The Bilingual CommunityHealthWorker (CHW) is a trusted liaison between patients, the healthcare system, and community resources. This role supports patients who face language, cultural, and social barriers to care by providing education, navigation, advocacy, and connection to services.

Key Responsibilities
Build strong, trusting relationships with patients and families using culturally and linguistically appropriate approaches

Identify and help address social drivers of health such as food insecurity, housing needs, transportation, and utilities

Collaborate with primary care, behavioral health, and care management teams to support whole-person care

SUMMARY: The CommunityHealthWorker is responsible for serving as a liaison/connector between the patient, care team, and the community with a focus on ensuring access to behavioral health and substance use services if needed. This position will help to bridge conversations with patients and remove barriers that prevent them from accessing health and social services.

Primary duties may include, but are not limited to:
Meeting the patient in clinic, facility or at home to help identify social determinants of health impacting patients health and general well-being.

Assists patients in accessing health-related services and community resources, such as accompaniment to specialist appointments and assistance with enrollment forms.
Facilitates communication between all parties (patients, families, colleagues, and community-based organizations) as needed.
Documents interactions with patients and on behalf of patients in medical record.
Attends meetings as required.
Maintain a resource file of available social service agencies and assistance providers in community for use in referring patient in future.

Required Education and Experience
Requires a High School diploma or GED equivalent from an accredited institution.
Proficiency with computers, electronic documentation, and willingness to learn new systems.
2 years of experience in a healthcare, community-based, or social work environment; or any combination of education and experience, which would provide an equivalent background.
Certified CommunityHealthWorker preferred.
Demonstrated understanding of and is part of the community being served.
Ability to read, write, and speak the native language of the population served.
Information Technology skills: Ability to use Laptops, Smartphones, Internet/online application systems, Microsoft Office, etc.
Experience in community work, education, health care, or related field preferred.
Bilingual-English/Spanish preferred
Demonstrated knowledge of navigating health insurance marketplace and/or other insurance and payment options for low income or indigent patients.
Must possess knowledge of a broad range of social service and assistance providers in community.
Demonstrated ability to work as a member of a team and work effectively with community agencies.
Must have valid drivers license and reliable insured transportation as travel is required.

What You'll Do

This position will work in collaboration with other clinical professionals to facilitate positive outcomes for our clients. The LPN maintains accurate and up to date medical documentation and client medication logs in addition to assisting other staff members in maintaining a therapeutic and healthy clinical environment for all clients during their short-term stay at the center.

Youll earn a competitive salary of $23 27/hour depending on skills and experience. This full-time position offers a primary schedule of Night Shift 7p to 7a, 12- hour shifts, with alternating weekends.
The rotations have 8 hours of overtime built in each pay period!

Community Engagement Navigator - Children & Families Specialty Plan DSS (Region 6)

LOCATION: This role is Field Based in Region 6 of North Carolina; candidates must reside within region 6 to be considered. This field-based role enables associates to primarily operate in the field, traveling to client sites or designated locations as their role requires, with occasional office attendance for meetings or training. This approach ensures flexibility, responsiveness to client needs, and direct, hands-on engagement.

Region 6 Includes: Bladen, Brunswick, Carteret, Columbus, Craven, Cumberland, Duplin, Jones, Lenoir, New Hanover, Onslow, Pamlico, Pender, and Sampson counties.
Our ideal candidates will live in Bolivia, Fayetteville, Kinston, Wilmington, or Jacksonville. However, all qualified candidates will be considered within the region.
HOURS: General business hours, Monday through Friday.
TRAVEL: Travels to and remains onsite at assigned DSS agency location several times a week.
The Community Navigator serves as a vital link between the health plan, local Departments of Social Services (DSS), and community stakeholders. This role focuses on removing barriers related to Social Determinants of Health (SDOH) and ensuring members receive the necessary support and services.
How you will make an impact:
* Engage with members who have been identified as possessing significant SDOH barriers.
* Collaborates with members, interdisciplinary teams, and external stakeholders to identify goals and develop care plans that address barriers related to Social Determinants of Health (SDOH), physical and behavioral health needs, and gaps in care..
* Supports members in accessing healthcare services and resources, while assisting with the development, maintenance, and engagement in new relationships.
* Interface with Care Managers to ensure that the individualized plan is aligned with existing or needed case management efforts.
* Act as a liaison between community stakeholders and the health plan.
* Additional expectations to include but not limited to:
* Proficient in maintaining focus during extended periods of sitting and handling multiple tasks in a fast-paced, high-pressure environment; strong verbal and written communication skills, both with virtual and in-person interactions; attentive to details, critical thinker, and a problem-solver; demonstrates empathy and persistence to resolve caller issues completely; comfort and proficiency with digital tools and platforms to enhance productivity and minimize manual efforts.
* Associates in this role are expected to have the ability to multi-task, including handling calls, texts, facsimiles, and electronic queues, while simultaneously taking notes and speaking to customers.
* Associates in this role will have a structured work schedule with occasional overtime or flexibility based on business needs, including the ability to work from the office as necessary.
* Performs other duties as assigned.
Minimum Requirements:
BA/BS degree in Public Health, Social Work, Rehabilitation Counseling or related field and a minimum of 2 years' experience with marketing, job development, job placement or social work; or any combination of education and experience which would provide an equivalent background.
Preferred Skills, Capabilities, and Experiences:
* Familiarity with Vocational Rehabilitation and supporting members with overcoming barriers to employment preferred.
* Experience working with Children, Youth, and Families who are being served by Local Departments of Social Services through Foster Care and Adoptive Assistance programs is strongly preferred.
* Experience serving the children and youth involved in Foster Care and Social Services is strongly preferred.
* The ability to demonstrate strong verbal and written communication skills in both virtual and in-person settings.
* Prior experience in managing the delivery of services to meet the needs of children and youth with complex physical and behavioral health needs is strongly preferred.
* Knowledge of resources, supports, services and opportunities required for safe community living for populations is strongly preferred.
