, HRSA guidance, and relevant regulations. * Regularly review contracts, policies and procedures, and OPAIS website for accuracy and compliance. * Provide support to the Risk and Compliance Manager for internal and external audits to maintain audit readiness. * Generate and review reports on program utilization, cost-savings, exceptions, and discrepancies. * Monitor inventories, purchasing records, and split-billing software to confirm program integrity and accurate data tracking. * Analyze data to identify optimization opportunities and compliance risks. * Submit monthly reports to management to include financial, utilization and compliance information. * Facilitate quarterly Pharmacy and Therapeutics Committee meetings. * Serve as a subject matter expert and primary point of contact for all 340B-related matters. * Provide training and ongoing education to staff involved with 340B processes, ensuring they understand policies, procedures, and compliance duties. * Stay informed of and implement changes to 340B regulations and best practices. * Assist CEO in developing and executing strategies to maximize 340b savings QUALIFICATIONS * Bachelors degree in healthcare administration, pharmacy, or public health required. * Experience with 340B program management, auditing, and compliance required. * Proficiency with 340B software, HER systems and Microsoft Suite required. * Completion of C340B (certified 340B administrator) or Apexus/340B University a plus. Obtains check-in information including chief complaint, current medications list, vital signs, etc. Furnishes the patient with appropriate information on health-related issues, as directed by the physician/nurse practitioner. Documents all patient encounters using appropriate medical terminology. Ensures that patient charts are completed and documentation is in the patient's chart. As directed by the physician/nurse practitioner, calls in prescriptions to the pharmacy. Properly inventories and orders routine medical and medically-related supplies and services. Upon physician/nurse practitioner order, advises patients of testing and referral results and ensures that appropriate follow-up call is provided. Explains billing process for SFS patients needing lab work, and lab discount information. Accurately record lab codes and diagnostic codes when drawing blood. Documents and submits billing codes for nurse visits on a daily basis.. Planning and Organizing: Organizes work, sets priorities, and determines resource requirements; determines necessary sequence of activities needed to achieve goals. Documents messages in EMR system, handles or distributes mail and faxes. Prepares patient documentation for clinician (have ER reports, test results, hospital records). Keeps exam room neat, clean and supplied. Responds to incoming and outgoing correspondence. Assures that appropriate signed releases are acquired and office records and forms are distributed and forwarded to appropriate parties. Takes messages off of the telephone line in a timely manner as directed by the Practice Manager. Communication and Teamwork: Participates as an active and contributing member of a team to achieve team goals. Works collaboratively with others, involves others, shares information as appropriate, share credit for team accomplishments. Manages patient flow and pays attention to the needs of the patient and the clinician. Actively supports the Mission, Vision and Values of CHCGD.
