Key Responsibilities
Client Journey Management
Serve as the clinical continuity partner from onboarding through renewal.
Ensure all diagnostics (genetics, bloodwork, imaging, intake) are complete before the first physician consult.
Be present on every physician-patient call to ensure communication needs are met, accurate notes are taken, and the health plan is updated in real time.
After each consult, finalize notes and coordinate scheduling of future calls with the Longevity Strategist and with the Physician.
Provide ongoing education, accountability, and coaching tailored to diagnostics.
Maintain consistency and a personalized experience through proactive check-ins and responsiveness in partnership with the Lead Care Concierge.
Diagnostic & Data Oversight
Ensure all diagnostics (genetics, bloodwork, imaging, wearables, intake) are completed, integrated into the client's record, and translated into action.
Manage all client data in the Catalyst tier in coordination with lead physicians.
Support the physician in real-time clinical documentation during calls.
Maintain and update the client's digital health report, roadmap, and active plan.
Monthly Coaching & Lifestyle Optimization
Review the client's health goals and clinical priorities.
Conduct structured monthly coaching calls (and interim touchpoints as needed).
Provide personalized support on sleep, stress, nutrition, movement, and daily routines.
Track biometric and behavioral data (e.g., Oura, CGM, Whoop) and proactively guide adjustments.
Review and troubleshoot supplement protocols and therapeutic response.
Provide tactical advice, mindset support, and motivation based on the client's life context.
Coordinate any necessary escalations or new diagnostics.
Therapeutic Management & Escalation
Coordinate regenerative therapy timelines and track response (exosomes, peptides, HBOT, etc.).
Actively participate in Longevity Strategy Team case reviews.
Serve as the clinical filter and translator ensuring complex data becomes clear, simple actions.
Ensure direct access to physicians for VIP clients while buffering unnecessary demands.
Collaborate via a shared communication platform (chat/email) that includes coach + physicians to maintain streamlined, visible communication.
Monitor client needs and flag urgent issues or escalations for physician's attention.
Reporting, Progress Tracking & Renewal Support
Help build, maintain, and present the Custom Annual Health Report.
Track clinical KPIs (labs, HRV, glucose trends, symptom resolution) and create narrative summaries.
Highlight progress wins for use in renewal framing and strategic updates.
Comfortable with modern technology tools, including AI prompting.
Ideal Candidate Profile
Must-Have Experience & Skills
3+ years in health coaching, functional medicine, or precision health environments.
Deep understanding of advanced diagnostics, data interpretation, and integrative therapies.
Proven ability to translate science into action for high-performing clients.
Familiarity with wearables, labs, peptides, and complex supplement stacks.
Excellent communication skills, empathetic, confident, and able to hold clients accountable.
Comfortable navigating relationships with physicians, executives, and UHNW individuals.

The PIC Community Health Worker works closely with medical and behavioral health care teams; fellow Optimus sites CHWs, and social services agencies to provide care coordination, connection to resources and support to improve clients' health and general well-being. Works in both clinical and community-based settings. Under the supervision of the PIC Program Director, they assess and provide interventions to aid patients to cope with social, emotional, economic, and environmental problems.
ESSENTIAL FUNCTIONS & RESPONSIBILITIES
1. Completes social determinants of health assessments, person-centered recovery action plans oriented to the client's cultural background including gender identity/sexual orientation. Work with teams to create a trauma informed environment for patients.
2. Provides behavioral health resources and interventions when needed (ex: providing breathing exercises for anxiety or sleep hygiene suggestions)
3. Schedules and behavioral health screenings, face-to-face whenever possible and clinically appropriate. Supports safe transitions of care for members moving between care settings.
4. Works with primary care providers and other CHWs to facilitate referrals to behavioral health department, works with patients to increase compliance with attending intakes, appointments and assists decreasing barriers to participation.
5. Assists clients in the clinic setting. Continuously identifies and resolves barriers to meeting goals and complying with the Individual Recovery Plan and reports barriers identified to the PIC Program Manager.
6. Documents all client encounters and care coordination efforts made on behalf of clients; maintains comprehensive electronic client files in a consistent and timely fashion.
7. Works with PIC team to provide accurate data collection for program reports as well as Optimus team-based care reporting needs.
8. Coaches and facilitates communications with clients in effective management of self-care. Assists clients in understanding care plans and instructions. Motivates clients to be active and engaged participants in their health and overall well-being.
9. May provide support and advocacy during medical and behavioral health visits or when necessary to guarantee clients' behavioral health and medical needs are being conveyed. Follows up with both clients and providers regarding action plans.
10. CHW will be held accountable and assessed by targeted measures from the PIC Grant.
11. Facilitates client access to community resources, including housing, food, and clothing assistance, transportation, parenting, providers to teach life skills, vocational, educational resources, and relevant mental health services. Assists clients in utilizing community services, facilitating appointments with community services agencies as well as with completion of applications for programs for which they may be eligible.
12. Works collaboratively and effectively within the care team. Establishes positive, supportive relationships with participants and provides feedback to other members of the team. Builds and maintains positive working relationships with the clients, providers, nurse case managers, agency representatives, supervisors, and office staff. Works to reduce cultural and socio-economic barriers between clients and agencies.
13. Travels as needed to community locations, various agencies, and other outreach destinations.
14. Attend meetings as scheduled or as requested.
15. Participate in supervision with supervisor as required.
16. Performs other duties as assigned.
ADDITIONAL GENERAL REQUIREMENTS

JOB QUALIFICATIONS/REQUIREMENTS
EDUCATION: High School Diploma required. Bachelor's degree in social services preferred.
EXPERIENCE: Previous experience in working with community-based programs for persons with behavioral health diagnoses. Preferred: Applicant has a well-developed understanding of chronic disease and its impact on behavioral/mental health treatment. Experience working with an ethnically, culturally, and racially diverse office staff and patient population.

Program Description:
Queens Global Families, the Perinatal Infant Community Health Collaborative (PICHC), is an initiative designed to improve maternal and infant health in the communities of Queens borough and to strengthen the network of health and social service providers in the area. The Community Health Worker (CHW) will work with low-income, high-need women of reproductive age to provide support and education with the purpose of informing their health decisions and behaviors and connecting them to resources through one-on-one interactions, home visits, and groups within the community. CHWs work with families to identify needs and goals, provide health education and support, and refer them to additional community resources.
Summary of Position:
The community health worker (CHW) will complete home visits on a regular basis as per the program description and mandates. The CHW will provide education and support with resources based on client's needs. The CHW will also run groups to support families within the program with the same goal of supporting health education and resources to the client.
Specifically the, Community Health Worker will:
Identify and assist individuals and families to access needed preventive and primary health care services (e.g., family planning, prenatal care, immunizations, pediatric care, WIC and other nutrition services, inter-conception care, etc.), with an emphasis on identifying and supporting pregnant women not enrolled in health care or supportive services
Provide assistance and/or referrals to obtain other essential support services such as housing, financial aid, food stamps, emergency food, clothing, transportation, immigration support, translation, and childcare
Support participants to develop birth plans and educate them on their reproductive rights and reproductive justice
Conduct basic health assessments, assist families to identify needs, provide basic health information, and make appropriate referrals through home visits
Provide advocacy, support and follow-up to determine if services are received and assist families with health behavior changes
Provide home visiting services for up to 25 clients at any given time and help conduct educational support groups.
Conduct community outreach to screen and connect community members to needed services.
Providing education in a group setting on prenatal care, post-partum support group, family planning education, circle of caring groups and other groups as developed by the program.

Qualifications
