    The Health Systems Strengthening Accelerator (Accelerator) is a five-year program funded by USAID's Office of Health Systems and the Bill and Melinda Gates Foundation that provides catalytic support to countries as they tackle vexing health systems challenges and accelerate progress toward self-reliance. The Accelerator is led by R4D with support from Health Strategy and Delivery Foundation (HSDF) and ICF.

    In its first four years, the Accelerator developed, applied, learned, and communicated about several new approaches to technical assistance (TA) for health systems strengthening (HSS), including applying an equity lens to reimagine TA for strengthening local systems and capacity to deliver more sustainable outcomes. Growing evidence indicates that HSS interventions that prioritize reaching the most excluded populations, such as expanding the role of community health workers, increasing the number of services offered through child health days, and implementing conditional cash transfers (to name just a few), can drive dramatic and equitable reductions in mortality.[1] Much of this literature focuses on the production of more equitable health outcomes. However, evidence (as well as USAID staff and partner experience) is more limited on how HSS interventions may become more equitable in their implementation by leveraging the four approaches to equity described in USAID's Vision for Health System Strengthening 2030: social accountability, social and behavior change (SBC), community systems for health, and intentional engagement of vulnerable populations,[2] in order to advance these equitable outcomes. Considering the ongoing COVID-19 pandemic, these equity approaches may play a key role in building community resilience in the face of future health crises.

    In its fifth year, the Accelerator will develop an “equity capstone” that contributes conceptual clarity, evidence, and programmatic knowledge to the equity foundation laid by USAID's Vision for Health Systems Strengthening 2030.

    Scope of WorkThe Accelerator seeks a global equity expert to develop a conceptual framework and programmatic landscaping that will serve to augment and advance the equity goal of USAID's Vision for HSS 2030. The consultant will build this effort on a previous framing effort conducted by USAID and its partners under USAID's Vision for Health Systems Strengthening 2030, Accelerator and partner experience, and the global evidence base. The framework will serve as a conceptual update and augmentation of USAID's Vision for HSS 2030 Equity section; and it will point to existing materials, cases, and other resources within categories of equity-enhancing approaches. It may also serve as an organizing framework for future equity-focused training/learning by USAID staff and partners.

    Activities and expected deliverables The consultant will lead and/or contribute to the following activities: Validate and augment a conceptual framework for advancing equity goals in the implementation of and outcomes from health systems strengthening efforts.Develop a landscape of existing evidence and learning gaps around equity approaches for health systems strengthening through a consultative process with USAID partners.Provide inputs to a learning module on equity for health systems strengthening for USAID staff (in PowerPoint or similar format).Anticipated Deliverables (to be confirmed)5-10 page framework and corresponding PowerPoint deck Landscape brief with table summarizing evidence and learning gaps Inputs to and comments on learning module Presentation at a virtual webinar

    Timeline and Level of EffortThe consultancy is expected to require approximately 26 days between September 2022 and September 2023. The consultant will work closely with an R4D team of 2-3 people, including a Senior Program Director, Senior Program Officer, and Senior Program Associate, who will provide inputs to work based on Accelerator experience, support coordination with partners and funders, manage the logistics of meetings and review processes, support the development of PowerPoint presentations, and lead the development of the learning module.
