The CommunityHealthWorker (CHW) serves as a Lung Health Navigator, combining outreach, engagement, and patient navigation to improve access to lung cancer screening for individuals at high risk. While supporting all community members, this role places a special emphasis on populations disproportionately affected by lung cancer, particularly American Indian and Alaska Native communities.
As part of a collaborative team, the CHW identifies people at high risk for lung cancer, determines eligibility for screening, and provides navigation support to coordinate screening and follow-up across community clinics, Fred Hutch Cancer Center, and partner organizations. Key responsibilities include outreach and navigation support to help individual address barriers to screening and ensure timely follow-up to support optimal care.
Responsibilities
* Assists with implementation, management and tracking of a Tobacco Use survey.
* Assists with patient navigation to appropriate Fred Hutch smoking cessation programing and lung cancer screening, and to community-based resources as needed.
* Provides smoking cessation counseling and associated resources to community members.
* In collaboration with the Fred Hutch American Indian/Alaska Native Patient Navigator, assists in culturally appropriate patient / family transitions between community-based care and Fred Hutch.
* Serves as a cultural broker/representative among Fred Hutch clinical and həliʔil Program staff, maintaining ongoing communication with affiliated programs and institutions, while representing the həliʔil Program to community partners and Indigenous/Tribal organizations.
* Fulfills and maintain all reporting requirements in accordance with Fred Hutch and həliʔil Program policies.
* Maintains metrics as per həliʔil Program guidelines, and identifies trends/gaps.
* Participates in staff meetings, activities and trainings to improve system coordination.
* Participates in orientation and training of Fred Hutch staff regarding patient education /patient navigation/coordination of services and cultural beliefs and practices of the population served.
Qualifications
Required:
* High School Diploma or equivalent
* Minimum of 2 years working in a hopsital or healthcare setting.
* Minimum of 2 years in patient advocacy, navigation, and/or coordination of care.
* Experience working with chronic disease management among Indigenous/tribal communities.
* Previous experience in tobacco education or cessation services.
* Familiarty and/or experience with the Indian Health Service (CHSDA) and government sponsored health care grants/initiatives for Indigenous communities.
* Experience in advocacy, outreach, coordination and collaboration with Indigenous communities and health care settings.
* Must have and maintain valid WA State Driver's License, valid insurance, and have clear driving record.
* Excellent interpersonal skills, written and verbal communication skills, and proficiency with
* Microsoft Office applications required (including Microsoft Outlook, Word, Excel, Power Point, Access and Visio). Attention to detail required, ability to troubleshoot or anticipate change desirable.
* Understanding of, and competence in, highly professional realm requiring diplomatic communications with internal and external groups, with effective fielding of inquiries and professional representation of the program.
* Works well independently with minimal supervision and is expected to use initiative and sound judgment in carrying out responsibilities within the framework of the department procedures.
* Must have a valid Washington Driver's license. Travel required: approximately 10%, may be required for overnight travel for meetings and events. Ability to work evenings and weekends
* Self-motivated team player, with demonstrated ability to work independently with minimal supervision. Diplomacy and understanding of political sensitivities. Tactful approach in handling confidential and sensitive information and a high sense of responsibility and discretion.
* Demonstrated understanding of medical terminology.
* Knowledge of health care and social practices in Indigenous communities.
* Experience and demonstrated ability in providing direct services to Indigenous families.

* Responsible for providing consistent documentation and patient follow to complete applications processes and ensuring correct assignment of insurance which may include helping patients switch insurance or insurance assignment by completing switch forms or contacting insurance providers directly on behalf of the patient.
* Maintain confidentiality of patient information in line with exchange and HIPAA guidelines by properly documenting patient interactions in approved programs, and meeting with clients in private spaces where patient information can be openly shared
* Required to review, build and work reports that may require outreach to specific patient populations to ensure HealthPoint patients are receiving eligible education on available services which includes uninsured, unengaged, and aging out reports
* Responsible for remaining current and participating in Annual Enrollment and Open Enrollment periods which includes actively sharing information with sites on performance, updates and changes along with being a point of contact for questions
* Represent HealthPoint at community events as determined by leadership to provide insurance, Basic Food, HealthPoint and other resources to clients.
* Attend internal and external meetings required of the role, which may include presenting material or sharing of information
* Assist in the development and implementation of outreach activities and materials to enhance and improve customer service and client relationships through internal and external relationships.

