Records and monitors the participants' progress toward goals within specific timeframes. Documents assessments and key patient updates in EMR system; documents relevant day-to-day activities and patient data. Prepares reports and documents as needed or requested. Assists patients with organizing their records, making follow-up appointments, attending follow-up appointments, and filling their prescriptions. Helps patients fill out applications, for example for Medical Assistance, Housing, and SNAP (Supplemental Nutrition Assistance Program). Provides advocacy, patient education and successful warm hand offs in accessing community-based and hospital-based programs. Assists patient in addressing and overcoming barriers with a range of concrete supports, including but not limited to: healthcare support services, behavioral health, financial assistance, child-care and caregiver support, housing, support with utility bills, food, financial entitlements, clothing, transportation, food pantries, violence prevention, social isolation and any other appropriate community resources. Provide intensive home and community-based outreach, motivational interviewing and goal setting, resource connection and accompaniment to medical appointments as needed to help patients appropriately utilize healthcare. This position will contribute to these MGH efforts through collaboration with MassGeneral Hospital for Children (MGHfC) and the Department of General Internal Medicine (DGIM) Population Health Management teams. MGHfC oversees a team of Community Health Workers. CHWs are trusted members of the community with the skills and experience to understand their patients' circumstances. By building trusting relationships and walking alongside their patients, CHWs help address medical and psychosocial needs in order to promote self-efficacy, help patients meet their goals, and improve health outcomes. The Community Health Worker (CHW) will be part of the pediatric CHAMPION Program's multi-disciplinary care team, which includes a physician and a dietitian. The evidence-based CHAMPION program and research study seeks to evaluate whether telehealth vs. in-person delivery of CHAMPION is equally effective at reducing child BMI and asthma control in diverse lower-income families. The CHAMPION program consists of individual and group visits guided by a set curriculum with a multi-disciplinary team (medical provider, dietitian, and community health worker), and other tools to support behavioral change and reduce body mass index (BMI) and asthma, such as follow-up phone calls, educational materials, social determinants of health screening and referral and text messaging. The CHAMPION CHW will provide care to children and adolescents with overweight/obesity and asthma, through group and individual visits, both in-person and virtually, and will participate in a Virtual Learning Community and Quality Improvement calls and activities as part of the research project. The CHAMPION CHW will support this program at the MGH Chelsea HealthCare Center and MGH Revere HealthCare Center. Hours are expected to be roughly 10am-6:30pm Mondays, Tuesdays and Thursdays based on clinical schedules, and Wednesdays and Fridays, 8:30am-5pm. The CHAMPION CHW will engage patients and their families in setting their own short-term goals and will track the benchmarks along the way toward the achievement of these goals. In addition, the Asthma -Healthy Weight Lifestyle CHAMPION CHW will work with patients to help decrease barriers to timely follow-up care and provide coaching to engage patients and families in identification and achievement of care goals. This is not a clinical position, but the role is part of the clinical team and requires a good knowledge of (and willingness to learn) basic clinical concepts and an understanding of when a referral to a licensed clinician is appropriate. Qualifications Key Areas of Responsibility: Works as an effective team member of the MGHfC and DGIM population health management program to provide health coaching and care coordination to patients and families. The ICT CHW serves as a liaison between the ICT members and external community-based service providers as needed, based on the needs of the patients. The ICT CHW is committed to continuous professional development within its scope of practice, including acquires and maintaining certification and additional training as applicable. As a care coordinator, manages a caseload of patients and follow them longitudinally to facilitate care planning and achieve goals. Job Responsibilities and Performance Standards: HRSN Screening and follow-up Management For positive screening results, further assess and formulate a plan of care in order to address patient goals. Implement the plan of care, including providing health education, facilitating access to needed services such as assists patients in obtaining or stabilizing housing, finances, food, utilities, educational/vocational opportunities, and community supports. Monitor patient progress over time, including making referrals to service providers and coordinating care as needed per plan of care-established goals. Communicates patient updates to the ICT team and modifies plan of care as needed Completes necessary documentation, i.e. (utility assistance, SNAP, disability, SSI, DTA, housing) Engagement Establish trusting relations with patients to facilitate their connection to the primary care team. Implement patient engagement strategies for patients identified as hard to reach and pose barriers to primary care access. Providers transitions of care updates to the integrated care team, including participation in pre-visit planning activities Engages (outreaches) with patients between visits either by phone, home, or community visits.
