Job Overview
The Community Health Workers (CHW) are trusted members of their community who help address chronic conditions, preventive health care needs, and health-related social needs. CHWs have the skills and experience to understand what at-risk or vulnerable patients are going through and help them get through difficult times. CHWs help patients address the social, medical, and community problems that lead to poor health. The goal is to help patients address real-life obstacles (e.g., social driver of health) that keep them from staying healthy, by supporting them with health care navigation, specialty appointments/referrals, PCP appointments/establishing routine care and preventative health screenings, cultural broker, financial services, and provide culturally appropriate health information on topics related to chronic disease management and prevention. CHWs receive training in Epic & Essette, DHCS requirements, trauma-informed care, motivational interviewing, health care navigation, harm reduction, mental health first aid, domestic/intimate partner violence, advocacy, insurance enrollment, social service/resource applications and referrals, and basic case management.
Minimum Requirements
A High School Diploma or GED is required.
Minimum of 2 years of experience working in health care or community outreach settings.
Bilingual English/Spanish/Mandarin/Cantonese, depending on location preferred.
A minimum requirement of a valid BLS certification or higher, following the American Heart Association (AHA) or the American Red Cross guidelines.

This involves the coordination of medical, behavioral, and social services to address the full range of student needs, focusing on those with complex and chronic conditions. The intention of our services is to help people who have not been able to get the needed help in terms of Housing, Mental Health, Substance Abuse treatment, and linkages to community resources to help minimize recidivism. The role of the ECM CHW is to help create a supportive network of services and resources for the client to prevent reincarceration and support long term permanent housing goals.What You Will Do:
The CHW is a field-based member of the ECM Care Team who has lived experience in the ECM Students' community and serves as the bridge between the ECM Student and the healthcare system.
Focuses support on ECM Students who are difficult to engage and/or who have cultural or linguistic barriers to care via field-based contacts including accompaniment to appointments.
Assists the RN Care Manager, BH Care Manager, and Care Coordinator in panel management by performing delegated activities as assigned.
Assists the ECM Care Team in engagement efforts of eligible ECM Students in the clinic and within the community.
Assists Students in navigating the healthcare system and community resources.
Follows up by phone and in person with eligible ECM Students and students enrolled in ECM.

Responsibilities
The CHW is an integral part of the member's care team and works closely with the Case Manager.
The CHW will lead the Connect For Life program, using Wider Circle's approved curriculum and materials to build trust, promote health, and encourage engagement.
The CHW will plan, promote, and host virtual and in-person events for members, fostering relationships with and among members to support wellness outcomes.
On a regular cadence and as needed, the CHW will reach out to members(by phone and in-person) to check-in with them, identify and address needs, and encourage participation in local events.

Job Description
This position will work with and assist people who use drugs (PWUD) by providing comprehensive harm reduction services for low-income Latinx and Black/African American residing in San Bernardino. Many of our participants will be uninsured, undocumented, homeless/housing insecure, LGBTQ+, reentry, and have substance use disorder (SUD) with co-occurring mental disorder (COD), HIV, and/or vital hepatitis and are in need of support services (outreach, counseling, navigational, and case management services).
Our multi-modality, trauma-informed, peer-driven harm reduction program (HRP) will be delivered by expertly trained staff at San Bernardino Waterman Location, during street-based and in-clinic outreach, and integrated into our continuum of programs that target similar populations. Services will include overdose prevention education/naloxone distribution, syringe services, support services, and linkage to comprehensive services that address medical, behavioral health, and other social determinants of health-driven needs. Annual numbers of individuals to be served are minimum of 300 unduplicated participants. Our overarching goal to reduce the impact of drug-related harm in San Bernardino through expanding access to comprehensive harm reduction services. As a Harm Reduction Peer Community Health Worker, shared lived experiences with potential clients is strongly desired.

Position Summary

Position Responsibilities
Job Duties (may include but are not limited to):
Assist lead to provide community transportation and para-transit dispatching and routing services to seniors
Serve as the assistant to primary point of contact in working with supervisory staff and customers in resolving customer issues and transit concerns related to dispatch services: 1) answer and properly respond to all email, facsimile and telephone calls for transportation reservations, cancellations, ride check status, service inquiries, and general information requests. 2) Use appropriate translation services for non-English speaking clients
Take reservations, handle cancellations and ride check status during the hours of 6:30 A.M. to 6:30 P.M. on Monday through Friday. Ensure that average hold times on ride check calls are maintained at no more than two (5) minutes. During periods when reservations staff are busy and during non-office hours, cancellations may also be taken by voice mail, email, or fax. Dispatching staff are on-duty from the time the vehicle is in service unit the last vehicle is out of service. Ensure that the Office on Aging “no stranded passengers” policy is enforced.
Assist lead, review, coordinate and adjust all vehicle itineraries prior to the service day, initiate call-backs of passengers as needed to adjust scheduled services, monitor and coordinate service resources to maximize system efficiency and performance, track cancellations and no-shows.
Assist drivers while they are in service to carry out the assigned trips on time, providing address assistance and telephoning passengers as needed. Monitor the performance of scheduled trips, reassigning trips and/or adjusting the number of vehicles in service as needed to ensure on-time performance in the most efficient manner.
Dispatching techniques for communicating and receiving information using computer based navigation tools. Knowledge of streets, landmarks, and geography of the County of Orange.
Understand the organization and operation of the Transportation Department of ABRAZAR and of outside agencies as necessary to assume assigned responsibilities.
Create and maintain a computer based schedule for dispatching services.
Respond in a courteous manner to requests and inquiries from the general public. Effectively handle difficult and sensitive citizen inquiries and complaints. Understand and follow oral and written instructions.
Operate office equipment including two-way communication devices, computers and supporting word processing, spreadsheet, and database applications.
Adapt to changing technologies and learn functionality of new equipment and systems. Perform a wide variety of general clerical duties.
Maintain accurate and complete records of transportation and paratransit services. Communicate clearly and concisely, both orally and in writing.
Establish and maintain effective working relationships with contacts in the course of the workday.

Responsibilities
The CHW is an integral part of the member's care team and works closely with the Case Manager.
The CHW will lead the Connect For Life program, using Wider Circle's approved curriculum and materials to build trust, promote health, and encourage engagement.
The CHW will plan, promote, and host virtual and in-person events for members, fostering relationships with and among members to support wellness outcomes.
On a regular cadence and as needed, the CHW will reach out to members(by phone and in-person) to check-in with them, identify and address needs, and encourage participation in local events.
As needed, the CHW will conduct home visits to support members, provide health and social support resources, and encourage participation in the Enhance Care Management program compliance with the care plan.
