The HealthWorker (CHW) will work with patients, families, care providers, and clinical care teams in partnership with the community to help patients achieve optimal health goals. The CHW will provide patients with support, education, referrals, and resources through comprehensive, coordinated, patient and family-centered care. The CHW will actively engage with patients in the community through outreach, home visits, and bridging the patient to clinical support and resources. The CHW will provide culturally appropriate, non-judgmental, compassionate care, while maintaining professional accountability and engaging in continuous quality improvement.
This role will be based out of the Women's Center in St. Louis Park with the expectation to support multiple locations/clinics.
Work Schedule:
* M-F, 7a-6p varied, occasional evening or weekend may be required
* 40 hours per week/1.0 FTE
Required Qualifications:
* Licensure/ Registration/ Certification:
* CHW certification within 18 months of hire
* CPR certification required
* Knowledge, Skills, and Abilities:
* Effective verbal and written communication with patients, families and co-workers required
* Maintains patient records and documentation in the electronic health record (EHR)
* Must have a valid driver's license and reliable personal transportation
* Demonstrated organizational and time management skills
* Motivational Interviewing skills

Summary:

General Job Statement:
The CommunityHealthWorker (CHW) will be working with the Pregnancy & Family Health team. This position requires a strong desire to work with birthing persons, partners, children and families. Pregnancy & Family Health programming assists guests in alleviating barriers in order to reduce health disparities and ensure healthy outcomes for birthing persons, babies and families. Some aspects of the role will include: assessing guest needs, hosting events and support groups, providing resources and referrals, providing and connecting guests to culturally specific care and support.
Minimum Qualifications:
High school diploma or GED and four years of experience in human services or community outreach or Associates degree with two years of experience in human services, or community outreach, preferred.
Must possess demonstrated ability to relate to individuals and families of varied ethnic, cultural backgrounds, ages and economic circumstances.
Experience and ability to build and develop constructive working relationships with a variety of individuals, assist with establishing a work plan based on assessment.
Demonstrated ability to execute outreach plans, collect data, and prepare reports.
Excellent verbal, written, analytical and group communication skills working with a multi-cultural environment.
The ability to fluently speak English and Spanish.
Valid Minnesota driver's license with good driving record, current insurance and ability to pass NorthPoint's driving policy.
Must be proficient in MS Word, MS Excel and computer tracking database.
Work Environment:
Work is performed in a standard food shelf environment with regular exposure to outside weather. Work involves lifting a minimum of 50 lbs or more on a regular basis, standing, stooping, lifting and pushing six to seven hours daily, requires hand-eye coordination in the use of equipment, organizing and shelving food items.

This position works directly with pharmacy patients, medical and nursing staff to manage complex medication needs. This includes: building and maintaining professional relationships with providers, patients, families, insurers, other units, and staff, coordinating workflow, ensuring compliance with state and federal regulations, anticipating business needs to assist in the growth and success of the business, inventory control and complex compounding and distribution of pharmaceuticals, IV solutions and documentation as ordered by prescribers.
Chemotherapy/IV room experience is ideal for this position!
* Maintain the IV hoods or isolators, clean room, ante room according to manufacturer's specifications and regulatory requirements as applicable by location and/or business need.
* Use Epic functionality of compounding/repackaging for anticipatory compounded
* Handle complex drug inventories for clinics
* Complex compounding including complex cancer treatments, home infusion pumps, investigational drugs, and serial dilutions for cytotoxic desensitization.
* Partner with care providers and pharmacists to ensure that patient receives the needed medication accurately, safely, in accordance with all legal requirements and policies and in a professional and timely manner.
* Assist in resolving any billing issues, obtaining prior authorization for medications as needed, verifying eligibility and escalating as appropriate.
* Under the direction of the pharmacist, utilize the appropriate computer system and protocols, prepare drug therapy and/or dispense medication in an accurate and timely manner in accordance with all legal, regulatory agency and Fairview requirements.
* Sterile compounding of specialty medications for a variety of disease states including cancer treatment. Compounding includes hazardous medications.
* Coordinate ordering, delivery of medications, supplies and equipment to patients, cancer clinic, and/or infusion center if applicable. May include other clinical areas as requested (ex: Fairview Home Infusion).
* Data collection, reporting, process improvement and maintenance of patient education materials.
* Maintain the IV hoods or isolators, segregated compounding area, clean room, ante room according to manufacturer's specifications and regulatory requirements.
* If applicable, utilize Epic functionality of compounding/repackaging for anticipatory compounds.
* Maintaining inventory and records for free drug program inventory.
* Maintaining inventory and records for treatments/supply obtained through our retail sites, when required by payer.
* Maintain REMS (Risk Evaluation and Mitigation Strategies) trainings and certifications specific to the site: Tysabri, Lemtrada, bispecifics, etc.
