Primary Duties and Responsibilities
Learn, in a hands-on way, how to navigate and build solutions within the OneStream Platform and gain practical experience with platform architecture and modular design
Assist in organizing and promoting Genesis block development activities, including workshops, demos, and educational sessions.
Help compile and communicate standards, use cases, and best practices for Genesis block development.
Contribute to communication campaigns that inform the OneStream ecosystem about Genesis initiatives, standards, and roadmap updates.
Collaborate with cross-functional teams to align efforts and promote a culture of continuous improvement.
Required Education and Experience
Pursuing a Bachelor's Degree or equivalent. Preferred business, project management, or communication majors.
Strong oral and written communication, presentation, organization, and time management skills.
Outstanding Microsoft Office Skills (MS Outlook, Word, Excel, and PowerPoint).
Preferred Education and Experience
Junior and graduate-level students preferred (graduating between December 2026 and May 2027).
Previous internship experience is nice to have but not necessarily required.

-Deliver plans of care to physician's offices for signature (as appropriate) and ensure timely return of such plans.
-Participate in company-sponsored programs and meetings.
-Performs other duties as assigned by the Administrator/General Manager, Sales Manager or other appropriate supervisory personnel.

* Provide outreach, case management and navigational services to offenders within 90 days of release or those recently released with chronic medical, psychiatric and substance use conditions.
* Collaborate with primary care provider (PCP) and the Behavioral Health Provider post release, to provide health and behavioral interventions that will maximize patient health outcomes.
SPECIFIC DUTIES AND JOB FUCTIONS:
Under supervision of C2C Program Manager, the C2C CommunityHealthWorker is responsible for performing the following essential job functions:
* Provides support, empowerment, education and case management services to offenders pending or recently released chronically ill individuals with co-morbid substance use and mental illness individually and in a group setting.
* Recruits patients by conducting outreach activities in the community, prison, or local jails.
* Conducts assessments of patient's substance use, physical activity, dietary habits, cigarette smoking habits, and social and economic resources.
* Educates clients with chronic illness about evidence-based standards of care and self-management of their chronic illness.
* Educates patients about the health care system and facilitates relationship building between the two.

* Provide direct education or engagement with contacted patients or connect to other resources for education as appropriate based upon initial assessment.
* Assist in maintaining assigned patient panels with covered Health Plans including helping patients change their primary care provider and supporting the Center in removing patients that are not served by the Center from its panels
* For newly assigned patients, coordinate a graduated intensity outreach program to engage patient in care.
* Assist contacted members with completion of assessment and address triggered interventions
* Contact assigned Health Plan members identified as either high or inappropriate utilizers of the emergency department and assist with scheduling follow-up appointment(s) with PCP or other health care staff (Behavioral Health, Dentist) as soon as possible.