Job Summary:
The Health Services Coordinator serves on the health services team supporting the overall operations of the department through a variety of administrative and clinical functions as directed by the Director of Health Services (DHS). The Health Services Coordinator preserves dignity and promotes independence for each resident while providing care and services according to each individual service plan and in accordance with MBK policy and procedure.
Duties & Responsibilities:
Conduct and coordinate assessments / evaluations of potential residents and make recommendations for admission in accordance with current rules, regulations, and community policies and procedures that govern resident assessment.
Draft initial individualized Service Plans and update as needed.
Review service plans with responsible parties and Executive Director as requested by DHS.
Ensure continuity of the assisted living residents' total care regimen.
Under the direction of the DHS, provide training and education as needed on a range of essential topics including competent medication delivery, acceptable treatments, safety protocols, emergency procedures, accurate record, and state requirements to provide the best possible resident care.
Perform all assigned duties accurately and timely including required documentation.
Serve as a medication technician, if needed.
Provide coverage of job duties within the department during absences, either through assistance in finding coverage, or personal completion of duties.
Check vital signs as directed and look for signs that health is deteriorating or improving.
Perform basic nursing functions such as treatments, medication delivery and managing resident emergencies ensuring residents are comfortable, well-fed, and hydrated.
Maintain adequate inventory of resident care supplies and demonstrate a commitment to minimizing waste of supplies and equipment.
Coordinate prescription orders with doctors' offices and manage pharmacy delivery of medications including oversight of the central storage, tracking and delivery of medications, and ensuring Medication Administration Records are completed according to company policy and State regulations.
Aid in the community marketing effort through positive interactions, acting as a liaison between the community, and families/outside health service providers.
Maintain a safe and secure environment for all staff, residents, and guests following established safety standards, policies, and procedures.
Understand and comply with all Federal, State, and local regulations, and all company policies and procedures.
Promote a spirit of teamwork and open communication in accordance with the MBK principles and core values.
Perform other job duties or special projects as assigned or requested by the Supervisor or Executive Director.
Education Requirements:
RN, LVN or LPN License that is active and in good standing is essential; adherence to all requirements to maintain license including CEU completion and timely renewal.
Experience Requirements (in years):
2+ years of prior related work experience functioning in a similar healthcare environment.
Required Competencies/Licenses/Certifications:
Must complete required Background clearances, health screening and provide negative TB test results within 7 days of employment (must be within the last 6 months).
Valid state driver's license and valid insurance or reliable method of transportation required.

POSITION SUMMARY
The Community Resource Coordinator provides services through the Resident Opportunities Self Sufficiency (ROSS) grant and/or other Self Sufficiency grants and other funding by assisting BHA program participants in achieving their economic opportunity goals and by connecting them to organizations with training, job placement and other opportunities to increase earned income while reducing their dependency on public assistance and rental subsidies. Perform case work for programs such as Home Ownership or other self-sufficiency programs by helping participants set goals, document and track progress, conduct follow- up appointments, provide community resources, and motivate or recognize participants when goals are met.
Back up other Community Resources Coordinators and program activities such as the Family Self Sufficiency (FSS) grant, Family Navigation, and Rental Assistance Program.
POSITION QUALIFICATIONS
Required Education and Experience
Education: Associate's Degree (two-year college or technical school) Required, Field of Study: Social work or related field.
Prefer Bachelor's Degree (four-year college or technical school), Field of Study: Social work or related field.
Experience:
3 plus years of experience in creating marketing materials, creating reports through databases or excel, and writing case notes.
3 plus years of direct customer service experience.

General knowledge of low-income housing programs would be helpful.

