Job Duties:
* Increase access to health services by identifying populations in need using various outreach methods
* Performs assessment based upon the initial reason provided for patient outreach (e.g., no care with primary care provider during the current year, health risk assessment completion, inappropriate emergency department use, recent hospitalization, a gap in routine care, and social need, etc.) and provides or connects to resources relevant to the identified needs.
* Coaches and motivates patients to engage in primary care to effectively manage their chronic disease, engage in preventive care as appropriate, and avoid inappropriate care settings (e.g., emergency department use for non-emergent concerns).
* Actively collaborates with other members of primary care teams to communicate patient needs, special patient arrangements, and areas of further support.
* Perform Social Determinants of Health (SDOH) screenings and address issues identified to eliminate barriers to care as appropriate.
* Provide outreach and engagement services remotely, on-site at Health Centers, or through other community-based settings, including emergency departments or other access points.
Education/Experience:
* Minimum of one year experience (two years preferred) in direct patient/client service exercising one or more CommunityHealthWorker (CHW) core roles (providing culturally appropriate health education, care coordination, coaching, and social support, individual and community assessment, conducting outreach, etc.)
* Completion of Louisiana's CommunityHealthWorker Training Program (CHWTP) and certification process or a substantially similar training/certification program, preferred. (May be completed after hire)

**Position Summary**
This is a full-time field worker opportunity in LA.
CommunityHealth Care Workers (CHW) serve the community as a bridge between the member (community) and the healthcare system (providers) through outreach and education. By providing these services, CHW's help Aetna members attain and maintain better health outcomes, improve relationships with healthcare providers and help both parties to become more acculturated with each other. This position will work under the direction of a designated professional. This position requires in-state travel 50-75% of the time.
Fundamental Components:
· Conduct home visits with Aetna members with chronic conditions to reinforce following the care plan, adjusting tasks to fit with the member's culture, language, and religion, improve access to care, identify health care needs, and reduce unnecessary hospitalizations.
· Educate healthcare professionals and providers on opportunities for improving and understanding the social determinants of health that may be impacting member's health and treatment plan through various communication channels.
· Build and maintain positive working relationships with the Aetna members, providers, nurse case managers, agency representatives, supervisors and office staff, from diverse cultural and socio-economic backgrounds.
· Work to reduce cultural and socio-economic barriers between clients and institutions.
· Assist with care plan implementation and Aetna member education during in-home visits, help develop care management strategies, and work with team members to provide linkages for the various health and social needs of members.
· Assist members to identify socio- economic issues that affect their overall health and develop health/social management plans and goals.
· Identify gaps and opportunities to strengthen systems of care within the member's community and assists members in utilizing community services, including scheduling appointments with social services agencies, (including transportation vendors), and assisting with completion of applications for eligible programs.
· Assist with patient medication adherence by: instructing the member on current medication list, reviewing medications with member and assist in obtaining refills.
· Teach disease self-management (i.e., nutrition, symptom tracking and reporting).
· Accompany members to appointments as needed.
· Enter and maintain member records in electronic health record system, compile reports and complete other program documentation in a timely manner (e.g., progress notes, letters), and other administrative responsibilities as needed.

Collaborates with HIM and physicians to ensure appropriate capture of coding that supports medical necessity criteria. Communicates CMG information to the interdisciplinary team in order to facilitate care planning.
Communicates approval status with referral sources, patients and families in a timely and effective manner. Interacts with team members to facilitate alternate placement in the event of denial or inability to admit to OLOL Rehabilitation Center.
Communicates information with patients and families about the rehab physician, expectations of the inpatient program, discharge planning, family training, and team conferences. Provides tours of the unit upon request.

Routinely visit and call referral sources to maintain quality relationships.
Inform referral sources of changes in services and contracts as well as achievements, building or equipment upgrades, goals met, and activities taking place at the nursing home.
Assist the Internal Admissions person in making the transition into our facility smooth and positive.
Assess possible admissions to deem appropriateness for placement.
Communicate with marketing team, referral sources, residents, residents' representatives, and the facility to coordinate smooth admission and/ or re-admission into our facility.

May assist with completion of admissions paperwork with the resident or their representative(s).
Inform the Interdisciplinary Team (IDT Team) of anticipated admission and obtain room number.
If in the facility, check resident's room for readiness prior to arrival: cleanliness, odors, bed made, working call light, water pitcher and overbed table present, all light bulbs working, soap and paper products stocked, walls, floors, and ceilings in good repair, etc.
May coordinate or participate in facility tours with potential admissions/referrals.
Follow up with new admissions or their responsible party to ensure satisfaction shortly after admission and thereafter (until resident becomes acquainted with new environment).
Maintain open communication lines with residents and their representatives after completion of the admissions process. Help refer them to the appropriate staff member for issues or concerns. Also, a friendly call to give an update on their loved one is impactful.
The Community Liaison may participate in Daily QA (morning) Meetings, Monthly/ Quarterly QA, Resident Council Meetings if requested, Care Plan Meetings upon request, in-service trainings, continuing education programs, and all other meetings called by Legacy or licensing entities.

Participate in community/ marketing events.

The case manager is expected to report to the shelter at least three times per week for check-ins, meetings, resource coordination, and team building.
Staff in the Housing Program of St. Vincent de Paul (“SVDP”) of Baton Rouge work in unity with all “SVDP” personnel, embracing the organization's mission of charity and justice. They collaborate with others of goodwill to alleviate need and address its root causes, making no distinctions among those served-recognizing in each person the face of Christ. Housing Program staff also actively participate in the local Continuum of Care to coordinate services, enhance support for guests, and reduce duplication of efforts
MINIMUM QUALIFICATIONS:
Education, Experience & Certifications:
BSW degree from an accredited university/college required.
Current license as a social worker with Louisiana State Board of Social Work Examiners.
Class E Driver's License and safe driving record.

Knowledge and experience working with co-occurring disorders (mental health and substance use) is preferred.
BLS for healthcare provider is required.
