LOCATION: This role is Field Based in Region 6 of North Carolina; candidates must reside within region 6 to be considered. The Community Navigator serves as a vital link between the health plan, local Departments of Social Services (DSS), and community stakeholders. This role focuses on removing barriers related to Social Determinants of Health (SDOH) and ensuring members receive the necessary support and services.
How you will make an impact:
* Engage with members who have been identified as possessing significant SDOH barriers.
* Collaborates with members, interdisciplinary teams, and external stakeholders to identify goals and develop care plans that address barriers related to Social Determinants of Health (SDOH), physical and behavioral health needs, and gaps in care..
* Supports members in accessing healthcare services and resources, while assisting with the development, maintenance, and engagement in new relationships.
* Interface with Care Managers to ensure that the individualized plan is aligned with existing or needed case management efforts.
* Act as a liaison between community stakeholders and the health plan.

Minimum Requirements:
BA/BS degree in Public Health, Social Work, Rehabilitation Counseling or related field and a minimum of 2 years' experience with marketing, job development, job placement or social work; or any combination of education and experience which would provide an equivalent background.
Preferred Skills, Capabilities, and Experiences:
* Familiarity with Vocational Rehabilitation and supporting members with overcoming barriers to employment preferred.
* Experience working with Children, Youth, and Families who are being served by Local Departments of Social Services through Foster Care and Adoptive Assistance programs is strongly preferred.
* Experience serving the children and youth involved in Foster Care and Social Services is strongly preferred.
* The ability to demonstrate strong verbal and written communication skills in both virtual and in-person settings.
* Prior experience in managing the delivery of services to meet the needs of children and youth with complex physical and behavioral health needs is strongly preferred.

* Serve as the initial customer service contact for the Inspections and Permitting Division for any development stakeholder with questions or concerns regarding the development process.
* Serve as liaison between the City and the business community, developers, architects, engineers, landowners, attorneys, citizens and contractors who are conducting development related business.
* Consult with the general public, contractors, design professionals, homeowners, elected officials and other staff regarding interpretations of City ordinances, state building codes, development plan reviews, application processes and permitting and inspections procedures.
* Troubleshoot issues and work to find potential solutions to concerns related to City regulations, staff decisions and processes in an effort to resolve issues while serving the public interest.
* Discuss compliance violation notices and mediate unresolved matters between City staff and external customers, confirming requirements are presented in an understandable manner; ensure feedback is provided frequently and efficiently; maintain records of the information provided for possible legal actions.
* Communicate with various individuals and groups about policies, procedures, and complex issues; help stakeholders navigate various development and construction processes in a customer friendly and professional manner.
* Coordinate and facilitate discovery meetings with relevant staff from appropriate City departments and outside agencies; provide efficient and effective responses to customer concerns in a timely manner.
* Drive to meetings and inspection sites, and perform inspection and plan review activities; maintain and develop comprehensive outcome tracking for assigned projects and City communications associated with the assigned projects.
* Review applications from the public to ensure necessary information is provided. Assist in providing appropriate information related to the completeness of the application and explain the application process.
* Research computer records and files regarding a variety of building related issues. Maintain files and reports regarding plan review activities and findings; prepare daily reports of plan review activities performed and results.

Experience:
Two years of experience in the building permit process including building, electrical, mechanical and plumbing trades; with general knowledge of zoning, site plan review, utility plan approval, stormwater approval or NCDOT is desirable.
Training:
Equivalent to completion of the twelfth grade supplemented by specialized training in all four trades (building, electrical, mechanical or plumbing) or a degree/certificate in construction management, engineering, architecture, planning or public administration.
LICENSING & CERTIFICATIONS:
Required:
Possession of an appropriate valid driver's license and ability to maintain insurability. Possession of or ability to obtain within two years a North Carolina Standard Level I certifications in Building, Electrical, Mechanical, or Plumbing.
Preferred:
North Carolina Inspection Level II certification in Building, Electrical, Mechanical and Plumbing.
From the time of closing, the selection process is anticipated to last approximately 4 - 6 weeks. The process will consist of a panel interview with the selected candidate being subject to a pre-employment drug screen, background check and driving history check.

* Engage with members who have been identified as possessing significant SDOH barriers.
* Collaborates with members, interdisciplinary teams, and external stakeholders to identify goals and develop care plans that address barriers related to Social Determinants of Health (SDOH), physical and behavioral health needs, and gaps in care..
* Supports members in accessing healthcare services and resources, while assisting with the development, maintenance, and engagement in new relationships.
* Interface with Care Managers to ensure that the individualized plan is aligned with existing or needed case management efforts.
* Act as a liaison between community stakeholders and the health plan.

Minimum Requirements:
BA/BS degree in Public Health, Social Work, Rehabilitation Counseling or related field and a minimum of 2 years' experience with marketing, job development, job placement or social work; or any combination of education and experience which would provide an equivalent background.
Preferred Skills, Capabilities, and Experiences:
* Familiarity with Vocational Rehabilitation and supporting members with overcoming barriers to employment preferred.
* Experience working with Children, Youth, and Families who are being served by Local Departments of Social Services through Foster Care and Adoptive Assistance programs is strongly preferred.
* Experience serving the children and youth involved in Foster Care and Social Services is strongly preferred.
* The ability to demonstrate strong verbal and written communication skills in both virtual and in-person settings.
