Duties/Responsibilities:
Provide a vital link between local communities and healthcare provider by helping individuals access resources and navigate systems.
Proactively outreach and engage identified individuals in need of services, follow up or social care screening by connecting via phone calls, home visits and/or in-person visits to other settings where patients can be found.

Cayuga Centers is the longest-standing provider of Functional Family Therapist (FFT) in the United States, proud leading the way since becoming New York State's first accredited FFT provider in 2000.
Our FFT program serves youth ages 11-18 and their families. This family-focused therapy operates on the belief that all families have strengths and are doing the best they can at any point. With FFT, the entire family unit works together to prevent out-of-home placement, intervene on behalf of family rebuilding, and unify the family unit. Responsibilities:
Recruit and engage allies, including groups that are disproportionately affected, to implement education campaigns targeting retail tobacco industry marketing with a goal to de-normalize and de-glamorize tobacco use.
Using a variety of communication tools, provide monthly information on the impact of tobacco industry marketing on youth initiation of tobacco use to local coalitions, community members and government decision makers. Provide updates at quarterly coalition meetings.
Collaborate with youth-focused, environmental and health-related organizations to reduce or eliminate tobacco use in outdoor areas. Oversees pet pantry order fulfillment, collaborates with partner organizations, facilitates the network of Little Free Pet Pantries, and represents the program at off-site events.
Communicates with clients requesting the Mary Ellen Crisis Boarding Program. Schedules intakes and returns with the Admissions department. Provides updates and support to clients while their pets are in Lollypop Farm's care.
