The CommunityHealthWorker participates as a member of the inter-disciplinary care team (ICT) to coordinate care for members.
Essential Functions:
Engage with the member in a variety of settings to establish an effective, professional relationship. Settings for engagement include but are not limited to; hospital, provider office, community agency, member's home, telephonic or electronic communication
Accompany members to appointments and other social service encounters when necessary
Coordinate logistics to support members' care plan goals and interventions - reminders, transportation, and childcare arrangements
Verify eligibility, previous enrollment history, demographics and current health status of each member
Contribute to assessments by gathering information from the member, family, providers and other stakeholders
Contribute to the development and implementation of the individualized care plan based on member's needs and preferences, reporting information to the Case Manager
Assist with identifying and managing barriers to achievement of care plan goals
Assist with empowering the member to manage and improve their health, wellness, safety, adaptation, and self-care through effective care coordination
Assist with the provision of health education and wellness materials as directed by the Case Manager(s) or Team Lead
Evaluate member satisfaction through open communication and monitoring of concerns or issues
Maintain appropriate documentation within protocols and guidelines of the Care Management program
Looks for ways to improve the process to make the members' experience with CareSource easier and shares with leadership to make it a standard, repeatable process
Regular travel to conduct member, provider and community based visits as needed to ensure effective administration of the program
Perform any other job duties as requested
Education and Experience:
High School Diploma or General Education Diploma (GED), is required
Minimum of two (2) years of experience in either volunteer or paid position working in community settings with at risk populations providing coordination of services is preferred

$35,900.00 - $57,300.00 CareSource takes into consideration a combination of a candidate's education, training, and experience as well as the position's scope and complexity, the discretion and latitude required for the role, and other external and internal data when establishing a salary level. In addition to base compensation, you may qualify for a bonus tied to company and individual performance. The CommunityHealthWorker is responsible for assessment, planning and implementing care strategies that are individualized by patient and directed toward the most appropriate, least restrictive level of care. They also Identify and initiate referrals for social service programs; including financial, psychosocial, community and state supportive services, and manage the care plan throughout the continuum of care as a single point of contact for the member. As a CommunityHealthWorker (CHW), you will act in a liaison role with Medicaid members to ensure appropriate care is accessed as well as to provide home and social assessments and member education. The coordinator also addresses social determinant of health such as transportation, housing, and food access.
Working Schedule: Monday through Friday between the hours of 8 am to 5pm. No nights, weekends, or holidays.

Primary Responsibilities:

Review plan benefits and identify appropriate programs and services based on health needs and benefits
Integrate health care and service needs into a plan or recommendation for member care and service

The **Health Educator** is responsible for responding to inbound inquiries for disease management or medication alert programs.
**How you will make an impact:**
+ Provides information to members eligible for disease management or medication alert programs.
+ Makes outbound attempts to program members to provide health information according to program standards.
+ Receives or places calls to members who have failed to enroll with disease management to encourage their enrollment in the correct disease management program.

**Minimum Requirements:**
+ Requires BA/BS in appropriate field of specialization and minimum of 2 years of related experience in direct care health education exercise instruction or patient education; or any combination of education and experience which would provide an equivalent background.
**Preferred Experience, Skills, and Capabilities:**
+ Prior experience in health coaching, disease management in a call center environment, and/or patient education and behavior change techniques and strong understanding of disease management principles is preferred.
+ Certification or advanced certification and/or license appropriate to field of specialty preferred.

Position Detail:
The Community Liaison serves as a vital connector between our healthcare services and the community. This role is responsible for driving business development and managing both inbound and outbound communications. The Community Liaison acts as the local market leader for B2B (business-to-business), B2C (business-to-consumer), and DTC (direct-to-consumer) engagement, representing our organization to referral partners, patients, and caregivers.
Key Responsibilities:
Business Development & Outreach:
Identify opportunities for in-services in the health care community and coordinate their implementation with appropriate persons
Build and maintain strong relationships with referral sources such as hospitals, physician offices, senior living communities, and other healthcare providers.
Identify and pursue new business opportunities to grow patient census across service lines.
Represent the organization at community events, health fairs, and professional networking functions.
Patient & Family Engagement:
Educate patients, families, caregivers, and legal representatives (POAs, guardians) on the benefits and services offered through Hospice.
Serve as a compassionate and knowledgeable resource to guide families through the enrollment process.
Communication & Coordination:
Develop relationships with medical staff offices, hospitals and nursing homes; act as a consultant to medical staff and hospital committees, as required to provide hospice and end-of-life information
Facilitate seamless communication between referral sources and internal clinical teams.
Ensure timely follow-up on referrals and maintain accurate documentation of outreach activities.
Market Leadership:
Provide formal and informal education opportunities with physicians, physician office staff, hospital social workers/discharge planners/patient unit staff, nursing home staff and staff in community agencies.
Collect and analyze data of referral patterns from all referral sources, to include physicians, social workers, discharge planners, hospital vs community physicians and SNFs.
Maintain a thorough knowledge of the community's health care market, competition, consumer demands and governmental policies which influence the agency's outreach goals and access for patients/families
Act as the face of the organization in the local market, promoting brand awareness and trust.
Monitor market trends and competitor activity to inform strategic planning.
Skills and Experience Required
Proven experience in healthcare marketing, sales, or community outreach-preferably in home health, hospice, or a related field.
Demonstrated ability to meet or exceed performance goals and key performance indicators (KPIs). This includes:
Referral volume targets
Patient admission and conversion rates
Community engagement metrics
Timely follow-up and documentation standards
Relationship development with referral sources
Strong interpersonal and communication skills, with the ability to engage diverse audiences.
Knowledge of Medicare/Medicaid and other payer sources is a plus.
Knowledge of Hospice eligibility and levels of care is preferred
Knowledge of customer referral source [CRM] management systems for activity tracking
Ability to work independently and collaboratively in a fast-paced environment.

You will empower families to address challenging and/or problematic behavior and to help children/youth aged 5-21 make life-transforming changes. Treatment progress is made through intensive interventions such as skill building, changing unhelpful family interactions, and increasing social support, to name a few.
The pay range for this position is $19.00 - $22.26 per hour based on experience, education, and/or licensure.
Essential Functions:
Provide direct clinical treatment using the I-FAST treatment model and principles. Some principles include leveraging strengths and focusing on the positive, understanding sequences of behavior, and increasing mature behavior.
Conduct a thorough assessment of the client and family: gather information on behaviors of concern and strengths in the family and their ecology to inform conceptualization of the problem behaviors and interactions within the family's ecological context.
Comfort working with a diverse community of clients.
Knowledge of the types of families in the community.
Continuously work to engage the primary caregiver, family members, supports, and community agency staff (school, probation, child welfare) in change-oriented treatment.
Dedicate time to weekly case planning and evaluation of case progress, with ongoing support from your supervisor and team members.
Receive regular training, professional development, supervision, and consultation activities designed to help you acquire extensive clinical skills within the I-FAST treatment model.
Assure, along with fellow clinicians, that clients have access to 24 hours/day, 7 days/week support as needed.

Minimum Requirements:
Experience working with multisystems such as: Court, Child Protective Services, Schools, OhioRise, Family Children First Council
Current license/certification, including LSW, LPC, LISW, or LPCC, is preferred.

Demonstrated a high degree of cultural awareness.
Experience with multi-need individuals and families.