As a Community Advocate you will provide trauma informed advocacy to adult survivors which may include support with protections orders, family law, CPS, housing navigation, safety planning and life skills, employment, etc. You will use a model of best practice trauma-informed advocacy with a strength-based approach.
Note: This position is fully onsite at YWCA Offices. RESPONSIBILITIES
Provide support to clients in navigating various institutions (i.e. Support clients and legal systems including assistance with filing protection orders, breaking leases, relocations, CPS, and interactions with law enforcement.
Responds to referrals and calls within required time frame.
Conducts intakes with new clients, including screening for eligibility and determining essential needs.
Housing assessments for new move-ins for emergency shelter.
Provide crisis intervention advocacy.
Collects and completes reporting and quarterly narratives using data and statistical information.
Develop and maintain relations with community partners and attend community meetings.
Facilitate and create presentations that are relevant to associated partners and offers services.
Engage in community networking activities to both increase access to services for survivors furthest away from opportunity specifically Black and African American survivors.
Maintain 20 hours of continued education hours about advocacy-based Domestic Violence.
Actively engages in agency-wide Race and Social Justice Initiative (RSJI) and strives for racially equitable outcomes; takes responsibility for creating and maintaining a safe and welcoming community by making room for populations who routinely encounter system oppressions
QUALIFICATIONS & CORE COMPETENCIES
(1) year of Direct Social Service experience or translatable lived experience in lieu of a social services degree.
Strong orientation and familiarity toward system navigating multiple systems (i.e., municipal, superior courts, legal systems, and law enforcement)
Experience working with survivors from marginalized communities, specifically African American communities.
Knowledgeable about gender-based violence, such as sexual assault, domestic violence, intimate partner violence and human trafficking.
Skilled at using Microsoft office suite including Outlook, Calendar, Teams, excel and virtual meeting technology such as ZOOM.
Valid Driver's License and ability to travel independently between multiple work sites within a day.
Willingness to complete a minimum of 20 hours of initial basic training that covers: advocacy-based counseling, history of domestic violence, safety planning and crisis intervention

What You'll Do
YWCA Seattle|King|Snohomish, one of the largest nonprofits in the region, focused on the needs of women, girls, and families in the community, is searching for a Community Advocate to join their team in the South King County, WA area.
The Community Advocate works within Gender-Based Violence Services at YWCA and provides culturally relevant services that are designed to meet the needs of survivors of domestic violence/sexual assault and commercial sexual exploitation (CSE).
As a Community Advocate you will provide trauma informed advocacy to adult survivors which may include support with protections orders, family law, CPS, housing navigation, safety planning and life skills, employment, etc. You will use a model of best practice trauma-informed advocacy with a strength-based approach.
Note: This position is fully onsite at YWCA Offices. RESPONSIBILITIES
Provide support to clients in navigating various institutions (i.e. Support clients and legal systems including assistance with filing protection orders, breaking leases, relocations, CPS, and interactions with law enforcement.
Responds to referrals and calls within required time frame.
Conducts intakes with new clients, including screening for eligibility and determining essential needs.
Housing assessments for new move-ins for emergency shelter.
Provide crisis intervention advocacy.
Collects and completes reporting and quarterly narratives using data and statistical information.
Develop and maintain relations with community partners and attend community meetings.
Facilitate and create presentations that are relevant to associated partners and offers services.
Engage in community networking activities to both increase access to services for survivors furthest away from opportunity specifically Black and African American survivors.
Maintain 20 hours of continued education hours about advocacy-based Domestic Violence.
Actively engages in agency-wide Race and Social Justice Initiative (RSJI) and strives for racially equitable outcomes; takes responsibility for creating and maintaining a safe and welcoming community by making room for populations who routinely encounter system oppressions
QUALIFICATIONS & CORE COMPETENCIES
(1) year of Direct Social Service experience or translatable lived experience in lieu of a social services degree.
Strong orientation and familiarity toward system navigating multiple systems (i.e., municipal, superior courts, legal systems, and law enforcement)
Experience working with survivors from marginalized communities, specifically African American communities.
Knowledgeable about gender-based violence, such as sexual assault, domestic violence, intimate partner violence and human trafficking.
Skilled at using Microsoft office suite including Outlook, Calendar, Teams, excel and virtual meeting technology such as ZOOM.
Valid Driver's License and ability to travel independently between multiple work sites within a day.
Willingness to complete a minimum of 20 hours of initial basic training that covers: advocacy-based counseling, history of domestic violence, safety planning and crisis intervention

Participate in all required Lifestyle Coach discussions (meetings, emails, or conference calls) and professional development opportunities, offering ideas and tools for facilitation to other team members.

Record weekly data for each participant (attendance, weight, physical activity minutes, food tracker completion) in electronic database according to data entry timing requirements. Participate in program progress and outcome tracking.
Provide physical activity coaching tailored to meet the various fitness levels of participants.
Qualifications:
At least one year of experience teaching lifestyle behavior change classes preferred.
Experience with referral management and/or care coordination to various services.
Knowledge of the fitness components of an aerobic or group exercise class and strength training.
You are committed to valuing and promoting diversity and contributing to an inclusive working and learning environment.
The ability to demonstrate skills in accordance with YMCA standards and effectively lead a safe aerobic/group exercise class.
