What You'll Do:
Coordinates and facilitates referrals to Hospice care based on Hospice admission criteria
Assist in the information gathering, education, and form completion aspects of patient intake, admissions, and transition to home
Develop a consultative relationship, in assigned accounts by making effective customer contacts and presentations
Develop and implement marketing and education programs to increase awareness of the company
Qualifications:
Bachelor's Degree in marketing, business administration, or other related fields is preferred. An acceptable combination of experience and/or training may be considered in lieu of formal education.
Minimum of one year of professional sales experience preferred.
Experience in the hospice and health care industry a plus.
Demonstrates good verbal and written communication, and organization skills.
Valid driver's license with an automobile that is insured in accordance with state requirements.
Strong customer service skills and ability to keep good relationships with customers, co-workers, and other business partners.

Services provided by this position include medication delivery and observation, transportation of consumers, social support, and providing support and supervision of consumers served by JMHC variety of programs. CHWs will work closely with PHNs, medical providers, CHP's primary care teams, and other community partners to link individuals in the community to primary care and identified social services. CHWs help patients to access community services specifically to their needs, educate patient on COVID-19 adherence, health initiatives and chronic disease preventions and management, promote healthy habits, secure medical appointment, linking residents to insurance, medical home and transportation to medical visits.

Summary:
Health educators will be responsible for reviewing participants' biometric screening results (total cholesterol, HDL, LDL, triglycerides, glucose, blood pressure, and body composition), as well as providing general information about health and nutrition during an optional 5 to 10 minute one-on-one.
Duties/Responsibilities:
* Review and explain participants' screening results (height, weight, BMI, body fat percentage, waist measurement, blood pressure, total cholesterol, HDL, LDL, triglycerides, and glucose).
* Proficiency in providing brief interventions aimed to help participants to understand what actionable behaviors can influence their health in an appropriate way
* Provide appropriate health recommendations to participants as needed
* Keep records of interactions with screening participants as directed by Labcorp Program Manager
Qualifications and Requirements:
* Registered Dietitian Nutritionist (RDN) with experience in health psychology and motivational interviewing
* Knowledge of HIPPA and OSHA
* Excellent customer service skills and ability to work in a fast-paced environment
* Basic tablet and computer skills
* Must have a reliable form of transportation
* Must be willing and able to pass a criminal background check
* Must be at least 18 or older
Pay Range:
$45 - $50 per hour
