POSITION SUMMARY: The Population Health Nurse is responsible for the review and implementation of clinical workflows and processes within the ECMP physician practices to ensure PCMH level 3 recognition. # This role is responsible for the determination of a population of individuals whose disease acuity has been determined to be vulnerable, using a stratification process that incorporates data from available visits, claims, lab, medication, and admissions and discharges. Job purpose Reporting to the Program Manager for Cancer Care Health Equity, the Community Health Worker for Cancer Care (Lung Cancer Screening) is a community ambassador, educator, and navigator for cancer prevention at Fair Haven Community Health Care (FHCHC). The role coordinates FHCHC's patient-facing efforts at preventing cancers of all types, including breast, cervical, colorectal, and lung. The role effectively communicates about cancer prevention in ways that resonate with communities served by FHCHC. Duties and responsibilities The Community Health Worker for Cancer Care (Lung Cancer Screening) will perform direct outreach to patients requiring cancer screening to provide education about the risk factors and the importance of early detection. Typical duties include but are not limited to: Assist patients in navigating the healthcare system, including scheduling appointments, obtaining referrals and navigating insurance coverage Coordinate care between primary care providers, specialists, and other healthcare facilities Establish and maintain relationships with community-based organizations that provide social services, including transportation, food assistance, housing and childcare Screen for and address the social determinants of health that may prevent access to cancer screening among FHCHC patients Collect and maintain accurate patient data, including demographics, screening status and outcomes Contribute to the development and evaluation of program processes and protocols Participate in the development and implementation of program strategies to increase cancer screening rates Collaborate with the marketing team to design and implement social media and other media campaigns for cancer screening education Performs other duties as directed by Qualifications Associate's degree, relevant experience and demonstrated knowledge of communities in New Haven is required.
